Endoscopic surgery: A narrative review of history, safety, and future perspectives, with lessons for transcanal endoscopic ear surgery (TEES).

The history of endoscopic surgery since the laparoscopic revolution offers critical lessons for the safe development of transcanal endoscopic ear surgery (TEES). Endoscopy evolved through three interdependent technological domains-illumination, light transmission, and imaging. Within otolaryngology, endoscopic sinus surgery (ESS) was established by the 1990s, yet TEES did not achieve mainstream adoption until approximately 2010. This 25-year interval represents an unusual accumulation of translatable experience from other specialties. TEES therefore entered clinical practice with access to doc

Electroacupuncture for the prevention of perioperative neurocognitive disorder in elderly patients undergoing general anesthesia: a systematic review and meta-analysis.

BACKGROUND: Perioperative neurocognitive disorder (PND) is a common complication following major surgery under general anesthesia, particularly among elderly patients, and adversely impacts postoperative recovery and quality of life. Although electroacupuncture (EA) has shown potential in preventing PND, conclusive evidence remains lacking. This study aimed to evaluate the effectiveness and safety of perioperative EA intervention for preventing PND in elderly patients undergoing general anesthesia surgery. METHODS: We systematically searched eight electronic databases [PubMed, Embase, Web of

Spinal Anesthesia After Thoracolumbar Fixation for Scoliosis: A Case Report Emphasizing the Role of Chest X-ray Reassessment.

Spinal anesthesia (SA) is widely favored for lower limb orthopedic surgeries due to its rapid onset, effectiveness, and lower risk compared to general anesthesia (GA), particularly in patients with comorbidities. Nevertheless, prior spinal surgery presents significant challenges to SA because of deformed anatomy, scarring, and altered surface landmarks. In this article, we report successful landmark-based SA using radiographic reassessment, without ultrasound, in an elderly patient with multiple comorbidities and previous thoracolumbar fixation for scoliosis who was scheduled for elective tota

The short video lecture for robotic bilateral axillo-breast approach to lateral neck lymph node dissection.

Lateral neck lymph node dissection (LND) along with total thyroidectomy is the standard treatment for thyroid cancer patients with metastases to the lateral neck lymph nodes. In general, lateral neck LND removes lymph nodes located at levels II to V ipsilateral to the thyroid cancer and preserves the spinal accessory nerve, internal jugular vein, and sternomastoid muscle during surgery. This video article was written to introduce the robotic bilateral axillo-breast approach for lateral neck LND and to describe the surgical method.

A 70-Year-Old Woman With Long-Term Nonresolving Pneumonia.

A 70-year-old woman who had received a diagnosis of pneumonia in the right lower lobe was treated with antibiotics at a general practitioner's clinic 9 months earlier. Her pneumonia had improved, but the cough and lung infiltrates persisted for > 6 months, so the patient was referred to our hospital. She had undergone surgery for breast cancer 30 years earlier but had no other medical history. She was not taking any medications and had no history of smoking, including passive smoking.

Intravenous Lipid Emulsion for the Treatment of Perioperative Cocaine Intoxication.

Symptomatic cocaine intoxication in the preoperative period is a potentially life-threatening condition, especially before emergent surgery. The anesthesiologist is faced with a dilemma where the patient is deemed unsafe for induction of general anesthesia but also in need of immediate surgical intervention. Cocaine is a local anesthetic and, as such, has been proposed to respond to lipid emulsion treatment as other local anesthetics would. We present a case supporting this statement and review the relevant published literature on the topic.

Dysphagia and an aberrant subclavian artery: more than just a coincidence.

Aberrant subclavian artery (arteria lusoria) is the most common congenital malformation of the aortic arch in the general population. As patients are more often asymptomatic, indications for correcting this abnormality, even in those having dysphagia, are very vague. While presenting a case of a 10-year-old girl who suffered from dysphagia lusoria for 2 years before being cured by a simple surgical translocation, we review the literature and debate various aspects of its management from diagnosis to surgery and cosmesis.

Airway Management Through a Facial Defect Resulting From Noma (Orofacial Gangrene): A Case Report.

At a rural district hospital in Burundi, a 3-year-old severely malnourished girl with Noma presented to the operating room for placement of a gastrostomy tube. The child had a large left-sided facial defect as well as trismus. After induction of general anesthesia, the anesthesia provider was unable to open her mouth. Given that the hospital has no fiberoptic bronchoscope, direct laryngoscopy and intubation were performed directly through her facial defect. After securing the airway, the surgery proceeded without difficulty.

[Giant prostatic adenoma].

We here report the case of a 75 year old patient, with lower urinary tract obstructive syndrome such as dysuria, nocturnal pollakiuria and dribbling. Clinical examination showed good general condition by examining the rectum with the finger, soft, painless prostate increased in volume with obliteration of the median sulcus. PSA level was 2,1ng/ml, prostatic weight was 240 g on ultrasound. The patient underwent surgery: transvesical adenomectomy with extraction of a giant adenoma weighing 246 g. Histologic examination showed adenomyofibroma.

Anesthetic management of an adult patient with Rett syndrome and limited mouth opening -A case report-.

Rett syndrome is a neurological disease that occurs only in females and it manifests with mental retardation, seizures, movement disorders, autistic behavior and abnormal breathing. A 19-year-old female with Rett syndrome underwent ophthalmologic surgery under general anesthesia at our institution. Airway control was difficult due to her limited mouth opening. We recommend that anesthesiologists should have proper knowledge about this disease and the patients to avoid the complications and problems that can be encountered during the perioperative period.

Focal cerebral ischemia after surgery in the "beach chair" position: the role of a congenital variation of circle of Willis anatomy.

A 50-year-old man underwent shoulder surgery in the beach chair position. His mean arterial blood pressure at arm level was approximately 65 mm Hg. Postoperatively, there was delayed awakening and a right hemiparesis. Radiologic evaluation revealed a congenital asymmetry of the circle of Willis that resulted in limited collateral flow to the left anterior and middle cerebral artery distributions. Similar anatomical variations are relatively common in the general population and may render some patients relatively and unpredictably more vulnerable to hypotension.

Acute pulmonary edema and airway hemorrhage in a goat during sevoflurane anesthesia.

A goat was scheduled for experimental surgery under general anesthesia. The first attempt of performing endotracheal intubation failed and provoked laryngeal spasm. After repeated succesful intubation of inhalation anesthesia was delivered in high concentrations of sevoflurane. Suddenly hypertension and tachycardia were observed, followed by foamy airway secretion and then severe airway hemorrhage. The authors hypothesize that laryngeal spasm provoked respiratory distress and pulmonary edema. The delivered high concentrations of sevoflurane probably enhanced a hyperadrenergic response, predisp

Cholecystectomy in the presence of a large patent foramen ovale: laparoscopic or open?

An obese patient with cholelithiasis and acute cholecystitis was scheduled for an emergency laparoscopic cholecystectomy. On preoperative workup, a 5-year-old echocardiogram showed a large patent foramen ovale (PFO) with a right-to-left shunt that was open at rest. Noting both the benefits of laparoscopic surgery and the substantial incidence of PFO in the general population (10% to 30%), the attending and consulting surgeons reasoned that the benefits of laparoscopy greatly exceeded an immeasurably small risk of paradoxical emboli.

Local anesthesia: A feasible option for pediatric frontalis sling surgery.

The purpose of this study was to demonstrate the suitability of local anesthesia in the pediatric age group for oculoplastic procedures. The authors present a case of frontalis sling surgery performed under local anesthesia in a 15-year-old boy with bilateral congenital ptosis. No significant technical difficulties were encountered during the procedure. Optimal intraoperative eyelid placement was facilitated by the patient's comfort and cooperation. For some selected children, local anesthesia is a good alternative to general anesthesia to obtain the best outcome.

Skin integrity in critically ill obese patients.

Almost one-third of intensive care unit (ICU) patients are obese. Obese patients pose a unique challenge for preventing skin breakdown, healing wounds, and preventing complications of surgery and prolonged immobility. Yet little research to date has been done to study the effects of obesity on skin integrity and wound healing in this patient population. This article discusses common skin conditions and wound complications associated with obese patients and general treatment guidelines as they pertain to the critically ill patient.

[Mediastinal parathyroid cystadenoma--a rare cause of primary hyperparathyroidism].

Parathyroid cysts located in the mediastinum are rare. They may be non-functioning or associated with primary hyperparathyroidism (PHPT). We present a patient with persistent PHPT despite previous parathyroid surgery. Parathyroid scintigraphy with 99mTc sestamibi showed no focus with radioactivity retention, but MRI revealed a large parathyroid cystadenoma in the mediastinum, which was successfully removed. In general, MRI is not the first choice for parathyroid imaging, but when the adenoma is localized at ectopic sites, MRI is a good imaging modality.

[Surgical management of ovarian ectopic pregnancy].

Ovarian pregnancies represent a small fraction of ectopic gestations. They are especially feared due to their life-threatening intraabdominal hemorrhage. Pre- and intraoperative diagnosis is difficult. Pathogenetically ovarian pregnancy arises from the retention of a fertilized ovum in the ovary. General guidelines for the medical management are missing. In this case report, we diagnosed a subacute ruptured ovarian pregnancy during surgery and performed a partial ovarectomy via operative laparoscopy. Other therapeutic options in managing ovarian ectopic pregnancy will be discussed.

[Anesthetic management for gastrectomy of a patient after coronary artery bypass graft with gastroepiploic artery].

We report anesthetic management of an 80-year-old man who had received coronary artery bypass graft with the gastroepiploic artery 5 years before the present gastrectomy. Preoperative angiography did not show patency of the grafted gastroepiploic artery. He underwent partial distal gastrectomy under general anesthesia with isoflurane in oxygen-enriched air combined with epidural block. The gastroepiploic artery was severed after temporary clamping which did not produce evidence of myocardial ischemia. There was no postoperative cardiovascular event after the surgery.
