Slipped capital femoral epiphysis. Results of treatment with a single cannulated screw.

Thirty patients with 36 slipped capital femoral epiphysis underwent pinning with a single cannulated screw. All patients were pinned in situ, allowed immediate weight bearing as tolerated, discontinued crutches at 6 weeks, and had a minimum of 1 year follow up (average 21 months). One patient had avascular necrosis, one patient had bilateral chondrolysis, and two patients required repinning.

Canthal fixation with a stainless steel screw: a report of ten patients.

During a period of six years (1981-1987) we have used the Nelson stainless steel screw for fixation of the medial canthal tissues to the medial orbital wall in nine patients and in the lateral wall in one. No complications have occurred in any of the patients, and we endorse its use.

The bent screw: a sign of postoperative recurrent dislocation.

Recurrent dislocation is a recognized complication following surgical treatment of prior dislocation, especially in the shoulder. Postoperative deformity of the surgical hardware should alert one to the possibility of this complication. I report two patients in whom bending of the internal fixation screws confirmed the clinical suspicion of recurrent postoperative dislocation.

The uncommon sprain. Ligamentous diastasis of the ankle without fracture or bony deformity.

A 19-year-old male patient presented with ligamentous diastasis of the ankle without fracture or bony deformity--an uncommon sprain that may be mistaken for the common ankle sprain. The author identifies four types of ligamentous diastasis of the ankle and discusses how they may be differentiated from the common ankle sprain, thus avoiding undertreatment and the initiation of premature joint degeneration.

Posterior lateral distraction spondylodesis using the twofold sacral bar.

From January 1978 until March 1985 more than 500 patients were operated on by posterolateral distraction spondylodesis using Zielke's twofold sacral bar for stabilization of the lumbosacral joint. Over 50% of these patients presented with low-back pain with or without neurologic deficits. A considerable number of these had been preoperated one to ten times. Those who had not been operated on before had the best results concerning lumbar back pain and neurologic deficits. Only 49% of the multioperated back patients became pain-free. Nevertheless, even in this group, decompression through distra

The coracoid transfer for recurrent anterior instability of the shoulder in adolescents.

Thirty-six shoulders with recurrent anterior dislocation or subluxation in thirty-four adolescent patients were treated with a modified Bristow procedure. The average length of follow-up was five years and ten months. Postoperatively, two patients had recurrent anterior subluxation on one occasion and none had recurrent anterior dislocation. The average loss of external rotation was 10 degrees. Despite the fact that young patients have a high rate of recurrence after anterior dislocation or subluxation of the shoulder, the modified Bristow procedure is an effective method of treating recurrent

A review of the Bristow operation for recurrent anterior shoulder dislocation in athletes.

Twenty-four recurrent dislocating shoulders, in patients ranging from 15 to 26 years, were evaluated after surgical treatment using the Bristow procedure. The operative procedure is discussed in detail recommending the use of an A-O malleolar compression screw, because of ease in insertion, to fix the caroacoid process to the anterior scapular neck. The Bristow procedure as described is an effective operation for achieving shoulder stability while maintaining nearly a complete range of motion in most cases. There was one case of recurrent dislocation after surgery which is described in detail.

Bone allografting: an original method for biological osteosynthesis and bone reinforcement in children with osteogenesis imperfecta.

PURPOSE: Osteogenesis imperfecta (OI) is a genetic disorder responsible for various symptoms including deformities and frequent fractures. Bone allografting is poorly documented in this condition. The objective of this study was to describe our experience and assessments in a consecutive series of OI patients. METHODS: Thirty-nine lower limb allograft procedures (28 femurs, 11 tibias) were performed in 26OI patients (mean age, 12.9 years). They were classified as type III of Sillence (17), type IV (6), and 3 recessive forms. The indications for surgery were correction of deformity (19), fract

Amlodipine accelerates bone healing in a stable closed femoral fracture model in mice.

Calcium channel blockers (CCBs), which are widely used in the treatment of hypertension, have been shown to influence bone metabolism. However, there is little information on whether CCBs also influence the process of fracture healing. Therefore, the effect of the CCB amlodipine on bone healing was studied in a stable closed fracture model in mice using intramedullary screw fixation. Bone healing was investigated by radiology, biomechanics, histomorphometry and Western blot analysis 2 and 5 weeks after fracture healing. Animals were treated daily (post operatively) per os using a gavage with a

Implant/Abutment Biomechanics and Material Selection for Predictable Results.

Change in crestal bone height and volume around endosseous dental implants is a complex issue involving interactions among many variables such as surgical approach, patient health, volume of hard and soft tissues, and implant configuration. Establishing and maintaining a soft-tissue seal around transmucosal abutments on bone-level implants or on the collar of one-stage tissue-level implants is paramount to maintaining crestal bone height, and failing to do so will result in apical migration of the soft tissue onto the implant. Bone-level implants have been constructed with many connection conf

Early bone healing around different implant bulk designs and surgical techniques: A study in dogs.

PURPOSE: To evaluate the bone healing response to different implant root shape designs in a dog model. MATERIALS AND METHODS: Three by eight millimeter screw-type short-pitch (SP) and large-pitch (LP) implants (Intra-Lock International, Boca Raton, FL, USA), and 4.5 x 6 mm plateau (P) implants (Bicon LLC, Boston, MA, USA) were placed along the proximal tibia of six dogs for 2 and 4 weeks. The combination of implant design and final osteotomy drilling resulted in healing chambers for the LP and P implants. The implants were nondecalcified processed to plates of approximately 30-microm thicknes

Missing osteogenic effect of expanded autogenous osteoblast-like cells in a minipig model of sinus augmentation with simultaneous dental implant installation.

OBJECTIVES: With natural bovine bone mineral (BioOss) as carrier, the study aimed at investigating the effect of autogenous osteoblast-like cells on bone regeneration in an orthotopic (maxillary sinus) and an ectopic (muscle) site. MATERIALS AND METHODS: Autogenous osteoblast-like cells were isolated from iliac cancellous bone of six minipigs and expanded in an autogenous serum-supplemented osteogenic medium. After confirmation of osteogenicity, the expanded cells were precultivated on BioOss granules for 1 week. Four milliliter of cell-seeded BioOss were used for sinus augmentation of right

Calcaneus Fracture and Posterior Arthroscopic Primary Subtalar Arthrodesis (C-PASTA).

UNLABELLED: Complex intra-articular calcaneal fractures often resulted in secondary pain and deformity, requiring subsequent subtalar arthrodesis1-3. The literature suggests that primary subtalar arthrodesis in the acute period has good functional results2,3. The literature also demonstrates that posterior arthroscopic subtalar arthrodesis for chronic arthritis has favorable results5. Thus, we propose an approach to treating these difficult intra-articular calcaneal fractures that utilizes a posterior arthroscopic primary subtalar arthrodesis technique-aptly named Calcaneus Fracture and Poster

An unusual case of atrophic mandible fracture in a patient with osteogenesis imperfecta and on oral bisphosphonate therapy: Case report.

Fractures of severely atrophic (height < 10 mm) edentulous mandibles are infrequent and challenging to manage. Factors such as sclerotic bone and decreased vascularity combined with systemic diseases complicate the management of such fractures. Osteogenesis imperfecta (OI) is a heterogeneous group of inherited disorders of type I collagen metabolism. Patients with OI characteristically present with histories of long bone fractures, deformities, blue sclerae, and opalescent dentin. However, fractures of the facial skeleton are rare. Bisphosphonate therapy has been proven to effectively reduce t

A clinical study on treatment of temporomandibular joint chronic recurrent dislocations by a modified eminoplasty technique.

Various augmentation techniques have been proposed to incarcerate the condyle for the treatment of temporomandibular joint chronic recurrent dislocation (CRD). Some of them have been used successfully, whereas some of them failed. The purpose of this study was to present an alternative eminoplasty technique, more specifically an inlay augmentation technique of articular eminence in the treatment of temporomandibular joint CRD. Twelve patients, 11 of whom had bilateral and the remaining 1 of whom had unilateral CRD, were evaluated. The same technique was used in the treatment of all patients. A

Modular noncemented total hip arthroplasty for congenital dislocation of the hip. Case report and design rationale.

The highest rate of failure and the greatest technical difficulty in total hip arthroplasty occurs with congenital dislocation of the hip (CDH). Predisposing factors are failure to secure special femoral components to fit an extremely narrow and straight medullary cavity with space for only a very thin mantle of cement. The acetabulum is usually atrophic, and bone grafts are commonly required to support a small-diameter cup. The young age of the average patient and high levels of activity contribute to cement failure. A new modular cementless prosthesis provides excellent immediate skeletal fi

Single-stage posterior surgical management of grade 5 L5 spondylolisthesis in a patient with neurofibromatosis type 1 and dural meningocele: illustrative case.

BACKGROUND: To the best of the authors' knowledge, this is the first reported case of Meyerding grade 5 L5 spondylolisthesis complicated by neurofibromatosis type 1 (NF1) and dural meningocele, successfully managed via a single-stage posterior approach-a well-established technique adapted for this complex scenario. Surgical management of grade 5 spondylolisthesis remains challenging. OBSERVATIONS: A 18-year-old female was admitted to the hospital for low back pain and radicular pain of both lower limbs. Radiographic imaging showed grade 5 spondylolisthesis of L5. On MRI, dural meningocele cou
