Comparison of first and second heart sounds after mechanical heart valve replacement.

In this article, the spectral features of first heart sounds (S1) and second heart sounds (S2), which comprise the mechanical heart valve sounds obtained after aortic valve replacement (AVR) and mitral valve replacement (MVR), are compared to find out the effect of mechanical heart valve replacement and recording area on S1 and S2. For this aim, the Welch method and the autoregressive (AR) method are applied on the S1 and S2 taken from 66 recordings of 8 patients with AVR and 98 recordings from 11 patients with MVR, thereby yielding power spectrum of the heart sounds. Three features relating t

[Valvular heart disease associated with coronary artery disease].

Nowadays, age-related degenerative etiologies have largely replaced the rheumatic ones and as a natural result of this etiologic change, coronary artery disease has become associated with valvular heart disease to a greater extent. Degenerative aortic valve disease has an important pathophysiological similarity to atherosclerosis and is the leader in this association. There is a general consensus that severely stenotic aortic valve should be replaced during bypass surgery for severe coronary artery disease. For moderate degree aortic stenosis, aortic valve replacement is usually performed duri

Meta-analysis of clinical outcomes following surgical mitral valve repair or replacement.

Although the use of mitral valve surgery has been successful at alleviating mitral valve disease, published studies on either replacement or repair have yielded mixed clinical outcomes regarding differences between repair and replacement. Meta-analysis of various outcomes from 29 published studies was conducted. Studies were separated into four groups by etiology of disease: ischemic; degenerative/myxomatous; rheumatic and mixed. The summary odds ratio for early mortality, comparing replacement to repair, was 2.24 (1.78-2.80), while the summary total survival hazard ratio was 1.58 (1.41-1.78),

Detection of prosthetic valve strands by transesophageal echocardiography: clinical significance in patients with suspected cardiac source of embolism.

OBJECTIVES: The purpose of this study was to determine how frequently prosthetic valve strands are associated with prosthetic mitral and aortic valves, as detected by transesophageal echocardiography, and to assess their significance in relation to clinical cardioembolic events. BACKGROUND: Strands attached to prosthetic mitral valves are a recently described finding of uncertain clinical significance. There are no reports of strands attached to aortic valve prostheses, and data are limited concerning the relation of valvular strands to cardioembolic events. METHODS: We identified all transe

Temporal changes in the causes of aortic stenosis: a surgical pathologic study of 646 cases.

Among 646 patients with pure aortic stenosis who underwent valve replacement at our institution between 1981 and 1985, the three most frequent causes were calcification of congenitally bicuspid aortic valves (38%), degenerative (senile) calcification of tricuspid aortic valves (33%), and postinflammatory (presumably rheumatic) calcification and fibrosis (24%). Among the 324 patients younger than 70 years of age, calcified bicuspid valves were observed in 50%. In contrast, among 322 patients 70 years of age or older, degenerative calcification accounted for 48% of the stenotic aortic valves. Du

Rheumatoid arthritis with severe aortic insufficiency and prolapse of the mitral valve.

The valvular lesions caused by rheumatoid arthritis have been known for a long time. The valves may be affected by unspecific inflammatory changes or, specifically, by rheumatoid granulomas identical with subcutaneous nodules that lead to the malfunctioning of the aortic or the mitral valve. We present a patient affected by a classical seropositive rheumatoid arthritis accompanied by a severe aortic insufficiency and a prolapse of the mitral valve with rheumatoid involvement of the histologic granulomatous type, in whom both valves were replaced in two phases. The hemodynamic results were good

Cardiac valvular replacement in carcinoid heart disease.

A woman with tricuspid regurgitation due to carcinoid heart disease underwent successful tricuspid valvular replacement. This was performed as soon as the patient became symptomatic, enabling her to return to a normal life untroubled by slowly growing liver metastases. Few early valvular replacements have been undertaken in this disease. This case report demonstrates its usefulness.

Surgical treatment of the dilated aortic root in a child with Marfan's syndrome.

Life-threatening cardiovascular manifestations of Marfan's syndrome are rare in young children. The authors report the case of a 4 1/2-year-old girl with Marfan's syndrome who had a large aneurysm of the ascending aortic valve replaced and both coronary arteries implanted into a prosthetic graft (Bentall's procedure). Other published reports of children with aortic aneurysms due to Marfan's syndrome are reviewed.

Intravascular haemolysis after valve replacement: comparative study between Starr-Edwards (ball valve) and Björk-Shiley (disc valve) prosthesis.

Seventy-four patients with single prosthetic valves (Björk-Shiley or Starr-Edwards) in the mitral or aortic position and 18 controls with rheumatic valvar heart disease were investigated for evidence of intravascular haemolysis. Serum lactate dehydrogenase (LDH) was used as the most sensitive indicator of haemolysis. Raised concentrations were found in a third of 39 patients with Björk-Shiley prostheses (mean 281 IU/l) and in all 35 patients with Starr-Edwards prostheses (mean 859 IU/l. Values were considerably higher in patients with Starr-Edwards prostheses and particularly in those with aor

Pulmonary valve replacement. Report of an operation performed for calcific pulmonic stenosis associated with total anomalous pulmonary venous drainage and atrial septal defect.

Calcification of the pulmonic valve is an uncommon lesion that is usually associated with other congenital cardiac defects. Prior attempts at valvotomy or partial excision of the puolmonic valve have carried a high mortality rate. Pulmonary valve replacement itself is an equally uncommon surgical procedure and, in the past, has been performed only during reconstruction of the right ventricular outflow tract, when the pulmonic valve itself had to be utilized as an aortic prosthesis, and in one patient in whom pulmonary regurgitation following valvotomy was not well tolerated. A case of calcific

Minimally invasive pulmonary valve replacement via left anterior mini-thoracotomy after Tetralogy of Fallot repair: a five-patient case series.

BACKGROUND: Pulmonary regurgitation is a common long-term consequence after Tetralogy of Fallot (TOF) repair and often leads to right ventricular dilatation requiring pulmonary valve replacement (PVR). Redo sternotomy carries risks including cardiac injury and bleeding. Minimally invasive alternatives aim to reduce surgical trauma while maintaining procedural safety. CASE PRESENTATION: We report five consecutive patients (aged 9-44 years) with severe pulmonary regurgitation and right ventricular dilatation who underwent minimally invasive PVR via a left anterior mini-thoracotomy (LAMT). All p

Apical Access Management in Transapical Transcatheter Mitral Valve Replacement.

BACKGROUND: The role of the surgical technique and anatomy in transapical mitral valve replacement are scarcely investigated. METHODS: Computed tomography scans, surgical reports and planning slides of 127 patients undergoing transapical mitral valve replacement with the Tendyne valve system (Abbott Vascular) at 15 centers, participating in a European observational study, were retrospectively analyzed and compared between patients with (cohort A) and without (cohort B) apical access complications (AACs). RESULTS: A total of 8 (6.3%) AACs were recorded, of which 7 of 8 were observed in the fi

Transcatheter tricuspid valve intervention in the presence of a cardiac implantable electronic device; state of the art review.

A large proportion of patients referred for transcatheter tricuspid valve intervention (TTVI) will have the presence of a cardiac implantable electronic device (CIED). In such patients, surgical correction of tricuspid regurgitation (TR) is associated with high rates of morbidity and mortality. Transvenous lead extraction (TLE) could potentially ameliorate CIED-induced TR; however, it carries inherent risks and frequently does not result in TR improvement. As multiple TTVI devices are in trial to gain regulatory approval, understanding which therapy is most appropriate among patients with a CI
