[Hemolysis-inducing substances in gastric secretion, incidence-rate of lysolecithin].

Samples of gastric contents from 152 patients with pyloric reflex were taken during gastroscopy after an overnight fast and examined for hemolytic activity. Hemolysis was induced by 97 specimens (64%). The hemolysis test was positive in 45% of patients with a histologically normal gastric mucosa, in 76% of patients suffering from chronic atrophic gastritis with intestinal metaplasia, in 83% with gastric erosions and in 67% with gastric ulcer. No lysolecithin was found in 9 of the 97 positive specimens. The other aspirates contained widely differing values up to 320 mg/100 ml. The average quant…

Short title: Anaesthetic mechanisms in the CNS Where and how do anaesthetics act? Mec.

General anaesthesia is a balance of hypnosis, amnesia, analgesia, and immobility, including the inhibition of noxious autonomic reflexes. Local anaesthesia implements the latter two elements in a conscious patient. This review article discusses several important aspects of anaesthesia, beginning with basic concepts such as the minimum alveolar concentration and afterwards developing into a discussion about the mechanisms of action of anaesthetics on a cellular level, introducing electrophysiological investigations in the brain to study hypnosis and amnesia, in the dorsal horn of the spinal cor…

Brain stem control of the phases of swallowing.

The phases of swallowing are controlled by central pattern-generating circuitry of the brain stem and peripheral reflexes. The oral, pharyngeal, and esophageal phases of swallowing are independent of each other. Although central pattern generators of the brain stem control the timing of these phases, the peripheral manifestation of these phases depends on sensory feedback through reflexes of the pharynx and esophagus. The dependence of the esophageal phase of swallowing on peripheral feedback explains its absence during failed swallows. Reflexes that initiate the pharyngeal phase of swallowing…

[A case of hypokalemic myopathy mimicking hemiparesis].

We report the case of an 86-year-old female with licorice-induced hypokalemic myopathy. For four years, she had been taking two kinds of Chinese medicines containing licorice. The patient presented with difficulty in holding her head up and proximal-dominant tetraparesis with significant laterality. In manual muscle tests, the right extremities showed grade 2/5 and the left extremities, grade 4/5. The general reflexes were decreased, and the bilateral Chaddock's reflexes were repeatedly positive. The sensory, cerebellar and autonomic systems were normal and discrete movements of all extremitie…

Attentional orienting triggered by gaze in schizophrenia.

The ability to detect the direction of another person's gaze and to shift our own attention reflexively in the same direction facilitates the sharing of attention with other people. Such sharing of attention would seem critical for the maintenance of normal social cognition. Social cognition is severely impaired in people with schizophrenia. So, we used spatial cuing paradigms to investigate reflexive (Experiment 1) and controlled (Experiment 2) attentional orienting triggered by gaze in schizophrenia. In Experiment 1, 30 patients and 24 controls detected targets appearing right or left of a c…

Control of airway caliber.

Tracheobronchial smooth muscle tone may be affected by 4 nervous mechanisms: (1) Vagal cholinergic parasympathetic nerves, which are the main agents for resting tone and most reflex bronchoconstrictions. Their activity is blocked by atropinic drugs. (2) Sympathetic adrenergic dilator nerves, which may act mainly on beta-adrenoceptors in the pulmonary bronchi; alternatively, they may inhibit ganglionic transmission in the vagal constrictor pathway. (3) Vagal nonadrenergic dilator nerves (NAIS). The neurotransmitter at these nerves is probably vasoactive intestinal polypeptide (VIP), although pu…

[Essential blepharospasm and botulinum toxin. An electrophysiologic study].

Thirty-five patients with essential blepharospasm were investigated electrophysiologically after local treatment with subcutaneous periocular injections of botulinum toxin. In all patients the blink reflex was measured on both sides. The results showed in some cases reproducible changes in the reflexes, indicating a central supranuclear origin for the spasm. Furthermore, an influence of afferent information on the "spasm threshold" via the trigeminal nerve is suggested.

Blepharospasm associated with olivopontocerebellar atrophy.

We report two cases of cranial dystonia (blepharospasm) associated with olivopontocerebellar atrophy (OPCA). The pathophysiology of blepharospasm appears to involve an increased excitability of the interneurons of the blink and corneal reflexes. It is hypothesized that blepharospasm associated with OPCA might be due to rostral brainstem lesions disrupting central dopaminergic and cholinergic pathways, resulting in disinhibition of brainstem reflexes or denervation supersensitivity of the facial nuclear complex.

Neonatal capsaicin does not affect unmyelinated efferent fibers of the autonomic nervous system: functional evidence.

Visceral nociceptive thresholds, viscero-visceral reflexes and the effects of efferent stimulation of autonomic nerves has been studied in adult rats treated at birth with capsaicin. These animals showed visceral analgesia and depressed visceral reflexes. However, stimulation of unmyelinated efferent fibers in autonomic nerves produced normal effects. It is concluded that neonatal treatment with capsaicin destroys selectively unmyelinated afferent fibers.

Gait apraxia in communicating hydrocephalus.

Apraxia of gait in patients with communicating hydrocephalus appears in the context of a generalised motor disorder that includes defective righting reflexes, generalised increased tone to passive movements, grasp reflexes, difficulty with serial movements of the hands and defective smooth pursuit eye movements. The inability to walk does not appear to be due to a motor disorder but to release of proprioceptive supporting reactions. This mechanism is triggered by proprioceptive stimuli.

Spinal nociceptive and non-nociceptive processing of cutaneous afferent input in people with chronic incomplete spinal cord injury.

In people with chronic spinal cord injury (SCI), the excitability of spinal reflexes of muscle afferent origins changes, likely contributing to sensorimotor impairments (e.g., spasticity). In contrast to changes in muscle afferent reflexes that are well-recognized, little is known about potentially altered processing of nociceptive and non-nociceptive cutaneous input in chronic incomplete SCI. Thus, this study aimed to systematically examine spinal processing of nociceptive and non-nociceptive cutaneous input in people with sensorimotor impairments due to chronic incomplete SCI, using non-inva…

Vestibular motor control.

The vestibular system is an essential sensory system that generates motor reflexes that are crucial for our daily activities, including stabilizing the visual axis of gaze and maintaining head and body posture. In addition, the vestibular system provides us with our sense of movement and orientation relative to space and serves a vital role in ensuring accurate voluntary behaviors. Neurophysiological studies have provided fundamental insights into the functional circuitry of vestibular motor pathways. A unique feature of the vestibular system compared to other sensory systems is that the same …

Inhibitory deficits in children with attention-deficit/hyperactivity disorder: intentional versus automatic mechanisms of attention.

Application of theoretically based tasks to the study of the development of selective attention has led to intriguing new findings concerning the role of inhibitory mechanisms. This study examined inhibitory mechanisms using a countermanding task and an inhibition of return task to compare deficits in intentionally, versus reflexively, controlled inhibition of attention in children with attention deficit hyperactivity disorder. Fifty children with attention-deficit/hyperactivity disorder (ADHD) were classified into one of three subtypes: predominantly inattentive (ADHD/PI), combined (ADHD/C), …

The articles of Babinski on his sign and the paper of 1898.

In 1896 Joseph François Felix Babinski described for the first time the phenomenon of the toes; nevertheless in this first paper he simply d … logical mechanisms proposed by Babinski are still valid today, e.g, he realized since 1896 that the reflex was part of the flexor reflex synergy and observed that several patients during the first hours of an acute cerebral or spinal insult had absent extensor responses. He also found that most patients with the abnormal reflex had weakness of dorsiflexi …

Intra-anaesthetic asystole in a Bulldog during positioning for brachycephalic obstructive airway syndrome surgery.

A male English Bulldog, aged 2.5 years, presented for brachycephalic obstructive airway syndrome corrective surgery for treatment of ventilatory difficulties and castration. The dog experienced asystole twice during positioning for surgery, suspected to result from the maxillomandibulocardiac reflex, a version of the trigeminocardiac reflex. Procedures were postponed and the dog was allowed to recover from anaesthesia. Echocardiography, electrocardiogram and 24 hour Holter monitoring were unremarkable. As ventilatory compromise persisted, the procedure was attempted again. Despite a period of …

Persistent Hiccups: An Unusual Presentation of Aspiration Pneumonia in the Elderly.

Persistent hiccups manifesting as the sole symptom of aspiration pneumonia is a rare occurrence. Approximately 10 cases have been reported in the last 15 years. Hiccups are defined as persistent if it occurs beyond 48 hours and intractable if it occurs continuously for one month. We highlight a case of an elderly man diagnosed with a subacute ischemic infarct of the right occipital lobe with a preserved gag reflex and swallow reflex. The patient's persistent hiccups began eight hours after an emetic episode. Typical signs of pneumonia were absent. Chest x-ray revealed bilateral lower lobe pulm…

Cough: The Emerging Role of the TRPA1 Channel.

Sneezing, cough, mucus secretion, and bronchoconstriction represent the main components of a coordinated and efficient reaction direct to expel or neutralize irritant agents from the respiratory system. A dense network of sensory nerves localized from the nose to the lower airways beneath the epithelium subserves this function. A variety of receptors and channels present in sensory nerve terminals by sensing irritant stimuli activate the system in emergence and initiate protective reflex responses, including cough. Previous and recent literature highlights the prominent role of some transient …

Electroclinical and genetic findings in a family with cortical tremor, myoclonus, and epilepsy.

We report a five-generation family showing cortical tremor, myoclonus, and epilepsy, originating from Naples, Italy. Eleven members, aged 24-56 years (mean: 39.2 years), suffered from hand tremor and myoclonus, whereas generalized seizures occurred in six. Electrophysiological study confirmed the presence of cortical reflex myoclonus in all affected members. In addition, giant somatosensory-evoked potential components and enhanced long loop reflex I were found also in three presymptomatic members who manifested hand tremor and myoclonus at upper limbs after 1.5 years of follow-up.
