The mechano-immune-vesicle regulatory circuit: a systems framework for bone homeostasis and regeneration.

Bone remodeling is a mechanically adaptive process that integrates physical loading with immune regulation during homeostasis, repair, and disease. However, the traditional view fails to fully explain how mechanical and immune signals are coordinated across cellular compartments. Emerging evidence indicates that mechanical forces, immune responses, and extracellular vesicles (EVs) function as an integrated communication system rather than independent regulators. Here, we propose a systems-level framework termed the "mechano-immune-vesicle regulatory circuit". In this framework, biophysical cue

Bridging skeletal stem cell diversity and human skeletal modeling.

Skeletal stem cells (SSCs) underlie skeletal development, homeostasis, regeneration, and aging, yet their identities and functions are highly heterogeneous across anatomical sites and life stages. Mouse genetic studies have identified multiple SSC populations-each residing in distinct niches such as the growth plate, periosteum, and bone marrow-and revealed their dynamic regulation across developmental, homeostatic, regenerative, and aging contexts. However, translating these insights to humans remains challenging due to species differences and limited access to physiological human skeletal ti

Cutaneous Thermography in Arthropathies: Quantitative Imaging, Machine Learning, and Clinical Translation.

Arthropathies are a major global health challenge because of their high prevalence, chronic progression, and significant impact on quality of life and health systems. Therefore, prompt and accurate diagnosis is critical for slowing disease progression and improving outcomes. Traditional imaging modalities, such as ultrasound and magnetic resonance imaging, suffer from significant limitations, including operator dependence, limited accessibility, high cost, and limited reproducibility. Infrared thermography has become a promising non-invasive imaging technique for identifying thermal variations

Electrical stimulation as an emerging strategy in bone repair: Mechanisms, applications, and advances.

UNLABELLED: The treatment of critical-sized bone defects remains a substantial clinical challenge in orthopaedics. Electrical stimulation (ES), which mimics the endogenous bioelectric and piezoelectric properties of bone tissue, has emerged as a promising non-invasive modality to facilitate bone regeneration. This review elucidates the multifaceted mechanisms through which ES coordinately enhances osteogenesis, angiogenesis, and inflammation resolution, primarily via the regulation of key signalling pathways and modulation of the immune microenvironment at molecular and cellular levels. We als

Lumbar spinal stenosis: current concept of management.

Lumbar spinal stenosis (LSS) is a common degenerative spinal condition where spinal canal narrowing causes symptoms such as neurogenic claudication, radiculopathy, and lower back pain. While non-operative and surgical approaches yield similar long-term outcomes, surgical intervention-particularly decompression-can provide earlier symptom relief, functional recovery, and fall prevention in selected patients with refractory symptoms. Recent advancements in surgical technologies and image guidance have brought about a paradigm shift in LSS management. Biportal endoscopic spine surgery (BESS) has

Proprioception and Cognitive Polarization in Autism: The Bipolar Tensile Model.

This article introduces the Bipolar Tensile Model, a neurosemiotic and integrative framework for understanding Autism Spectrum Disorder (ASD). The model reinterprets ASD not as a collection of deficits, but as the outcome of an unresolved tension between two complementary hemispheric cognitive modalities: symbolic-sequential and corporeal-perceptual. In typical development, these modalities are dynamically harmonized through an extended interhemispheric network, comprising the Default Mode Network, major cerebral commissures (corpus callosum, anterior and hippocampal), and the cerebellum. In A

Fetal mental programming: maternal psychological homeostasis as a placenta-mediated determinant of cognitive and emotional development - an integrative perspective.

BACKGROUND: Fetal programming has traditionally been interpreted through metabolic and endocrine paradigms, while maternal psychological regulation remains underdeveloped as a biological determinant of neurodevelopment. Emerging evidence from placental biology, epigenetics, and fetal neuroimaging suggests that maternal psychological states may act not only as contextual exposures but also as dynamic physiological signals during pregnancy. CONTENT: This article proposes an integrative perspective in which maternal psychological homeostasis is conceptualized as a multidimensional regulatory sta

Neuroimmune Regulation of NLRP3 Signalling in Parkinson's Disease.

Parkinson's disease (PD) is increasingly conceptualized as a disorder of disrupted neuroimmune integration rather than isolated dopaminergic degeneration. Among innate immune mediators, the nucleotide-binding oligomerization domain, leucine-rich repeat, and pyrin domain-containing 3 (NLRP3) inflammasome has emerged as a central, stress-responsive signalling platform within the degenerating nigrostriatal system. This review synthesizes evidence positioning NLRP3 not as an autonomous inflammatory switch, but as a dynamic convergence node embedded within interconnected regulatory networks. Beyond

Minoxidil as a Prodrug: Review of Chemical, Pharmacological, and Technological Aspects in Alopecia Therapeutics.

Alopecia is a prevalent condition that affects both sexes, characterised by miniaturisation of hair follicles and changes in the dynamics of the hair cycle, such as androgenetic alopecia associated with various systemic factors, including the COVID-19 pandemic. Minoxidil (base), initially developed as an oral antihypertensive, is currently used in the treatment of alopecia, acting as a prodrug that requires hepatic sulphation to generate its active metabolite, minoxidil sulphate. Topical formulations use minoxidil sulphate, the active pharmaceutical ingredient, directly on the hair follicles.

Perioperative Anxiety in Adults: A Narrative Review of Pathophysiology, Assessment, and Multimodal Management Strategies.

Perioperative anxiety is a common psychophysiological stress response experienced by patients before and after surgery, with a global prevalence of approximately 48%. Its occurrence is influenced by multiple factors including age, sex, type of surgery, and psychosocial determinants. The underlying pathophysiological mechanisms are complex, involving multi-system interactions such as autonomic nervous system imbalance, dysregulation of the hypothalamic-pituitary-adrenal (HPA) axis, dysfunction of limbic system neural circuits, and neuroinflammation. Current assessment strategies are evolving fr

Strengthening trauma care in low- and middle-income countries through guideline adaptation and scalable system reforms.

Trauma is a leading global cause of death and disability, with nearly 90% of fatalities occurring in low- and middle-income countries (LMICs), where prehospital care, infrastructure, and trained personnel are limited. This narrative review synthesizes literature (2019-2025) on the initial management of adult polytrauma, and explores strategies to adapt global guidelines in resource-constrained settings. Contemporary frameworks such as advanced trauma life support (ATLS) and World Health Organization essential trauma care emphasize context-sensitive sequencing, permissive hypotension, balanced

Ultrasound-Triggered Tendon-Bone Comb With Axially Graded Electrical Gradients for Tendon-Bone Interface Integration.

Loss of electrophysiological guidance from endogenous fibrous-channeled electrical conduction critically impairs repair of the injured tendon-bone interface (TBI). Clinical analysis indicates that the inconsistent efficacy of conventional electrostimulation (ES) therapy arises from the limited penetration depth and insufficient focusing capacity of electrical currents in biological tissues. In this study, we developed a tendon-bone comb, termed the gradient microneedle (GMN), to improve spatial precision of electrical-signal delivery through multiple independent channel units and thereby promo

Carboxyl-enriched molecularly imprinted interface with enhanced electrostatic binding for ultrasensitive oxytocin detection in a portable electrochemical platform.

Oxytocin is a key peptide hormone regulating pregnancy and parturition, yet its accurate quantification remains challenging due to its ultra-low abundance and poor stability. Here, we report a portable electrochemical sensing platform based on an electrostatically engineered molecularly imprinted polymer interface for ultrasensitive OT detection in maternal serum. Pyrrole-2-carboxylic acid (P2CA) was rationally selected as the functional monomer to introduce carboxyl groups that enhance interfacial negative charge density, thereby strengthening electrostatic interactions with OT and improving

A Multichannel Flexible Interface for Environmental-Robust Laryngeal Signal Decoding.

Achieving robust human-machine interaction in noisy, constrained, or speech-impaired environments remains a significant challenge for conventional voice-based systems. Here, we present a wearable, flexible, and multichannel piezoresistive interface capable of decoding laryngeal and submandibular motion during complex speech behaviors. The system integrates a micropyramid polydimethylsiloxane (PDMS) sensing layer coated with conductive polypyrrole (PPy) onto a multichannel electrode array supported by a flexible polyimide (PI) substrate, providing superior skin conformity, high strain sensitivi

Stem Cells of the Hypothalamic-Pituitary-Adrenal Axis.

Endocrine tissues must continuously adjust their structure and function in response to circadian cues, metabolic state, and physiological stress. Within these dynamic systems, stem cells contribute not only to tissue maintenance and repair but also to functional plasticity. The hypothalamic-pituitary-adrenal (HPA) axis exemplifies this integration of local stem cell regulation with systemic endocrine control. In this Review, we highlight the shared regulatory principles and tissue-specific adaptations of stem cell populations across the hypothalamus, anterior pituitary and adrenal gland. We di

Stones in genomic storms: transposable element-driven variation in Prunus fruit trees.

Transposable elements (TEs) are major components of plant genomes and drivers of structural and functional diversification. In Prunus species, including peach, almond, apricot, cherry, and plum, TEs constitute more than half of the genome and shape evolutionary trajectories and phenotypic traits. This review synthesizes knowledge on TE composition, evolutionary dynamics and functional impacts in Prunus, highlighting advances enabled by long-read sequencing and structure-aware annotation pipelines. Both class I retrotransposons and class II DNA transposons contribute to genome expansion, gene d
