We report two singular cases of sudden death in which the cause was advanced tuberculosis infection. In the first case, a 24-year-old man died suddenly following massive hemoptysis due to erosion of the pulmonary vessels. The second case involved a 29-year-old man who died unexpectedly from asphyxia secondary to hemoptysis caused by fibrocavitatory tuberculosis. Toxicological screening and HIV testing were negative in both cases. Medico-legal autopsy, combined with detailed histological examination, was essential to determine the exact cause of death.

BACKGROUND Post-radiation angiosarcoma (PRA) is a rare, invasive mesenchymal tumor associated with prior cancer radiotherapy. The diagnostic criteria for PRA include a history of radiation exposure, the development of a new malignant tumor with a long latency period within the irradiated field, and a histological type different from that of the primary malignancy if radiation was administered for the original cancer. Although PRA can arise in any part of the body, it most commonly occurs in the skin. Only a few cases of vascular sarcoma secondary to radiotherapy for cervical cancer have been r

Lennox-Gastaut syndrome is a form of severe childhood epilepsy caused by a variety of etiologies including structural abnormalities, inflammatory conditions affecting the brain, malignancy, injury, or unknown causes. Sodium channelopathies have been linked to multiple seizure disorders, including Lennox-Gastaut syndrome, due to their role in action potential propagation in the brain. SCN2A is one such voltage gated sodium channel found primarily in the central nervous system. We present a case of a 6-year-old female with Lennox-Gastaut syndrome and a heterozygous SCN2A variant who became unres

BACKGROUND: Mutations in the MECOM gene have been recognized as a causative factor in MECOM-associated syndrome, which encompasses a spectrum of hematologic and extra-hematologic manifestations. Hematologic features range from isolated thrombocytopenia to severe bone marrow failure, while extra-hematologic manifestations may include skeletal, cardiac, renal, and other abnormalities. Here, we present a case of a Han Chinese newborn with a previously unreported variant in the MECOM gene.
CASE PRESENTATION: We report a 0-day-old female Han Chinese neonate who presented with severe thrombocytopen

RATIONALE: While secondary hepatic lymphoma can develop in up to 50% of patients with non-Hodgkin lymphoma and approximately 20% of those with Hodgkin disease, it is uncommon for patients to present with severe hepatic involvement, manifesting initially as jaundice and abdominal pain. These nonspecific symptoms can complicate diagnosis and subsequently delay the initiation of appropriate treatment.
PATIENT CONCERNS: A 68-year-old female presented to our hospital with complaints of abdominal pain and jaundice.
DIAGNOSES: Laboratory tests and clinical symptoms are often nonspecific for this at

INTRODUCTION: Guillain-Barré syndrome (GBS) is a disease affecting the peripheral nerves, leading to a constellation of motor, sensory, and autonomic symptoms. It typically has a monophasic course, and recurrence is the exception rather than the case, so much is not known about it. We discuss the risk factors, presentation, and course of patients with recurrent Guillain-Barré syndrome.
CASE PRESENTATION: A 28-year-old Ethiopian male presented with progressive ascending weakness that started from the lower extremities but spread to involve his respiratory muscles and resulted in respiratory fa

While early reports of CRTC1::TRIM11 cutaneous tumors suggested a predominantly indolent clinical course, a few cases have been documented with lymph node and distant metastases. We report herein a metastasizing tumor with a lethal outcome. It presented as a subcutaneous mass on the back of a 17-year-old teenager. Histopathologically, the tumor was subepidermal and composed of amelanotic spindle and epithelioid cells with nuclear pleomorphism. The tumor cells were immunoreactive for Sox10 and S100, focally positive for Melan-A, and negative for PRAME. Whole genome and whole transcriptome seque

A 71-year-old male patient with multiple liver and lymph node metastases of gallbladder cancer was diagnosed with gallbladder neuroendocrine carcinoma based on liver tumor biopsy. He received chemotherapy with attention to the medical history of interstitial pneumonia and achieved a partial response. The patient died of acute exacerbation of interstitial pneumonia 12 months after diagnosis. A pathological autopsy was performed which confirmed the diagnosis of primary neuroendocrine carcinoma of the gallbladder. The autopsy revealed no adenocarcinoma component, indicating the possibility of tra

Post-transplant lymphoproliferative disorder (PTLD) affects from 0.8-2.5% of kidney transplant patients, with peaks in incidence within 1 year and 10-14 years post-transplant. Very late-onset PTLD (VL-PTLD) can occur over 10 years post-transplant, reportedly up to 30 years, but its characteristics remain unclear. A 61-year-old man developed PTLD 45 years after kidney transplant, presenting with fever and shortness of breath. Computed tomography revealed lung and liver masses and biopsy-confirmed stage IVB monomorphic PTLD. Despite obtaining an initial improvement, the patient died on day 66. V

The uncommon malignant dysontogenetic neoplasm known as pleuropulmonary blastoma (PPB) typically affects children. It forms a part of the pulmonary sarcomatoid carcinoma family and is frequently distinguished by a biphasic form with both mesenchymal and epithelial components. Here, we describe two highly unusual adult PPB cases. We report here two cases of PPB, both presenting with cough and chest pain with a bulky disease in lung. The diagnosis of PPB was made by combining imaging, histological, and clinical data. The disease showed an aggressive course with both patients dying due to the dis

Esophageal metastasis from breast cancer is rare and seldom diagnosed before death, with no established standard treatment. We report 3 cases managed at our institution. Case 1:A 78-year-old woman previously underwent surgery for invasive ductal carcinoma(ER+, PgR+, HER2-)at the age of 65. Eight years postoperatively, the first recurrence occurred, followed by progressive dysphagia 5 years later, leading to the diagnosis of esophageal metastasis. Case 2:A 63-year-old woman had surgery at age 48 for invasive ductal carcinoma(ER+, PgR+, HER2-). Fourteen years postoperatively, she experienced a r

Elasmobranchs, including sharks and rays, are commonly housed in public aquariums due to their ecological significance and educational value. The common guitarfish (Rhinobatos rhinobatos), currently listed as 'Critically Endangered' by the IUCN, is particularly susceptible to population declines due to overfishing and bycatch. While generally considered robust, individuals in captivity may experience stress-related health issues, increasing their susceptibility to infectious diseases. This study investigates a mortality event affecting three 20-year-old guitarfish kept in a public aquarium. Th

Diffuse midline glioma, is a highly aggressive deep-seated glioma whose diagnosis must be confirmed through histopathological analysis of stereotactic biopsies. Hemorrhagic complications after intracranial biopsies may occur, potentially leading to severe neurological sequelae or significantly altering the outcome. A 55-year old male with no significant medical history presented to the local emergency department with 4 days of diplopia. A magnetic resonance imaging (MRI) confirmed the presence of tumor whose characteristics were highly suggestive of a high-grade infiltrating causing blocked hy

This case report describes a patient with a medical history of schizophrenia, found in a coma with hyperthermia, likely due to classic heatstroke. The white blood cells observed on the blood smear showed cytological abnormalities characterized by multilobed nuclei, which could be early signs of cell death. The evolution into multiorgan failure led rapidly to death.

BACKGROUND Agenesis of the dorsal pancreas (ADP) is a rare congenital anomaly resulting from incomplete development of the dorsal pancreatic bud, affecting the body and tail of the pancreas. While often asymptomatic, it can present with clinical abnormalities such as abdominal pain, diabetes, or obstructive jaundice, and may be associated with chronic pancreatitis and tumors. We report the third documented case of a high-grade neuroendocrine tumor located at the head of the pancreas in a patient with complete ADP, along with a review of the literature. CASE REPORT An 86-year-old man presented

The sternum, essential for protecting intrathoracic structures and enabling thoracic stability and breathing motion, is a vital part of small animal thoracic anatomy. Evaluating the sternum is crucial in small animal practice, with typical anatomy including eight to nine sternebrae and species-specific variations. Congenital sternal anomalies, such as pectus excavatum, pectus carinatum, and sternal cleft, though rare, can have significant clinical implications. Sternal cleft, characterized by incomplete fusion of sternebrae, is particularly uncommon in veterinary literature, with limited docum

Ewing sarcoma (ES) is a rare, aggressive malignancy seldom found in the urinary bladder. We report a 33-year-old male diagnosed with extra-skeletal ES of the bladder following symptoms initially attributed to prostatitis. Despite multimodal therapy-chemotherapy, radiation, and partial cystectomy-he developed early widespread metastases and died within a year. Pathology revealed positive surgical margins and limited treatment response. This case highlights the diagnostic and therapeutic complexity of bladder ES, the importance of surgical margin status, and the patient's autonomy in surgical de

Methotrexate (MTX) is a versatile drug used in various fields, including oncology and rheumatology, acting by inhibitory effects on DNA synthesis and cellular replication. While low-dose MTX is a first-line treatment for rheumatoid arthritis and psoriasis, overdose or incorrect dosing can lead to severe toxicity. We present three cases of accidental MTX poisoning, highlighting the diverse clinical manifestations and challenges in management. Patients presented with fever, weakness, mucocutaneous lesions, and pancytopenia, necessitating intensive care and supportive treatment. These cases under

A 9-year-old female spayed boxer presented for a dorsal cranial mass. Cytology diagnosed a sarcoma. Contrast-enhanced computed tomography (CT) of the head revealed a symmetrical, soft tissue attenuating mass with subtle mineralization and a contrast-enhancing capsule at the dorsal calvarium. There was adjacent frontal bone lysis and contrast enhancement of the parietal lobe. This supported a sarcoma of bone or cartilage origin. Postmortem examination additionally revealed a pulmonary tumor embolus. A final diagnosis of grade III multilobular osteochondrosarcoma (MLO) was made. This is the firs
