Fifty consecutive patients over the age of 15 years undergoing tonsillectomy had one tonsillar fossa obliterated by 2/0 Polydioxanone (PDS II) suture. The opposite side was used as a control. Pain was assessed on a visual analogue scale from the first to tenth post-operative day. Although on the initial post-operative days the pain was more on the sutured side, 41 patients subsequently experienced significant pain relief on that side (P = 0.0001). No complications were encountered due to the tonsillar fossa obliteration. On the 10 day review, the 41 patients indicated in their questionnaire th

An elongated styloid process may be a source of craniofacial and cervical pain. The syndrome is characterized by a variety of symptoms including difficulty in swallowing, sore throat, glossodynia, headache and hemifacial pain. Sometimes, the pain is localized or radiates to the jaw and ear and may simulate pain of dental origin. Diagnosis is readily made by radiographic examination and palpating the tonsillar fossa. The only effective treatment is surgical shortening of the styloid process. Three patients, two women and a man, underwent surgery in our department for symptomatic elongation of t

The Authors examined 515 children with abdominal pain in the outpatient clinics for a period of two years. The pain was frequently periumbilical or mid epigastric with a history of more than two months. Persistent lactose malabsorption was found in 252 children (48,9%), which justifies the use of Breath Hydrogen Test as a first diagnostic procedure for assessing recurrent abdominal pain. Putative pathogens were identified in the stool specimens of 21 patients. Oesophagitis was diagnosed in 18 children and duodenal ulcer in one. 91 patients (less than 6 years old) improved after successful trea

Achalasia cardia is a motility disorder of the esophagus characterized by a lack of esophageal peristalsis and failure of lower esophageal sphincter opening during swallowing. Symptomatic patients present with a long history of difficulty swallowing, along with chest pain and vomiting. Two male patients, 55 and 24 years old, with proven achalasia cardia by high-resolution esophageal manometry (HRM), underwent a successful peroral endoscopic myotomy (POEM) procedure at our center. This was the first time this procedure was done in Nepal. In this case report, we have demonstrated that a novel th

Dorsal surgical procedures have a well-established role in the treatment of both radiculopathy and myelopathy caused by cervical spondylosis. Laminectomy and laminoplasty procedures can both lead to postoperative kyphosis because of the removal of the dorsal supporting structures of the neck. Minimally invasive or minimal-access spinal surgery procedures of the dorsal cervical spine are evolving techniques with the goal of decompressing the neural structures with minimal disruption of the dorsal supporting structures. We think that this will lead to less postoperative pain and a decreased inci

The postoperative motility dysfunction (POI) is a frequent secondary complication of surgical procedures. The POI results from a number of simultaneous factors of both local and systemic nature. Activation of inhibitory neural reflexes and a mediator response to the surgical procedure are among the main ones. Neurohumoral response of the organism to painful stimuli and the effect of the opioid medication used during the perioperative period, are the modulating factors. The intraabdominal procedure with manipulation with the intestine results in activation of the mediator response, followed by

Under the hypothesis that the surgical management of diffuse esophageal spasm requires the elimination or reduction of episodes of dysphagia and chest pain and prevention of postoperative gastroesophageal reflux, long esophageal myotomy and fundoplication had been performed. However, there have been some cases with unsatisfactory results. We describe herein a new surgical procedure of long myotomy of the esophagus and gastric cardia with a complete fundic patch operation for the patient with diffuse esophageal spasm. The advantages of this procedure are to preserve the separation of each myoto

A total of 117 patients underwent 124 functional stereotactic procedures with the Leksell Gamma Knife to treat trigeminal neuralgia, movement disorders and chronic pain. Target identification was performed with stereotactic magnetic resonance imaging alone without electrophysiologic localization. Clinical observation of these patients over 1-41 months indicates that the efficacy of the procedure is comparable with that seen with open stereotactic procedures where electrophysiological localization is used. Using current techniques, the complications of functional neurosurgery with the Gamma Kni

A 43-year-old female was hospitalized on October 25, 1993, due to severe back pain. She was diagnosed with annulo aortic ectasis, Stanford type-A acute aortic dissection and pectus excavatum caused by Marfan's syndrome. On November 30, 1993, single-stage surgery was performed, including modified Bentall procedure, aortic arch replacement, elephant trunk method and sternoplasty. Concerning the procedure for pectus excavatum, sternoplasty was selected because we thought that sternoplasty could endure the long procedure which would cause infection compared with sternal turnover. Postoperative cou

Appropriate sedation of pediatric oncology patients during painful diagnostic and therapeutic procedures is a recognized problem. At the Memorial Sloan-Kettering Cancer Center the pediatric oncologists and anesthesiologists have developed a cooperative program that has provided safe and effective sedation of outpatients. The experience with 100 consecutive outpatients is reported. Patients ranged from 3 months to 17 years of age. Procedures included lumbar puncture and/or intrathecal chemotherapy, multiple bone marrow aspirations, bone marrow biopsies, and removal of indwelling central vein ca

BACKGROUND: Both stereotactic radiosurgery (SRS) and percutaneous glycerol rhizotomy are excellent options to treat TN in patients unable to proceed with microvascular decompression. However, the influence of prior SRS on pain outcomes following rhizotomy is not well understood.
METHODS: We retrospectively reviewed all patients undergoing percutaneous rhizotomy at our institution from 2011 to 2022. Only patients undergoing percutaneous glycerol rhizotomy following SRS (SRS-rhizotomy) or those undergoing primary glycerol rhizotomy were considered. We collected basic demographic, clinical, and

The primary objectives of the study were to analyse the robotic approach and ultrasound-guided paravertebral block compared to thoracoscopic intercostal nerve block after robotic pulmonary lobectomy on postoperative pain and opioids use. The secondary objectives were to analyse and compare patients' necessity of additional antalgic drugs and patients' performance during respiratory therapy, following robotic surgery and in the two groups. Consecutively, 52 patients undergoing robotic pulmonary lobectomies were treated either with ropivacaine-based intercostal nerve block or paravertebral block

PURPOSE: Disorders of the temporomandibular joint (TMJ) occur frequently, with a prevalence of 15 to 18%. Total joint replacement (TJR) surgery is indicated for severe joint damage associated with impaired function, pain, or occlusal change for which other treatments have been unsuccessful. The aim of this study was to assess changes in pain and range of motion (ROM), as well as postoperative complications and comorbidities, in patients receiving TJR surgery at Massachusetts General Hospital (MGH).

BACKGROUND: The incidence of pelvic ring fractures in the elderly significantly increased. Because of persistent pain and immobilization associated with this injury, surgical treatment is recommended. To minimise comorbidities and surgical risk, percutaneous techniques are becoming more relevant. In-screw cement augmentation of sacroiliac screw fixation is a promising procedure; however, clinical follow-up data remain scarce. This study investigated the safety and possible complications of the procedure along with a 1-year follow-up.
METHODS: Thirty-four patients (treated with 43 screws) were

RATIONALE AND SCOPE OF THE GUIDELINES Pain is a subjective unpleasant sensory and emotional experience. Acute pain occurs irrespective of age and has a prevalence of about 5% of the general population. Surgical procedures and painful diagnostic procedures are the main causes of this unpleasant and dangerous phenomenon for hospitalized children. It should be remembered that maintaining homeostasis in a child undergoing surgery is also affected by provision of an adequate level of analgesia and sedation as well as nerve conduction block within the surgical site. Even though both paediatric anaes

INTRODUCTION: Follicular unit extraction (FUE) is a safe and effective procedure in the hands of an expert. Side effects, particularly those which can lead to significant morbidity or mortality, are unacceptable as the procedure is done purely for cosmetic reasons. Any modification that decreases the risk associated with the procedure should be promoted.
AIM AND OBJECTIVE: The study was conducted to determine whether FUE can be carried out effectively with the elimination of nerve blocks and bupivacaine from the procedure.
MATERIALS AND METHODS: The study was conducted in 30 patients sufferi
