Avulsion fracture of the coracoid process in a patient with chronic anterior shoulder instability treated with the Latarjet procedure: a case report.

INTRODUCTION: Shoulder dislocations can cause acute and chronic instabilities that need to be addressed in order to restore joint functioning. The transfer of the coracoid process has become a feasible surgical procedure in patients with shoulder instability. Several concomitant injuries after recurrent dislocations have been described. CASE PRESENTATION: A 32-year-old German man presented to our department with a history of recurrent shoulder dislocations. He was diagnosed with an avulsion fracture of the coracoid process and dislocation of an osseous piece with attachment to the conjoined t

Surgical treatment of brachial plexus posterior cord lesion: a combination of nerve and tendon transfers, about nine patients.

Lesions of the posterior cord of the brachial plexus are rare. The symptoms are usually described as palsy of the deltoid and triceps brachii muscles and of the extensor muscles of the wrist, thumb and fingers. If there is no recovery, our strategy is to operate on these patients 6 months after the injury using a combination of nerve and tendon transfers. We present a series of nine patients, two with a partial palsy and seven with a complete palsy of the posterior cord. We performed five nerve transfers to the axillary nerve, four using intercostal nerves and one using the ulnar nerve. Six pa

Fibrin sealant in the treatment of acute ruptures of the Achilles tendon: long-term results.

OBJECTIVES: This study aims to point out the long-term results of fibrin sealant in the treatment of acute ruptures of the Achilles tendon. PATIENTS AND METHODS: Between January 1998 and December 2007, 42 patients (42 males; mean age 37.8 years; range 27 to 56 years) admitted to our clinic for acute ruptures of the Achilles tendon were treated with a fibrin sealant. The average time to surgery from injury was 2.0 (range 1 to 4) days. The patients were divided into two groups; in the first 32 patients fibrin sealant was used which was heated for preparation. In the second group of 10 patients

Vibratory facilitation of strength in fatigued muscle.

OBJECTIVE: To assess the effects of tendon vibration on isometric torque and electromyographic activity of fatigued muscles. DESIGN: Subjects performed a 30-trial isometric (2-s maximal effort contractions alternating with 6-s rest periods) elbow extensor fatigue series. Three additional trials with the same work-to-rest ratio were then performed in which the triceps extensor tendon was subjected to a 60-Hz, 1-mm vibration during the rest intervals. SETTING: A biomechanics research laboratory. PARTICIPANTS: Thirteen healthy women without history of upper-extremity injury or neurologic disor

Serratus anterior dysfunction. Recognition and treatment.

Recognition of scapular winging may be difficult, and potential errors in treatment can result. Such treatment errors may cause morbidity for the patient. In addition, electrical evidence of long thoracic nerve injury usually is required to confirm the etiology of scapular winging as being caused by serratus anterior dysfunction. Although various conditions may result in scapular winging, primary serratus anterior dysfunction can be treated effectively by transfer of the pectoralis major tendon; however, this surgical approach sometimes may given an unacceptable cosmesis, and there may be loca

Impact of Surgical Timing on Clinical Outcomes in Anatomic Double-Bundle Anterior Cruciate Ligament Reconstruction Using Hamstring Tendon Autografts.

BACKGROUND: To date, no studies have analyzed the influence on clinical outcomes of the interval between an anterior cruciate ligament (ACL) injury and double-bundle (DB) reconstruction with hamstring tendon autografts. HYPOTHESES: (1) Performing ACL reconstruction sooner after an injury will reduce postoperative anterior and rotatory knee instability, (2) postoperative range of knee motion or functional results will not be affected by different intervals between injury and surgery, and (3) preoperative isokinetic peak torque of the quadriceps and hamstring muscles will be lower in patients u

Endoscopic Transfer of Flexor Hallucis Longus Tendon for Chronic Achilles Tendon Rupture: Technical Aspects and Short-Time Experiences.

BACKGROUND: Chronic Achilles tendon ruptures can lead to reduced power of plantar flexion in the ankle with impaired gait ability. The open 1- or 2-incision technique for flexor hallucis longus transfer has proven good functional outcome but has the disadvantage of relatively extensive surgery performed at a vulnerable location. To reduce the risk of soft tissue problems, the flexor hallucis longus transfer can be performed endoscopically. MATERIAL AND METHOD: An endoscopic technique for flexor hallucis longus transfer is presented together with the experiences from the first six patients ope

Recurrent instability of the shoulder after age 40.

Twelve patients who had recurrent instability of the shoulder with onset after age 40 were reviewed. Eleven had anterior instability, and one had a posterior dislocation. The average age of the patients was 62.7 years. Symptoms began soon after initial injury. All patients with anterior instability had ruptured the subscapularis and anterior capsule from the lesser tuberosity, whereas the posterior dislocator had torn the infraspinatus and upper teres minor with the posterior capsule from the greater tuberosity. No patient had a Bankart lesion. Stability was restored in all cases by reattachin

Ultrasound-guided percutaneous rotator interval approach for simulated adhesiolysis: A proof-of-concept feasibility and anatomical safety study.

BACKGROUND: Several key structures involved in frozen shoulder are located in the anterior shoulder capsule. While ultrasound-guided corticosteroid injections and capsular hydrodilatation have demonstrated favorable outcomes through an anterior approach, percutaneous interventions aimed at mechanical adhesion release remain insufficiently investigated. This study evaluated the feasibility and anatomical safety of an ultrasound-guided simulated percutaneous adhesiolysis of the rotator interval in a cadaveric model. METHODS: The procedure was performed in a single non-pathological Thiel-embalme

Remnant-Preserved Single-Bundle Anterior Cruciate Ligament Augmentation in Multi-ligamentous Knee Reconstruction.

This study was conducted to present remnant-preserving anterior cruciate ligament (ACL) augmentation as a useful option for partial ACL injury in multiligament knee injury (MLKI) cases, which may also contribute to conserving graft resources. The present study involved patients diagnosed with MLKI at our institute from Spring 2006 to February 2021. A total of 71 MLKI cases were provided surgery due to knee instability and disability. For every patient, an arthroscopic diagnostic was performed to ensure that ACL tear and a remnant were present. When the ACL remnant was classified into group 2,

Clinical Outcomes and Complications With Open vs Minimally Invasive Achilles Tendon Repair.

BACKGROUND: There is no current consensus on whether to use an open or minimally invasive (MIS) approach for Achilles tendon repair after acute rupture. We hypothesized that patients in both open and MIS groups would have improved patient-reported outcome scores using the PROMIS system postoperatively, but that there would be minimal differences in these scores and complication rates between operative techniques. METHODS: A total of 185 patients who underwent surgery for an acute, unilateral Achilles tendon rupture between January 2016 and June 2019, with minimum 1-year follow-up were include

Effect of arthroscopic shoulder release on shoulder mobility and bone deformity following brachial plexus birth injury: a systematic review and meta-analysis.

BACKGROUND: Specific information to guide clinical practice is lacking for the effects of arthroscopic release on bone and joint deformities, as well as the additional benefits of tendon transfer, in children with brachial plexus birth injury. The aims of this study were (1) to evaluate changes in shoulder mobility and bone and joint deformity, (2) to evaluate the effect of release with and without tendon transfer on the same outcomes, and (3) to evaluate the perioperative and long-term complications. METHODS: We conducted a systematic review and meta-analysis. Four databases were searched us

Bone adaptation to altered loading after spinal cord injury: a study of bone and muscle strength.

Bone loss from the paralysed limbs after spinal cord injury (SCI) is well documented. Under physiological conditions, bones are adapted to forces which mainly emerge from muscle pull. After spinal cord injury (SCI), muscles can no longer contract voluntarily and are merely activated during spasms. Based on the Ashworth scale, previous research has suggested that these spasms may mitigate bone losses. We therefore wished to assess muscle forces after SCI with a more direct measure and compare it to measures of bone strength. We hypothesized that the bones in SCI patients would be in relation to

Neurovascular and tendinous damage with placement of anteroposterior distal locking bolts in the tibia.

OBJECTIVE: To determine the proximity of anteroposterior locking bolts inserted into the distal metaphyseal tibia to nearby neural, vascular, and tendinous structures. DESIGN: A cadaver study. SETTING: University trauma center. METHODS: Sixteen legs (8 matched pairs) were nailed in either neutral (Group 1) or 10 degrees of internal rotation (Group 2) and locked using one anteroposterior bolt. The anterior tibial and extensor hallucis longus tendons and neurovascular bundle were identified, and their respective locations in relation to the bolt head were measured. Average distances were calc

Arthroscopic Subscapularis Release and External Rotation Tendon Transfer for Brachial Plexus Birth Palsy: Surgical Technique.

Patients with brachial plexus birth injuries commonly develop problems at the shoulder. These problems include posterior subluxation of the shoulder, internal rotation contractures, and weakness of external rotation. The most common technique for the treatment of these is the release of the shoulder joint, the release of the internal rotators, and the transfer of latissimus teres and teres major tendons. In this article, we present a detailed technique with an accompanying step-by-step video to demonstrate how to perform these procedures.

Editorial Commentary: You Have Some (Infrapatellar Branch of the Saphenous) Nerve!

Infrapatellar branches of the saphenous nerve are always present at the anteromedial aspect of proximal tibia. Iatrogenic injury to these branches is a known consequence of knee surgery, be it an arthroscopic portal, harvesting of a hamstring or bone-patellar tendon-bone anterior cruciate ligament graft, or opening wedge opening high tibial osteotomy. Their anatomic course can be variable. High-resolution ultrasound may provide an improved diagnostic tool for better identification.

Instability of the metacarpophalangeal joint of the thumb. Reconstruction of the collateral ligaments using the extensor pollicis brevis tendon.

Reconstruction of the collateral ligaments of the metacarpophalangeal joint of the thumb was performed for fourteen ruptures of the ulnar and two ruptures of the radial collateral ligament, one to eighteen months after injury. The extensor pollicis brevis tendon was used to reinforce the resutured ligament. Symptoms were relieved in all cases after follow-ups ranging from one to seven years, even in two thumbs with mild degenerative arthritis of the metacarpophalangeal joint.

Differences in triceps surae muscle dynamometry and electromyography between adult males and females.

The triceps surae performs vital functions during locomotion and possesses shock-absorbing capacity. The injury rate of the Achilles tendon is higher in males than females. Quantification of the triceps surae muscle force outputs across sexes has not been determined. This study aimed to investigate and quantify sex-related triceps surae isokinetic parameters and activation magnitudes. A repeated-measures comparative design is implemented. A total of 20 males and 20 females joined this study. An isokinetic dynamometer and a synchronized electromyography system were employed to measure isokineti
