Mortality after inpatient stay for proximal humeral fractures.

BACKGROUND: A proximal humeral fracture is well established as a fracture of fragility in elderly patients. However, this injury does not receive the same emphasis on post-injury management as a femoral neck fracture. The objectives of this study were to establish the influence of sustaining a proximal humeral fracture on mortality and to identify the variables predictive of 5-year mortality. METHODS: Between January 2007 and January 2011, 288 consecutive patients who were admitted after sustaining a proximal humeral fracture were identified from the clinical coding department. Data were retr

Arthroscopically assisted shoulder arthrodesis: is it an effective technique?

PURPOSE: The purpose of this study was to compare clinical and radiographic outcomes of open and mini-open arthroscopic arthrodesis. METHODS: Twelve patients underwent arthroscopically assisted glenohumeral arthrodesis over a 5-year period; none were lost to follow-up. Surgery was performed with the patient in the lateral decubitus position, with the arm positioned in 30° each of flexion, internal rotation, and abduction. The articular cartilage was arthroscopically removed from the humerus and glenoid, creating flat opposing surfaces. One to 2 Kirschner wires were inserted percutaneously thr

Straining of the intact and fractured proximal humerus under physiological-like loading.

Surgical treatment of proximal humeral fractures remains challenging in elderly patients, primarily due to insufficient implant fixation. Both bone quality and physiological-like loading conditions are commonly overlooked during pre-clinical in vitro evaluation. However, this knowledge is necessary in order to improve surgical treatment of the proximal humerus and the mechanical behavior of implants, particularly in patients with complex fractures and weak bone stock. We hypothesize that the bone quality has a high influence on the bone straining, independent of the arm position. The goal of t

Alpine ski injuries and their prevention.

Alpine skiing is a popular sport with significant risk of injury. Since the 1970s, injury rates have dropped from approximately 5 to 8 per 1000 skier-days to about 2 to 3 per 1000 skier-days. The nature of the injuries has also been transformed over the same period. Lower leg injuries are becoming less common while the incidence of knee sprains and upper extremity injuries is becoming more common. Much of this change can be attributed to advancements in binding technology, which effectively reduces lower leg injury, but does not adequately address the issue of knee sprains. Along with design,

Comparative efficacy of latissimus dorsi and teres major vs. pectoralis major tendon transfers combined with Exactech Equinoxe reverse total shoulder arthroplasty in improving internal rotation: a preliminary result.

BACKGROUND: Reverse total shoulder arthroplasty (rTSA) is effective in treating massive irreparable rotator cuff tears (mRCT) and cuff tear arthropathy (CTA); however, improving internal rotation (IR) remains challenging. Tendon transfers have been introduced to improve active IR alongside rTSA, yet no clinical studies have compared their effectiveness. This study compares the clinical efficacy of rTSA combined with latissimus dorsi and teres major (LDTM) and pectoralis major (PM) transfers in improving IR in patients with mRCT and CTA who have a loss of active IR. METHODS: This retrospective

Decreasing Incidence of Youth Wrestling Injuries: A 10-Year Analysis of National Injury Data.

BACKGROUND: Wrestling is among the most common youth sports in the United States, with about 260,000 high school participants annually. There is a lack of literature investigating wrestling injury profiles and mechanisms of injuries. In the past 15 years, urgent care utilization has increased, and National Federation of State High School Associations (NFHS) concussion protocols have been developed and implemented. PURPOSE/HYPOTHESIS: The purpose of this study was to analyze causes, diagnoses, body parts, and trends associated with wrestling injuries presenting to US emergency departments. It

Increased Incidence of Upper Extremity Soft Tissue Injuries and Orthopaedic Surgeries in Patients with Eating Disorders.

BACKGROUND: Despite an established increased fracture risk in eating disorder patients, no studies, to our knowledge, have investigated the association between eating disorders and upper extremity soft tissue injury or surgery incidence. Given the association of eating disorders with nutritional deficiency and musculoskeletal sequelae, we hypothesized that patients with eating disorders would have an increased risk of soft tissue injury and surgery. The aim of this study was to elucidate this link and investigate if these incidences are increased in patients with eating disorders. METHODS: Co

Arthroscopic Onlay Biceps Tenodesis Utilizing 2 Knotless All-Suture Anchors.

BACKGROUND: Tenodesis is an effective surgical technique for the management of a pathologic long head of the biceps tendon. Tenodesis is performed with either an onlay (tendon opposed to cortical surface) or inlay (tendon docked into a reamed intra-cortical tunnel). Both techniques can be performed either from a subpectoral location, which is typically done through an open approach, or from a suprapectoral location, which is performed arthroscopically. The arthroscopic suprapectoral approach, when compared with its subpectoral counterpart, has been shown to report similar patient outcomes and

Minimal important difference and patient acceptable symptom state for common outcome instruments in patients with a closed humeral shaft fracture - analysis of the FISH randomised clinical trial data.

BACKGROUND: Two common ways of assessing the clinical relevance of treatment outcomes are the minimal important difference (MID) and the patient acceptable symptom state (PASS). The former represents the smallest change in the given outcome that makes people feel better, while the latter is the symptom level at which patients feel well. METHODS: We recruited 124 patients with a humeral shaft fracture to a randomised controlled trial comparing surgery to nonsurgical care. Outcome instruments included the Disabilities of Arm, Shoulder, and Hand (DASH) score, the Constant-Murley score, and two n

Imaging findings of injuries associated with badminton.

Badminton is characterized by distinctive biomechanical demands: high-velocity overhead strokes combined with forceful lunging, landing, and rapid multidirectional movement. These repetitive high-load mechanics generate a broad spectrum of musculoskeletal abnormalities, ranging from acute ligamentous and tendon injuries to cumulative stress-related remodeling. The shoulder and lower extremities are most frequently affected, reflecting the dual demands of explosive upper limb acceleration and high-impact lower limb deceleration. From an imaging standpoint, accurate evaluation requires recogniti

Predictors of Functional Outcomes of Nonoperatively Treated Extra-articular Distal End Radius Fractures in Older Individuals: A Prospective Clinical Study.

BACKGROUND: Distal radius fractures (DRFs) are some of the most common injuries suffered by the elderly; however, there are varied opinions on management. As there is limited research on patient factors affecting functional outcomes in conservatively managed patients with DRFs, this study aimed to find predictive factors for functional outcomes in such patients. METHODS: Patients with DRFs satisfying the inclusion criteria were managed with closed reduction and slab followed by cast application. Radiological parameters (radioulnar angulation, dorsal tilt, radial height, and ulnar variance) an

Mechanism matters: A 10-year experience of ballistic injuries of the upper extremity.

INTRODUCTION: Upper Extremity gunshot wounds represent a significant strain on community and hospital resources, and reports of their epidemiology are varied. We hypothesized that demographic and socioeconomic variables would be associated with variable injury patterns and management, and that two distinct populations would be affected by upper extremity ballistic injury based on violent versus accidental, self-inflicted mechanism. MATERIALS & METHODS: Retrospective review of all adult patients sustaining ballistic injury to the upper extremity at a single urban Level I trauma center over 10

Imaging Review of Obstetric Sequelae of Maternal Diabetes Mellitus.

Diabetes mellitus, whether preexisting or gestational, poses significant risk to both the mother and the developing fetus. A myriad of potential fetal complications in the setting of diabetic pregnancies include, among others, congenital anomalies, delayed fetal lung maturity, macrosomia, and increased perinatal morbidity and mortality. Congenital anomalies most commonly involve the nervous, cardiovascular, genitourinary, and musculoskeletal systems. Delayed fetal lung maturity, probably secondary to hyperglycemia suppressing surfactant secretion, is a major determinant of perinatal morbidity
