Initial clinical experience with surgical technique of robot-assisted transperitoneal laparoscopic partial nephrectomy.

BACKGROUND: The incidental finding of small renal masses has increased due to widespread use of computed tomography as a diagnostic procedure. Some patients with either exophytic renal masses less than 4 cm and suboptimal renal function, a solitary kidney and bilateral renal tumors, or genetic predisposition to renal tumors are considered candidates for laparoscopic partial nephrectomy (LPN). A technical difficulty of LPN is performing laparoscopic intracorporeal suturing under the pressure of warm ischemia time. Because robotic systems have been shown to provide easier intracorporeal suturing

Robotic-assisted partial cystectomy with en bloc excision of the urachus and the umbilicus for urachal adenocarcinoma.

We report two cases of urachal adenocarcinoma managed with robotic-assisted partial cystectomy. A detailed description of the robotic technique including methods used to resect the tumor, urachus, and umbilicus en bloc is described. A review of the management of urachal adenocarcinoma is presented. The robotic approach is technically feasible and safe, and is an attractive alternative to traditional open or laparoscopic-assisted partial cystectomy for this uncommon genitourinary malignancy.

Totally robotic low anterior resection with total mesorectal excision and splenic flexure mobilization.

Some limitations of conventional laparoscopy have been overcome by the enhanced dexterity of the robotic da Vinci system, and its use in gastrointestinal procedures is evolving. However, difficulties accessing multiple quadrants of the abdomen with the first robotic system led to a rather slow introduction of the da Vinci into the field of abdominal surgery compared with its success with urologic and cardiac procedures. The new da Vinci S HD system offers improved range of motion that allows for easier access to a wider surgical field. The authors developed a new "one-step" setup to perform a

Identification of the retrotrigonal layer as a key anatomical landmark during robotically assisted radical prostatectomy.

OBJECTIVE: To define the gross and microscopic anatomy, and the surgical significance, of the newly described retrotrigonal layer of the bladder neck, as an aid during robotic radical prostatectomy (RRP). MATERIALS AND METHODS: The data for this study were obtained from five fresh cadaveric dissections and 100 consecutive RRPs. Five male cadavers with no previous pelvic or urethral surgery were dissected to expose the posterior bladder neck and identify the retrotrigonal layer. This same layer was reproducibly identified in all 100 RRPs. The gross appearance of this layer, and its relationshi

Peri-operative Considerations in Total Endoscopic Atrial Septal Defect Repair with Robotic Assistance.

The development of minimally invasive techniques represents a significant improvement in the repair of atrial septal defect by total endoscopy. Robot-assisted repair obviates the need for a sternotomy or thoracotomy. This is the case report of a 45-year-old male, who underwent atrial septal defect repair through the total endoscopic technique. The peri-operative management and associated problems in the post-operative period have also been described.

Hands-on robotic unicompartmental knee replacement: a prospective, randomised controlled study of the acrobot system.

We performed a prospective, randomised controlled trial of unicompartmental knee arthroplasty comparing the performance of the Acrobot system with conventional surgery. A total of 27 patients (28 knees) awaiting unicompartmental knee arthroplasty were randomly allocated to have the operation performed conventionally or with the assistance of the Acrobot. The primary outcome measurement was the angle of tibiofemoral alignment in the coronal plane, measured by CT. Other secondary parameters were evaluated and are reported. All of the Acrobot group had tibiofemoral alignment in the coronal plane

Concomitant management of renal calculi and pelvi-ureteric junction obstruction with robotic laparoscopic surgery.

OBJECTIVE: To present technical recommendations for robotic-assisted laparoscopic pyeloplasty (RALP) and stone extraction, as patients with kidney stones proximal to a pelvi-ureteric junction obstruction (PUJO) present a technical challenge, and have traditionally been managed with open surgery or percutaneous antegrade endopyelotomy. PATIENTS AND METHODS: From November 2002 to April 2005, 55 patients had RALP for PUJO; eight of these had concomitant renal calculi. Stone burden and location were assessed with a preoperative radiological examination. Before completing the PUJO repair, one robo

Feasibility study of a mini, bone-attached, robotic system for spinal operations: analysis and experiments.

STUDY DESIGN: In this investigation, a new concept of a miniature, bone-attached, medical robotic system for spinal operations is presented. As part of the design parameters of the robot, the forces and moments applied by the physician during insertion of Kirschner wires to soft tissues and drilling in hard tissues were examined. A theoretical model for the expected error of the robotic system due to the applied force has been derived and verified experimentally. The results of a clinical experiment that was carried out on a cadaver support the theoretical model derived and the miniature, bone

[Primary rotatory stability of hip endoprostheses stems after manual and robot assisted implantation].

AIM: In our study we compared the primary rotatory stability of hip endoprostheses which were implanted with the help of a robot with that of manually implanted stems. METHOD: We examined three different types of prosthesis-stems: Osteolock (Stryker-Howmedica), CBC (Mathys), Excia (Aesculap). Furthermore, we examined two types of prostheses which could not be implanted with a robot (Sl-Plus/Endoplus; Hipstar/Stryker-Howmedica). 10 stems of each prosthesis type were implanted in identical polyurethane foam blocks; 5 of them were implantated manually, 5 with the help of a robot (CASPAR, URS-Ort

A single bicaval venous cannula used for optimization of endoscopic robotically assisted procedures of the heart.

Robotically assisted heart operation is one of the minimally invasive approaches that have revolutionized many surgical procedures. Robotic procedures on the heart, however, are by no means routine, and their optimization requires development of new instruments and techniques. Described here is the robotically assisted use in a sheep model of a new peripherally inserted bicaval venous cannula in the totally endoscopic closure of atrial septal defect. The exposure of the surgical target area was successful with no obstructions in the surgical view. The cannula shows sufficient flow characterist

Robotically assisted cardiac surgery: minimally invasive techniques to totally endoscopic heart surgery.

Over the past decade, advancements in cardiac surgery occurred secondary to improvements in technology and the desire for a less invasive approach to operations in general. Minimally invasive cardiac surgery has progressed from partial sternotomy incisions to totally endoscopic open-heart procedures with robotic-assistance. There are 2 major companies that produce robotic equipment for use in cardiac surgery. These companies must undergo Food and Drug Association (FDA) mandated clinical trials on each cardiac surgical procedure, before it can be approved for public use. The surgeon must demons

Comparison of robotic-assisted and manual implantation of a primary total hip replacement. A prospective study.

BACKGROUND: Robotic-assisted total hip replacement has become a common method of implantation, especially in Europe. It frequently has been postulated that robotic reaming would result in an improved clinical outcome due to the better fit of the prosthesis, but that has never been demonstrated in a prospective study, to our knowledge. The purpose of this study was to compare robotic-assisted implantation of a total hip replacement with conventional manual implantation. METHODS: One hundred and fifty-four patients scheduled for total hip replacement were randomly assigned to undergo either con
