Skin irritation from a facial mask in Class III malocclusion: evaluation of individual silicone chin cups in a group of 100 children.

BACKGROUND: Reverse headgear (a facial mask) is one of the most frequently used interceptive orthodontic devices in patients with Class III malocclusion. It is a simple device but may present some drawbacks related to pressure on the skin generated by the forehead and chin supports linked to the maxillary intraoral appliance by elastics. This can cause injury to the chin and lips. METHODS: Patients between 7 and 8 years of age were selected from the Unit of Pediatric Dentistry. Orthopedic therapy with a rapid palatal expander and reverse headgear was prescribed for all subjects. The patients

Complications of Proximal Interphalangeal Joint Injuries: Prevention and Treatment.

Proximal interphalangeal joint injuries are one of the most common injuries of the hand. The severity of injury can vary from a minor sprain to a complex intra-articular fracture. Because of the complex anatomy of the joint, complications may occur even after an appropriate treatment. This article provides a comprehensive review on existing techniques to manage complications and imparts practical points to help prevent further complications after proximal interphalangeal joint injury.

Maxillary growth after maxillary protraction: Appliance in conjunction with presurgical orthopedics, gingivoperiosteoplasty, and Furlow palatoplasty for complete bilateral cleft lip and palate patients with protruded premaxilla.

BACKGROUND AND AIMS: In bilateral cleft lip and palate (BCLP) with premaxillary protrusion, a good outcome with adequate maxillary development is difficult to achieve. The purpose of this article is to evaluate the maxillary growth after using presurgical orthopedics (PSO), gingivoperiosteoplasty (GPP), Furlow palatoplasty, and maxillary protraction appliance (MPA) for BCLP with premaxillary protrusion. PATIENTS AND METHODS: Seven patients with complete BCLP with premaxillary protrusion were treated by PSO, cheiloplasty, GPP, and Furlow palatoplasty. MPA was used as part of the protocol for 6…

Thoracic pedicle subtraction osteotomy in the treatment of severe pediatric deformities.

The traditional surgical treatment of severe spinal deformities, both in adult and pediatric patients, consisted of a 360° approach. Posteri … al osteotomy, with no consistent results on the use of a single posterior-based thoracic pedicle subtraction osteotomy in the treatment of such challenging group of patients. The purpose of the present study was to review our operative experience with pediatric patients undergoing a single level PSO for the correction of thoracic kyphosis/kyphoscoliosis in …

Class I malocclusion treatment: influence of a missing mandibular incisor on anterior guidance.

This case report describes the orthodontic treatment of a patient with a deep-overbite Angle Class I malocclusion, agenesis of a mandibular central incisor, and 2 supernumerary teeth, which caused impaction of the mandibular first premolars. The 15-year-old patient also had a convex profile, maxillary dentoalveolar protrusion, and deficiency of space for the correct alignment of teeth. Therefore, treatment consisted of fixed appliance therapy, cervical headgear, extraction of the supernumeraries and the mandibular and maxillary first premolars, and mesiodistal reduction of the maxillary inciso…

Successful conservative treatment of chronic atlantoaxial rotatory fixation in a child with torticollis.

A 7-yr-old girl was diagnosed with atlantoaxial rotatory fixation, a serious but treatable cause of acquired torticollis in children and not well known by clinical physicians. Two and a half years after conservative treatment, she had recovered completely. This case report suggests that if the patient has no previous adequate rehabilitation treatment, then conservative treatments are effective for chronic but stable atlantoaxial rotatory fixation.

Nonextraction treatment of a skeletal Class III malocclusion.

This case report describes the nonsurgical, nonextraction therapy of a 16-year-old boy with a skeletal Class III malocclusion, a prognathic mandible, and a retrusive maxilla. He was initially classified as needing orthognathic surgery, but he and his parents wanted to avoid that. The Class III malocclusion was corrected with a rapid palatal expander and a maxillary protraction mask followed by nonextraction orthodontic treatment with fixed appliances, combined with short Class III and vertical elastics in the anterior area. The height of the maxillary alveolar process and the vertical face hei…

Distraction osteogenesis in the management of severe maxillary hypoplasia in cleft lip and palate patients.

The aim of this study was to systematically review literature reporting on the use of external distraction osteogenesis (DO) and internal DO in the treatment of severe maxillary hypoplasia in cleft and palate patients. Literature research has been performed using the PubMed database of the National Library of Medicine and National Institutes of Health from 1966 to August 2007. We used cleft lip and palate and distraction osteogenesis as key words. Of the 104 articles found, we only considered the Anglo-Saxon literature, which reported on the correction of the maxillary hypoplasia with DO techn…

Asymmetric bilateral hip dislocation after motor vehicle accident - a case study and review of the literature.

Bilateral asymmetric dislocations of the hip are rare compared to unilateral dislocations. This case study describes a female restrained passenger involved in a motor vehicle accident who sustained asymmetric bilateral hip dislocations. The patient underwent successful closed reduction of both hips. The clinical course and follow-up assessment of the patient was otherwise uneventful. Computed tomography scans, essential for diagnosing intra-articular loose bodies and subtle fractures, were performed after reduction and revealed in the right hip a nondisplaced acetabular rim fracture of the pos

Orthodontics. Part 9: anchorage control and distal movement.

Anchorage is an important consideration when planning orthodontic tooth movement. Unwanted tooth movement known as loss of anchorage can have a detrimental effect on the treatment outcome. Anchorage can be sourced from the teeth, the oral mucosa and underlying bone, implants and extra orally. If extra-oral anchorage is used, particularly with a facebow then the use of at least two safety devices is mandatory.

Slipped capital femoral epiphysis associated with peripheral osteoarticular tuberculosis.

We report a case of slipped capital femoral epiphysis that developed associated with a peripheral osteoarticular tuberculosis lesion located at the proximal metaphysis of the femur in contact with the growth plate in a 12-year-old boy. Multiple factors have been involved in slipped capital femoral epiphysis pathogenesis, but we believe an osteoarticular tuberculosis lesion is not a common finding as a possible etiological factor causing weakness of the growth plate and, therefore, the femoral head displacement.

Improvement of masticatory function after orthodontic treatment. Two case reports.

In patients with temporomandibular joints dysfunction (TMD), the masticatory function is often altered in that: (1) the intercuspal position (ICP) is less frequently achieved; (2) the mean vertical displacement from ICP at the end of opening is above normal; and (3) the isometric muscle contraction pattern during closure tends to be suppressed. We describe two cases of growing patients with severe malocclusion, who showed the same functional alterations as patients with TMJ before treatment, and normalization of the masticatory function after orthodontics.
