Potential phenomena in thalamus and cortex.

1. The cortical response to stimulation of an afferent path consists of two parts, a specific response indicating projection of the afferent and a response of the alpha mechanism. The former of these under the influence of strychnine can be identified with the cortical spikes of paroxysmal activity. 2. The response of the geniculate and other simply arranged nuclei are monophasic spikes. The diphasic spikes of the cortex and elsewhere are inferred to be assignable to two different groups of cells related by synaptic passage. 3. The unit cell potential may be interpreted as the resultant of une

Ventral Column Spinal Cord Stimulation for Pain Management: Mechanisms, Clinical Evidence, and Future Directions.

Spinal cord stimulation (SCS) is a form of neuromodulation that delivers electrical impulses to the spinal cord, altering pain signals to provide relief from chronic pain. Since its introduction in the 1960s, SCS has primarily targeted the dorsal columns of the spinal cord. While SCS has proven effective for many neuropathic pain conditions, its limitations have driven exploration of alternative anatomical targets for electrical stimulation. Work in the 1970's suggested that ventral column spinal cord stimulation (VC-SCS) might offer superior pain relief by targeting the spinothalamic tracts (

Transnasal ventral brainstem stimulation reverses anesthesia-induced unconsciousness in sheep.

BACKGROUND: Brainstem reticular formation (BRF) is crucially involved in regulating arousal, making it a potential target for reversing general anesthesia. The transnasal approach provides a minimally invasive access to the ventral brainstem. However, whether this approach can effectively modulate arousal remains unknown. OBJECTIVE: To evaluate the efficacy of transnasal BRF electrical stimulation in reversing anesthesia-induced unconsciousness. METHODS: Three 12-month-old male sheep (40-50 kg) were implanted epidural/bone electrodes close to ventral brainstem via a transnasal approach. Unde

Localizing seizure onset zones in surgical epilepsy with neurostimulation deep learning.

OBJECTIVE: In drug-resistant temporal lobe epilepsy, automated tools for seizure onset zone (SOZ) localization that use brief interictal recordings could supplement presurgical evaluations and improve care. Thus, the authors sought to localize SOZs by training a multichannel convolutional neural network on stereoelectroencephalography (SEEG) cortico-cortical evoked potentials. METHODS: The authors performed single-pulse electrical stimulation in 10 drug-resistant temporal lobe epilepsy patients implanted with SEEG. Using 500,000 unique poststimulation SEEG epochs, the authors trained a multic

Acupuncture activates a direct pathway from the nucleus tractus solitarii to the rostral ventrolateral medulla.

Our previous studies have shown that electroacupuncture (EA) at the Jianshi-Neiguan acupoints (P5-6, overlying the median nerve) attenuates sympathoexcitatory responses through its influence on neuronal activity in the rostral ventrolateral medulla (rVLM). The nucleus tractus solitarii (NTS) receives input from somatic nerve stimulation. Connections between the NTS and the rVLM during EA stimulation have not been investigated and thus were the focus of the present study. Seven to ten days after unilateral microinjection of a rhodamine-conjugated microsphere retrograde tracer (100 nl) into the

Genetic background dominates the susceptibility to ventricular arrhythmias in a murine model of β-adrenergic stimulation.

In cardiovascular research, several mouse strains with differing genetic backgrounds are used to investigate mechanisms leading to and sustaining ventricular arrhythmias. The genetic background has been shown to affect the studied phenotype in other research fields. Surprisingly little is known about potential strain-specific susceptibilities towards ventricular arrhythmias in vivo. Here, we hypothesized that inter-strain differences reported in the responsiveness of the β-adrenergic pathway, which is relevant for the development of arrhythmias, translate into a strain-specific vulnerability.

ARM-based visual processing system for prosthetic vision.

A growing number of prosthetic devices have been shown to provide visual perception to the profoundly blind through electrical neural stimulation. These first-generation devices offer promising outcomes to those affected by degenerative disorders such as retinitis pigmentosa. Although prosthetic approaches vary in their placement of the stimulating array (visual cortex, optic-nerve, epi-retinal surface, sub-retinal surface, supra-choroidal space, etc.), most of the solutions incorporate an externally-worn device to acquire and process video to provide the implant with instructions on how to de

Dorsolateral prefrontal cortex transcranial magnetic stimulation and electrode implant for intractable tinnitus.

OBJECTIVE: Tinnitus is a distressing symptom that affects up to 15% of the population; no satisfactory treatment exists. We present a novel surgical approach for the treatment of intractable tinnitus based on electrical extradural stimulation of the dorsolateral prefrontal cortex via an electrode implant. Tinnitus can be considered an auditory phantom phenomenon similar to deafferentation pain in the somatosensory system. It is characterized by gamma-band activity in the frontal cortex that can be visualized with the use of electroencephalography, magnetoencephalography, and functional magneti

Projection of cardiovascular afferents to the lateral nodulus-uvula of the cerebellum in rabbits.

Vagal and aortic afferent projections to the nodulus and uvula of the cerebellar vermis were examined in anesthetized and paralysed rabbits. Electrical stimulations of vagal and aortic nerves at a frequency of 1/s produced field potentials in the lateral cortical region of the contralateral nodulus-uvula with a latency of 6.5-19 ms. These potentials were negative in the molecular layer and positive in the granular layer. At a higher stimulation rate (30/s), these potentials were reduced by 60-65% of the control value. These results indicate that the lateral cortical region of the nodulus-uvula

Identification of the midbrain locomotor region and its relation to descending locomotor pathways in the Atlantic stingray, Dasyatis sabina.

The midbrain locomotor region (MLR) in the Atlantic stingray, Dasyatis sabina, was identified and characterized. Stimulation (50-100 microA, 60 Hz) of the midbrain in decerebrated, paralyzed animals was used to elicit locomotion monitored as alternating activity in nerves innervating an antagonist pair of elevator and depressor muscles. Effective sites for evoking locomotion in the midbrain included parts of several nuclei: the caudal portion of the interstitial nucleus of the medial longitudinal fasciculus and the caudomedial parts of the cuneiform and subcuneiform nuclei. This region did not

Characterization of descending modulation of nociception from the A5 cell group.

The present study examined the role of the A5 catecholamine-containing cell group in descending modulation of the nociceptive tail-flick (TF) reflex and regulation of blood pressure and heart rate in rats lightly anesthetized with pentobarbital. Systematic mapping studies throughout the A5 cell group, rostral to caudal, showed that electrical stimulation in and near the A5 cell group at intensities as low as 25 microA was sufficient to inhibit the tail-flick (TF) reflex without producing a significant pressor response. Microinjections of glutamate into the same sites to selectively activate ce

Mapping of ventricular tachycardia induced by thoracic neural stimulation in dogs.

To investigate ventricular tachycardias produced in healthy canine myocardium by stimulation of sympathetic ganglia or cardiac nerves, we simultaneously recorded a surface ECG and 63 ventricular electrograms in anesthetized open-chest dogs. Isochronal and isopotential maps were generated off-line by computer. Ventricular tachycardia with uniform beat-to-beat morphology was induced in 13 or 22 dogs by electrical stimulation of the left stellate ganglion (five experiments), the left middle cervical ganglion (four experiments), the left caudal pole cardiopulmonary nerve (two experiments), or the

Stimulation of the human ventral tegmental area increases strategic betting.

Learning is a fundamental aspect of human behaviour and is essential for adapting to new environments and situations. The ventral tegmental area is a critical brain area containing neurons that release dopamine to signal reward, drive learning and bias decision-making. Human data on the ventral tegmental area's effects on cognition are scarce, and no studies have causally manipulated the human ventral tegmental area. Here we studied a unique group of patients who had deep brain stimulation surgery in the ventral tegmental area to improve pain due to trigeminal autonomic cephalalgias refractory

Multisite Hebbian Plasticity Restores Function in Humans with Spinal Cord Injury.

OBJECTIVE: Spinal cord injury (SCI) damages synaptic connections between corticospinal axons and motoneurons of many muscles, resulting in devastating paralysis. We hypothesized that strengthening corticospinal-motoneuronal synapses at multiple spinal cord levels through Hebbian plasticity (i.e., "neurons that fire together, wire together") promotes recovery of leg and arm function. METHODS: Twenty participants with chronic SCI were randomly assigned to receive 20 sessions of Hebbian or sham stimulation targeting corticospinal-motoneuronal synapses of multiple leg muscles followed by exercise
