Biot's respiratory pattern under mechanical ventilation: Two case reports and literature review.

BACKGROUND: Biot's breathing, first described in 1876, remains an underrecognized yet clinically relevant marker of pontomedullary dysfunction. In mechanically ventilated patients, its identification through ventilator waveforms provides an objective correlate of central respiratory control failure. CASE PRESENTATION: We report two elderly female patients admitted to the intensive care unit for acute neurological deterioration, both requiring invasive mechanical ventilation initially delivered in controlled mandatory modes. After sedation withdrawal and transition to spontaneous ventilation,

Peri-Implantitis: Biofilm-Induced Disease or Prosthetically Driven Complication?

Peri-implantitis is a prevalent biological complication of implant therapy; however, whether it should be understood primarily as a biofilm-induced infection or as a consequence of prosthetically driven mechanical factors remains unclear. This narrative review synthesizes microbiological, biomechanical, and prosthetic evidence to address this question from an interdisciplinary perspective. Peri-implant biofilms occupy a distinct microbial niche, and their dysbiotic transition and the accompanying host response appear to be central to disease initiation, while an implant-directed foreign body r

Trauma-Induced Thyroid Storm After Road Traffic Accident Polytrauma.

Thyroid storm (TS) is a rare, life-threatening exacerbation of hyperthyroidism, where the thyroid gland produces too much of the thyroid hormones, often triggered by causes such as infection or surgery. Although rare, trauma can precipitate the transition from thyrotoxicosis to TS. Though typically seen in patients with known hyperthyroidism - especially Graves' disease - the onset of TS can be unpredictable and rapidly fatal if not promptly addressed. We present a 20-year-old male who developed TS following a road traffic accident. On arrival, the patient was in hypovolemic shock and subseque

Surgical and anatomical nuances in trigeminal neuralgia due to cerebellopontine angle lipoma: illustrative case.

BACKGROUND: Trigeminal neuralgia (TN) is a highly debilitating condition often caused by neurovascular compression. Less commonly, cerebellopontine angle (CPA) tumors can be the culprit, while intracranial lipomas are an exceedingly rare etiology. Treatment options include medical management and surgical interventions ranging from neurovascular decompression alone to mechanical rhizotomy and tumor resection. OBSERVATIONS: The authors present the case of a 30-year-old woman with medically refractory TN found to have a CPA lipoma abutting the root entry zone of the trigeminal nerve. A decision

Propylthiouracil Induced Neutropenia in Thyroid Storm: A Case Requiring Urgent Total Thyroidectomy.

Thyroid storm is a rare but life-threatening complication of hyperthyroidism that requires urgent management. While propylthiouracil (PTU) is a key treatment, it carries multiple risks, including potentially fatal risk of agranulocytosis. We report a 40-year-old female with Graves' disease who presented with thyroid storm and compressive goiter. High-dose PTU (1 g loading dose followed by 200 mg every 4 hours) was initiated; however, severe neutropenia (ANC: 0.22×10ˆ3/µL) developed within 48 hours, necessitating immediate drug discontinuation. Bridging therapy with hydrocortisone, cholestyrami

Predicting trajectories of illness using RNA velocity of whole blood.

Transcriptomic analyses reveal the status of cells, tissues, or organisms, across states of health and disease. RNA velocity adds a temporal dimension to single cell analyses, predicting future transcriptomic and phenotypic states, based on the current spliced and unspliced mRNA of each cell. We hypothesized that RNA velocity could be adapted to predict future clinical state of individuals with acute and chronic illnesses, using their whole-blood transcriptomes. We developed VeloCD, a method for quantitative prediction of transitions in clinical state from a single time-point RNA sample. This

Isolated posterior mitral leaflet elongation as a rare mechanism of systolic anterior motion in hypertrophic cardiomyopathy: a case report of successful treatment with mavacamten.

BACKGROUND: Hypertrophic cardiomyopathy (HCM) is the most common inherited cardiac disorder. Left ventricular outflow tract (LVOT) obstruction typically results from systolic anterior motion (SAM) of the anterior mitral leaflet. CASE SUMMARY: We describe a 26-year-old man with HCM in whom SAM was generated by isolated elongation of the posterior mitral leaflet, a rare mechanism with important clinical implications. He was diagnosed during childhood by a paediatric cardiologist and followed until transition to adult care. On presentation, he was in NYHA (New York Heart Association) class II wi

Bridging pediatric and adult neuro-oncology: Insights into adolescents and young adults (AYA) central nervous system tumors.

Adolescents and young adults (AYA) with central nervous system (CNS) tumors represent a distinct and historically underrecognized population that spans the traditional pediatric-adult oncology divide, with unique biological, clinical, and survivorship considerations. The central nervous system (CNS) tumor spectrum in this age band is thus broad: pediatric-type entities driven by MAPK-pathway alterations, adult-type IDH- and H3-mutant gliomas, embryonal tumors, ependymomas, and intracranial germ cell tumors all appear, and prognosis and treatment are increasingly determined by molecular subtype

Return to Intended Oncologic Therapy After Brain Metastasis Surgery: Mapping the Early Postoperative Pathway.

Surgery for brain metastases (BM) is embedded in a multidisciplinary oncologic pathway. This study evaluated oncologic readiness and return to intended oncologic therapy (RIOT) after surgery for BM. Patients undergoing surgery for histologically confirmed BM were retrospectively identified. Oncologic readiness was defined as sufficient postoperative clinical and neurological recovery to allow the next indicated oncologic treatment. RIOT was defined as postoperative radiotherapy and/or systemic treatment within 30 days after surgery. Exploratory analyses assessed factors associated with oncolog

Evolving Paradigms in Acute Myeloid Leukemia: Personalized Approaches to Therapy Across Age and Risk Groups.

Acute myeloid leukemia (AML) is an aggressive hematologic malignancy characterized by the clonal proliferation of myeloid precursors and rapid progression. Historically consisting of intensive chemotherapy, AML management has evolved significantly due to advances in molecular diagnostics and risk stratification. This review discusses current therapeutic paradigms in AML, emphasizing the growing role of personalized medicine across age and risk groups. For younger, fit patients, intensive regimens such as the "7 + 3" protocol remain the standard, often enhanced by targeted agents like FMS-like

Adaptive cancer therapy: can non-genetic factors become its achilles heel?

The recurrence of clinically advanced cancers is an evolutionary consequence of standard-of-care chemotherapies generally administered at maximum tolerated doses to kill as many cancer cells as possible. The inevitable appearance of resistance raises the possibility of shifting treatment goals from complete tumor eradication to long-term disease control. The latter approach is employed by adaptive therapy, which aims to inhibit the evolutionary dynamics governing the spread of resistant tumor phenotypes. Adaptive therapy changes focus from the cancer cells that are responsive to therapy to tho

Circulating Tumor Cells for the Monitoring of Lung Cancer Therapies.

Lung cancer, as one of the most prevalent and lethal malignancies, requires immediate and effective therapeutic solutions. Therefore, additional innovative methods are continually sought to achieve optimal treatment outcomes. Various markers are used to select the most effective therapies, assess clinical responses, and facilitate follow-up care for the patients. Circulating tumor cells (CTCs) remain a valuable biomarker in clinical management of cancer patients due to the range of information they provide and their high prognostic and predictive potential in monitoring anticancer therapy. CTC

Unseen Victims: Forensic Investigation of Sexual Violence Against Adult Males in Turkey.

This study investigates sexual violence against adult men in Turkey, with particular attention to cases involving refugees and migrants. While sexual violence is often perceived as predominantly affecting women, adult men especially those from marginalized groups are also at risk, yet their experiences remain largely underreported and understudied. Using a retrospective analysis of forensic case records over a ten-year period in Eskişehir, a key transit city in western Turkey, this study examines demographic and case-related factors such as age, nationality, location of the incident, and the v

Integrated microfluidic cartridge for rapid and colorimetric detection of Mycoplasma pneumoniae.

Mycoplasma pneumoniae is a major cause of community-acquired pneumonia, yet its diagnosis remains challenging due to overlapping symptoms with other respiratory pathogens. In this study, an integrated microfluidic cartridge platform has been developed that combines three synergistic advancements: a chemical lysis reagent (GuHCl/EDTA) enabling direct genomic DNA release from M. pneumoniae collected by nasal swabs within 2 min without extraction steps, a novel dual-dye system (hydroxy naphthol blue/SYBR Green I) that leverages complementary fluorescence transitions from red to green for enhanced

Comprehensive rehabilitation of a serviceman with triple limb amputation following combat polytrauma: a case study from Ukraine's wartime context.

The ongoing war in Ukraine has led to a significant rise in combat-related polytrauma injuries, including high-level multiple amputations, delayed evacuations, infectious complications, and limited access to rehabilitation services. This clinical case demonstrates a comprehensive, patient-centred approach to the rehabilitation of a 25-year-old serviceman with triple amputations caused by a mine-blast injury. The patient progressed through pre-prosthetic and prosthetic rehabilitation stages, achieved independent mobility with prosthetics, transitioned to the K3 functional level, and adapted to

School Performance and Learning Challenges in Children and Adolescents with Congenital Heart Disease.

Children with congenital heart disease are at risk for neurocognitive and neurodevelopmental challenges, learning disabilities, and psychosocial difficulties. Our narrative review draws literature-based evidence for the effect of congenital heart disease on school performance. We conducted a literature review and extracted relevant articles from Google Scholar, Medline, and PubMed. Evidence showcases that school performance is impacted directly by congenital heart disease. This seems to be underlined by different factors inherent to congenital heart disease, like inattention, language difficul

Place of High-Flow Oxygen Therapy (HFOT) in the Perioperative Period: A Thoracic Surgery Case Report.

High-flow oxygen therapy (HFOT) has emerged as a promising approach in perioperative respiratory management, particularly in preventing postoperative respiratory complications, although its application in thoracic surgery remains underexplored. We report the case of a 58-year-old female with a history of pulmonary tumor and metastatic bone disease who was admitted for thoracic surgery due to dyspnea (NYHA class III). Preoperative evaluation revealed poor general condition with stable hemodynamics. Given the elevated risk of respiratory complications, HFOT was initiated for both preoxygenation
