Displaced Radial Shaft Fracture: The Dorsal (Thompson) Approach to the Forearm.

This video demonstrates a displaced radial shaft fracture repaired through a dorsal (Thompson) approach to the forearm. The patient is an 18-year-old man who sustained a left elbow dislocation and ipsilateral radial shaft fracture (OTA/AO: 22-B2) while playing basketball. The patient underwent a closed reduction of the elbow joint and was indicated for operative repair of the radius. Ultimate fixation included a compression plate and nonlocking screws through a dorsal approach to the radius. Anatomic reduction and stable fixation was obtained. The posterior interosseus nerve was identified and…

Anomalous constitution of the brachioradialis muscle: a potential site of radial nerve entrapment.

The anatomical relationships of structures in the elbow region in the context of surgical approaches require to be studied carefully and diligently. The main aim of the present study is to report an accessory muscle in the elbow region which was detected during routine anatomical dissection and discuss its clinical importance. The muscle belly measured 4.8 x 2.1cm and it fused with the brachioradialis distally. Proximally, it was seen to blend with the deltoid and the brachialis muscles. The radial nerve was observed to interpose between this accessory muscle and the brachioradialis. This coul…

Arthroscopic synovectomy for hemophilic joint disease in a pediatric population.

Children with hemophilia can develop progressive arthropathy. Arthroscopic synovectomy has been used to reduce hemarthroses, but few long-term results have been published. In this article the authors review their first 12 years of experience. Data are reported on 44 pediatric patients (69 joints: 39 ankles, 21 elbows, 7 knees, 2 shoulders). The median age at surgery was 10 years Median follow-up was 79 months. Joints with sufficient follow-up data showed a median bleeding frequency decline of 84% (P < 0.001). Median arc of motion was stable or improved in the year after surgery in ankles, elbo…

A case of calcium pyrophosphate dihydrate crystal deposition disease presenting as an acute polyarthritis.

We report a case of calcium pyrophosphate dihydrate crystal deposition disease (CPDD) presenting as an acute polyarthritis. A 66-yr-old woman was admitted with a 5-day history of fever and multiple joint pain including wrists, elbows, shoulders, knees, and ankles developed 5 days before admission. Her plain radiographs of wrists, elbows, shoulders, knees, and ankles showed chondrocalcinosis. The pubic symphysis, lumbar intervertebral discs, and both hip joints, which were asymptomatic, also had calcium deposits. The compensated polarized microscopic examination of the joint fluid, aspirated fr…

Lichen nitidus of the palms: a case with peculiar histopathologic features.

Palmar involvement in lichen nitidus is infrequent. In such cases, the histopathologic findings of palmar lesions are usually identical to those of extrapalmar ones. We report on the case of a patient with multiple tiny papules located on the palms and elbows. A biopsy specimen from the elbow showed the typical features of lichen nitidus, but a biopsy from the palm disclosed an inflammatory infiltrate mostly disposed around the bases of rete ridges and composed of lymphocytes and histiocytes with some giant cells both in the dermis and in the epidermis. This location of the infiltrate is simil…

Bilateral congenital pseudarthrosis of the olecranon.

A case of bilateral congenital pseudarthrosis of the olecranon is reported in a 15-month-old boy. This abnormality was noted at birth because of a fixed flexion deformity of both elbows. The left side was operated on at the age of 16 months and the right side at 21 months. Surgical exploration showed a mobile pseudarthrosis with a 2-cm gap between the tip of the olecranon and the nucleus, each one covered with cartilage. This cartilage was excised, the triceps tendon was lengthened to allow the lowering of the nucleus, and both fragments were fixed with nylon threads. The elbow was kept extend…

Congenital insensitivity to pain with anhidrosis.

Congenital insensitivity to pain with anhidrosis is a rare disorder. Its primary clinical features include congenital analgesia, which leads to self-mutilation; inability to sweat, which leads to defective thermoregulation; and mental retardation. A five-year-old boy with consanguineous parents and no family history of the disorder presented with ulcerating lesions on both knees and elbows. His family had discovered the lack of sensation to pain and anhidrosis. Physical examination revealed ulcers on both knees and elbow, self-mutilation of the tongue, fingers, and toes. Sensation to touch was…

Postburn heterotopic ossification: insights for management decision making.

An analysis of 1,478 consecutive admissions to the University of Kansas Burnett Burn Center revealed 18 patients who developed heterotopic ossification (1.2% incidence). Seventeen patients developed this disease in the elbows; one patient had shoulder involvement. Two patients eventually developed bilateral elbow disease. All patients initially presented with pain and limitation of joint motion. The diagnosis was then confirmed radiographically. All patients suspected of heterotopic ossification received physical therapy consisting of active range of motion only. Ten patients responded to cons…

Bracing arms increases the capacity for sustained hyperpnea.

Patients with severe chronic obstructive pulmonary disease (COPD) frequently lean forward, bracing their arms. We wondered whether the resulting shoulder girdle support improves the function of the ventilatory pump. We tested this possibility in 4 normal men by measuring the maximal ventilation that they could voluntarily sustain for 4 min while seated with their elbows braced firmly on a table and while seated with their elbows held just above the table. Bracing the arms increased ventilatory capacity significantly in all subjects, but the magnitude of the change was small (8%). We attribute …

Association Between Upper Limb Dysfunction and Six-Month Postoperative Pain Following Mini-Open Synovectomy for Lateral Epicondylitis: A Retrospective Cross-Sectional Study.

Background Refractory lateral epicondylitis (LE) often requires surgical intervention; however, the extent to which postoperative upper limb dysfunction contributes to chronic pain remains unclear. In this study, we aimed to investigate the association between postoperative upper limb dysfunction and pain intensity at six months following mini-open synovectomy for LE. Methods This single-center, retrospective, cross-sectional study included 28 elbows that met the predefined eligibility criteria between January 2020 and December 2023. The primary outcome was pain intensity at six months postope…

Predictors of Outcome Following Tenotomy in Patients with Recalcitrant Epicondylopathy.

BACKGROUND: Elbow tendinopathies are associated with tenderness, pain, and functional disability with ensuing socioeconomic costs. There is lack of consensus regarding the best treatment for patients recalcitrant to first-line conservative treatments. Percutaneous needle tenotomy is considered a regenerative approach that injures the tendon to elicit a healing response. OBJECTIVE: To investigate whether demographic characteristics, clinical factors, baseline sonographic entities, or their interactions are related to the likelihood of responding positively to needle tenotomy over a 1-year foll…

[Long-term follow-up after intravenous injection of mercury--two cases report].

In this paper we present the long-term follow-up of two patients, after injection of metallic mercury. Case 1. In 1997, 29-years-old man injected himself to left elbow about 20 ml of metallic mercury by mistake (he was heroin abuser for short time). Mercury concentration in the blood was 400 microg/L. X-ray of the chest, abdomen and affected elbow area showed radiopaque foreign material (depots of mercury). Depots of mercury were also visible on the tricuspid valve in echocardiography. Mercury from the soft tissue left elbow pit was partially surgically removed. During 15 years follow-up two t…
