The innovative use of a large-scale industry biomedical consortium to research the genetic basis of drug induced serious adverse events.

The International Serious Adverse Event Consortium (SAEC) is a pharmaceutical industry and FDA led international (501 c3 non-profit) consortium, focused on identifying and validating DNA-variants useful in predicting the risk of drug induced, rare serious adverse events (SAEs). As such, it functions with the explicit purpose of enhancing the 'public good'. Its members are (i) organizations engaged principally in the business of discovering, developing and marketing pharmaceutical products, or (ii) a charitable, governmental, or other non-profit organization with an interest in researching the

Sociodemographic and Health-Risk Determinants of COVID-19 Vaccine Booster Preferences and Willingness to Pay in Singapore: Discrete Choice Experiment.

BACKGROUND: Waning immunity and recurring COVID-19 waves point to the importance of sustained booster vaccination strategies. With repeated boosters needed to sustain population-level immunity, evaluating public preferences for COVID-19 booster vaccination is important for informing future vaccination strategies, as changing perceptions of urgency, threat, and motivation may alter willingness to receive vaccine boosters compared with initial uptake. OBJECTIVE: This study aimed to explore the public's stated preferences and implicit willingness to pay for COVID-19 vaccine boosters in Singapore

Cultivating Innovative, Pragmatic, Randomized Controlled Registry Trials Embedded in Hemodialysis Care: Conference Proceeding From Gardener's Grove 2023.

PURPOSE OF THE CONFERENCE: Hemodialysis is a life-sustaining treatment for patients with end-stage kidney disease. However, patients on dialysis continue to face poor quality of life and short life expectancies. Despite this, the nephrology community conducts the fewest randomized controlled trials of any medical discipline, relying instead on expert opinion to guide many aspects of hemodialysis care. There is a need to conduct high-quality pragmatic randomized controlled trials in hemodialysis to drive evidence-based practice. To this end, the Innovative Clinical Trials in Hemodialysis Center

A real-world preventive primary care model for cardiorenal metabolic disease: clinical impact of a personalised care approach in Harrow, North West London.

BACKGROUND: Cardiorenal metabolic (CRM) disease, is identified by the co-location of multiple disorders including obesity, diabetes, hypertension, cardiovascular disease and chronic kidney disease (CKD). Early intervention is essential to slow CKD progression, reduce cardiovascular risk, and improve quality of life. The Harrow CRM Hub project established a personalised, multidisciplinary pathway to identify high-risk patients, optimise clinical management, and provide access to lifestyle and psychosocial support. This paper reports on the clinical outcomes achieved within the first year of imp

Assessing parent interaction with deaf infants: A quantitative survey of UK professional practice.

BACKGROUND: Between 90% and 95% of deaf children are born to hearing parents who often need support with how to adapt their communication. Parent-child interaction (PCI) is an important predictor of deaf children's future language development. It is therefore necessary for professionals to assess parents' strengths and needs to identify areas for intervention. Qualified teachers of the deaf (QToDs), speech and language therapists (SLTs), psychologists, and national deaf child and adolescent mental health (NDCAMHS) professionals regularly support families with deaf children. With no current evi

Using a low-cost, real-time electronic immunization registry in Pakistan to demonstrate utility of data for immunization programs and evidence-based decision making to achieve SDG-3: Insights from analysis of Big Data on vaccines.

BACKGROUND: Despite the proliferation of digital interventions such as Electronic Immunization Registries (EIR), currently, there is little evidence regarding the use of EIR data to improve immunization outcomes in resource-constrained settings. To achieve the Sustainable Development Goal (SDG) of ensuring healthy lives and well-being for all ages, particularly for newborns and children under the age of 5 (goal 3b), it is essential to generate and use quality data for evidence-based decision making to overcome barriers inherent in immunization systems. In Pakistan, only 66 % of children receiv

Evaluation of mass treatment with ivermectin program reach and survey coverage for onchocerciasis elimination in selected endemic areas of Ethiopia.

Currently, national governments of onchocerciasis endemic African countries are working towards the elimination of the disease using mass drug administration (MDA) with ivermectin as a primary strategy. Attainment of this goal requires implementation of prolonged high MDA coverage in all endemic areas, and vigilant monitoring and evaluation of the program. This study was thus conducted with the purpose of i) providing estimate of ivermectin coverage, ii) validating the MDA coverage reported through community drug distributors (CDDs), iii) determining the factors associated with MDA coverage, a

Community Perspectives and Behavior Change Strategies for Integrated Long-Acting Reversible Contraception and Long-Acting Injectable Preexposure Prophylaxis Roll Out for Adolescent Girls and Young Women in Uganda (COMPASS): Protocol for a Mixed Methods Implementation Science Study.

BACKGROUND: HIV incidence among adolescent girls and young women (AGYW) aged 15-24 years in Eastern and Southern Africa remains unacceptably high. Unprotected sex increases the risk of HIV and other sexually transmitted infections and unintended pregnancies, reflecting an unmet need for effective contraception among AGYW. Long-acting injectable preexposure prophylaxis (LAI-PrEP) and long-acting reversible contraception (LARC) offer effective, durable protection against HIV and unintended pregnancy. However, uptake and persistence remain suboptimal due to low awareness, fear of side effects, st

Heartburn Center Set-Up in a Community Setting: Engineering and Execution.

Background: Optimal management of gastroesophageal reflux disease (GERD) requires a concerted team of physicians rather than an individual approach. While an integrated approach to GERD has previously been proposed, the practical execution of such a "center of excellence" (COE) has not been described, particularly in a community setting. Ranging from initial consultation and diagnosis to surgical intervention for complex disease, such an approach is likely to provide optimal care and provide surveillance for patients with a complex disease process of GERD. Methods: We report our approach to im

Patient-Centered, Theory-Based, Online Intervention to Promote a Physically Active Lifestyle for People With Multiple Sclerosis: Protocol for a Randomized Controlled Trial.

BACKGROUND: People with multiple sclerosis (MS) experience life challenges due to the nature of disease progression. Although an active lifestyle has been shown effective for symptom and relapse management, most people with MS lead a sedentary lifestyle and do not reach the recommended physical activity (PA) guidelines. OBJECTIVE: This study aims to test the preliminary efficacy of a codeveloped, online self-management intervention based on the health action process approach (HAPA) adapted for people with MS to cultivate a physically active lifestyle. METHODS: The 8-week intervention program

Preoperative Activities of Daily Living Dependency is Associated With Higher 30-Day Readmission Risk for Older Adults After Total Joint Arthroplasty.

BACKGROUND: With recent Medicare payment changes, older adults are increasingly likely to be discharged home instead of to extended care facilities after total joint arthroplasty (TJA), and may therefore be at increased risk for readmissions. Identifying risk factors for readmission could help re-align care pathways for vulnerable patients; recent research has suggested preoperative dependency in activities of daily living (ADL) may increase perioperative and postoperative surgical complications. However, the proportion of older surgical patients with ADL dependence before TJA, and the impact

Integration of HIV/AIDS services with maternal, neonatal and child health, nutrition, and family planning services.

BACKGROUND: The integration of HIV/AIDS and maternal, neonatal, child health and nutrition services (MNCHN), including family planning (FP) is recognized as a key strategy to reduce maternal and child mortality and control the HIV/AIDS epidemic. However, limited evidence exists on the effectiveness of service integration. OBJECTIVES: To evaluate the impact of integrating MNCHN-FP and HIV/AIDS services on health, behavioral, and economic outcomes and to identify research gaps. SEARCH METHODS: Using the Cochrane Collaboration's validated search strategies for identifying reports of HIV interve

High-Resolution Mapping of Environmental Suitability for Reported Ebola Spillover Events in Uganda to Inform Surveillance and Preparedness.

BACKGROUND: Uganda experiences recurrent Ebola disease (EBOD) outbreaks, yet national suitability maps often pool virus species or are too coarse for district-level planning. We aimed to generate species-resolved, 1 km relative environmental suitability surfaces for reported Ebola spillover events in Uganda; quantify the contribution of environmental, anthropogenic, and reservoir-related predictors to modeled suitability; and translate continuous outputs into operational prioritization tiers for district surveillance and preparedness. METHODS: We compiled 71 laboratory-confirmed reported Ebol

Estimating the societal cost of type 2 diabetes in Malmö, Sweden: a register-based cost analysis.

BACKGROUND: With the prevalence of Type 2 Diabetes (T2D) projected to increase, understanding its potential consequences on healthcare systems is crucial for adequately preparing society to address this growing challenge. In 2019, Malmö, Sweden's third-largest city, joined the Cities for Better Health global initiative to tackle the multifaceted challenges associated with T2D, including its significant economic burden on the healthcare system and the broader community. Understanding the economic burden of T2D on the healthcare system will facilitate optimisation of the initiatives undertaken b

Towards interventions on school dropouts for disabled learners amidst and post-COVID-19 pandemic.

BACKGROUND: Despite objective arguments for inclusive education, there is a dearth of mechanisms to reduce dropouts amongst disabled learners in the extant literature. Thus, this article is one of the outputs of a study, which was conducted after a consistent observation of dwindling numbers of disabled learners who succeed in basic education in South Africa. Of late, the dropout rate increased because of adherence to lock down regulations amidst the coronavirus disease 2019 (COVID-19) pandemic. This triggered the need for research on co-creating interventions to mitigate the rate of dropouts

Evaluation of an Artificial Intelligence-Augmented Digital System for Histologic Classification of Colorectal Polyps.

IMPORTANCE: Colorectal polyps are common, and their histopathologic classification is used in the planning of follow-up surveillance. Substantial variation has been observed in pathologists' classification of colorectal polyps, and improved assessment by pathologists may be associated with reduced subsequent underuse and overuse of colonoscopy. OBJECTIVE: To compare standard microscopic assessment with an artificial intelligence (AI)-augmented digital system that annotates regions of interest within digitized polyp tissue and predicts polyp type using a deep learning model to assist pathologi

Undisclosed Antiretroviral Therapy Use at Primary Health Care Clinics in Rural KwaZulu Natal South Africa: A DO-ART Trial Sub-study.

Accurate reporting of antiretroviral therapy (ART) uptake is crucial for measuring the success of epidemic control. Programs without linked electronic medical records are susceptible to duplicating ART initiation events. We assessed the prevalence of undisclosed ART use at the time of treatment initiation and explored its correlates among people presenting to public ambulatory clinics in South Africa. Data were analyzed from the community-based delivery of ART (DO ART) clinical trial, which recruited people living with HIV who presented for ART initiation at two clinics in rural South Africa.
