The data of the literature demonstrate the poor global results of surgical treatment of the principal gastrointestinal cancers: oesophagus, stomach, bile ducts, liver, exocrine pancreas, colon and rectum. Surgery which was thought to be curative all too frequently proves to be simply a palliative procedure. Acute pancreatitis associated with the passage of gallstones through the sphincter of Oddi might be caused by obstruction of pancreatic exocrine secretion or by duodenopancreatic reflux. Possible complicating factors are discussed, which are necessary in order to turn the interstitial edematous inflammation caused by pancreatic duct obstruction into hemorrhagic necrotizing pancreatitis. Furthermore, clinical and experimental data are reviewed concerning a possible collapse of the protective mechanisms which usually prevent contact of duodenal contents and pancreatic tissue.

The authors report the short interval results of a series of 183 patients operated on for hydatic cyst of the liver; all had undergone "the conservative operation" of resection of the bulging dome. Mortality and morbidity rates are similar to those of recently published studies on radical treatment. We prospectively evaluated the incidence, clinical features, radiographic findings, and course of biliary and pancreatic disease caused by ascariasis in an endemic area in India. Ascariasis was an etiologic factor in 40 (36.7%) of the 109 patients studied who had biliary and pancreatic diseases. A total of 180 patients with malignant obstructive jaundice have been treated by 5 different methods: surgical resection; surgical by-pass; percutaneous prosthesis; endoscopic prosthesis; and endoscopic sphincterotomy (for papillary tumours).
