BACKGROUND: There are no specific ocular findings for infectious endocarditis. We report a case of infectious endocarditis detected by combining orbital color Doppler imaging (CDI) and ophthalmological findings.
CASE: A 47-year-old man suffered from lumbar pain and low-grade fever. He had undergone heart surgery for a ventricular septal defect and received blood transfusions 20 years earlier, and he had started interferon therapy for chronic hepatitis C 8 months previously. Systemic examinations suggested either collagen disease, malignant lymphoma, or infectious disease.
OBSERVATIONS: The p…

PURPOSE: To study a new surgical option of primary placement of a hydroxyapatite-coated sleeve into the Bioceramic implant during enucleation or evisceration.
DESIGN: Retrospective, observational case series.
METHODS: A standard enucleation or evisceration was performed, followed by the preplacement of a hydroxyapatite-coated sleeve into the Bioceramic implant. Care must be taken to ensure the sleeve has been positioned centrally when the implant is put inside the orbital socket. Complications such as sleeve exposure, Bioceramic implant exposure, and infection were closely observed.

We report an unusual case of orbital cellulitis in a cat. Drainage via the pterygopalatine fossa and medical treatment were performed. Failure of resolution and loss of ocular structure and visual function with a poor prognosis led to enucleation. During surgery, plant material and purulent exudate were present in the orbit. Histopathology showed an intraocular foreign body of grass that had migrated through the sclera causing a suprachoroidal abscess with choroidal and retinal detachment.

Frameless neuronavigation has been increasingly used in intracranial and spinal neurosurgery. However, the application of neuronavigation in orbital surgery has not yet been reported. The purpose of this study was to evaluate whether frameless navigation in the orbit provides sufficient clinical application accuracy and thus a useful tool in orbital surgery. A frameless infrared-based neuronavigation system (VectorVision, BrainLAB, Heimstetten, Germany) was used in the microsurgical removal of 11 orbital tumors (five cavernomas, three lymphomas, two dermoid tumors, and one rhabdomyosarcoma). T…

PURPOSE: To evaluate microscopically the signs of inflammatory reaction due to the fixation of an orbitary implant (magnet, covered by gold).
METHODS: An experimental study was performed in 54 adult female rabbits, divided into 3 groups of eighteen with 3 subgroups of 6; an evaluation at 1, 3 and 6 postoperative weeks was performed. An implant (magnet, covered by gold) was fixed in the inferior orbital rim using biologic glue, screw or unabsorbable suture (Mersilene 5.0). The opposite orbits (without implants) represented the control group.
RESULTS: Microscopic signs of inflammatory reaction…

The rare case of a penetrating cranio-orbital injury and the surgical treatment is presented. A 38-year-old woman was brought to the Emergency Unit of the University of Leipzig Hospital after suffering a severe craniocerebral injury from a broken cut-off wheel. A computed tomography (CT) scan demonstrated the entrance of the cut-off wheel with extension from the left sinus maxillaris and frontalis through the median part of the left-sided orbit to the anterior skull base. After removing the cut-off wheel and metal splinters, the neurosurgeon performed an osteoplastic bifrontobasal trepanation …

PURPOSE: We describe the first reported cases of cerebral vasospasm, ischemia, and infarct after orbital decompression surgery for Graves disease-related ophthalmopathy.
METHODS: We present two patients who had inadvertent penetration of the dura mater during orbital decompression surgery. One case was recognized at surgery; the other was not. All patient data were gathered and relayed in accordance with state and U.S. laws.
RESULTS: Postoperative cerebrospinal fluid leak, radiographic evidence of subarachnoid hemorrhage, and severe neurologic morbidity occurred in each case. Cerebral ischem…

We describe a case of an orbital floor fracture resulting in obstruction of the maxillary sinus ostium by herniated orbital contents. Our patient had delayed enophthalmos in a manner similar to silent sinus syndrome or imploding antrum syndrome. Obstruction of the maxillary sinus ostium by orbital contents carries the potential for delayed-onset enophthalmos and therefore suggests an additional indication for orbital floor fracture repair.

We report a rare case of bilateral orbital metastases as the presentation in a 63-year-old woman. Biopsy of a diffusely infiltrated medial rectus muscle suggested metastatic adenocarcinoma. Investigation revealed a palpable mass of the right breast not shown on mammography or sonography. Invasive lobular carcinoma was found at core-needle biopsy with histological features identical to those of the orbital lesion. Metastases to the extraocular muscles are uncommon, particularly as the initial abnormality in the absence of disseminated disease.

PURPOSE: To review the clinical features and treatment strategies for microphthalmic globes with orbital cyst.
METHODS: : The clinical records of 23 patients treated for microphthalmos associated with orbital cysts were reviewed retrospectively.
RESULTS: Most cases of congenital microphthalmos with orbital cysts were first noted at birth, and all were confirmed by means of computed tomography and/or ultrasonography. Six of the 23 cases were bilateral. Sixty-seven percent of bilateral and 29% of unilateral cases also had other congenital malformations. Mild microphthalmos was noted in 3 orbit…

A 9-month-old infant presented with orbital cellulitis and recent discharge from a hair-bearing pit above the eyebrow. Orbital imaging demonstrated a tubular lesion and sinus tract extending from a hypoplastic sphenoid wing, through the lateral orbit, to the skin surface. Complete excision of the dermoid cyst and sinus tract was performed through an eyelid crease approach.

BACKGROUND: The management for blow-out fractures is controversial. Some studies suggest early surgical treatment, others a conservative attitude.
PATIENTS AND METHODS: We studied retrospectively the data of patients with blow-out fracture, referred to our Neuro-Ophthalmology and Strabismus Clinic from July 1993-May 2000. Out of 48 patients, 22 were operated. We evaluated all 12 patients who underwent pre- and postoperative orthoptic examinations.
RESULTS: The patients' age at the time of the accident was 7.8 to 67.8 years (median 25.3). The delay between accident and operation was 2 to 91 d
