Functional recovery after open talar neck fracture with foot dislocation: A case report.

We report a Gustilo-Anderson type IIIA open talar neck fracture (Hawkins type II) with medial foot dislocation after a fall from a ladder. Treatment consisted of single-stage surgical management including meticulous irrigation and debridement, anatomical reduction with screw fixation, combined with systemic antibiotics, local vancomycin application, plaster cast immobilization, and adjunctive hyperbaric oxygen therapy. Fracture healing proceeded without major complications, and screw removal was performed at 1½ years due to local irritation. At final follow-up, 3½ years after injury, the patie…

Foot function affects pitching motion: players with floating toes show inconsistent pitching motion.

The purpose of this study was to evaluate how floating toes affect baseball pitching motion. Fifteen young male baseball players pitched from a pitching mound after physical examination, including floating toes, determining if any toes are not in contact with the ground. At least 20 pitches were thrown and the 10-best pitches were selected for the analysis. Images of the ball release point for sagittal and coronal views were captured from the videos recording the pitching motion. The positions of the ball, wrist, and leading foot were measured and the variability of each position over the 10 p…

Enhancing diabetic foot osteomyelitis diagnosis with metagenomics next-generation sequencing, proof of concept.

Diabetic foot osteomyelitis (DFOM) is a serious medical condition that necessitates robust diagnostic tools for effective clinical management. Conventional diagnostic methods for DFOM rely heavily on bacterial culture, which is time-consuming and may fail to capture the full microbial diversity present in infections. This pilot study explored the utility of metagenomics next-generation sequencing (mNGS) as a complementary diagnostic tool for DFOM. We retrospectively analysed ten bone biopsies from nine diabetic persons using both routine microbiological culture and mNGS. Routine culture identi…

Subtalar joint dynamics in progressive collapsing foot deformity: A cross-sectional progression model.

BACKGROUND: Peritalar subluxation (PTS) contributes to progressive collapsing foot deformity (PCFD), yet its pathophysiology remains unclear. This study analyzed subtalar joint (SJ) dynamics throughout deformity progression. METHODS: A retrospective review of 153 WBCT scans classified PCFD into four stages: (1) no PTS, (2) PTS without impingement, (3) PTS with sinus tarsi impingement (STI), and (4) PTS with subfibular impingement (SFI). Three-dimensional measurements of middle facet subluxation and incongruence, foot-ankle offset, SJ offset, and lateral process offset were compared between gr…

Demineralized Bone Matrix in Foot and Ankle Surgery: Products, Mechanisms, and Clinical Outcomes.

BackgroundBone grafting is a commonly used adjunct in foot and ankle surgery to enhance osseous healing in fracture fixation and arthrodesis. Although autologous corticocancellous bone is commonly used, donor site morbidity and limited graft availability have driven the development of allogenic alternatives such as demineralized bone matrix (DBM).FormulationsDemineralized bone matrix is produced through decalcification of donor bone, preserving a collagen-rich scaffold and biologically active proteins that support osteoconduction and osteoinduction. Commercial formulations vary in carrier syst…

Targeting the gut microbiota: emerging strategies to enhance healing of diabetic foot ulcers.

Diabetic foot ulcer (DFU) affects up to 34% of diabetic patients, with a 1-year recurrence rate of approximately 40%. This review primarily focuses on type 2 diabetes mellitus (T2DM), the most common form of diabetes associated with DFUs. Emerging evidence shows that gut microbiota critically influences DFUs healing through immune modulation (e.g., Treg/Th17 balance), regulation of inflammatory responses via short-chain fatty acids (SCFAs) that inhibit NF-κB, the gut-immune-skin axis, and systemic effects of microbial metabolites. Microbiota-targeted interventions-probiotics, prebiotics, fecal…

Next-generation multifunctional scaffolds for diabetic foot ulcers: Harnessing moringa bioactives and nanotechnology.

Diabetic Foot Ulcers (DFUs) are a serious global health crisis, mainly because of persistent bacterial biofilm formation and the rise of Methicillin-Resistant Staphylococcus aureus (MRSA) infections. Current therapeutic strategies are less effective and fail to resolve the core challenges of multidrug resistance and defective tissue regeneration. These limitations require the development of multifunctional therapeutic systems. This review discusses a dual-action platform - Moringa based smart scaffolds. These scaffolds are designed to provide a targeted solution for MRSA infected DFUs. The the…

Preliminary evaluation of a novel serotype O foot-and-mouth disease mRNA vaccine.

Foot-and-mouth disease virus (FMDV) is one of the most significant animal pathogens worldwide, severely impacting the health and productivity of pigs, cattle, sheep, and other ungulates. Although the traditional vaccines have played a crucial role in epidemic control, inactivated vaccines face persistent challenges concerning the potential for virus dissemination and pressures from serotype and subtype matching. However, the manufacture of attenuated vaccines is forbidden, and the efficiency of alternative vaccines for immune protection is still inadequate. Consequently, there exists an urgent…

Relationship between 3D lower-limb kinematics and foot propulsive force during breaststroke kicking.

In breaststroke, leg kick propulsion plays a major role in swimming velocity; however, the relationship between propulsive force and its kinematics remains underexplored. This study aimed to identify leg movements that enhance foot propulsive force. Fourteen male swimmers performed 10 seconds of maximal-effort breaststroke kicks while holding a board with their upper limbs in a swimming flume. Leg kinematics and foot propulsive force were measured using a motion-capture system and eight pressure sensors on the left foot. During the knee extension phase, foot propulsive force correlated signifi…

Impact of weightbearing on progressive collapsing foot deformity shape : a geometric morphometric analysis.

AIMS: Weightbearing CT (WBCT) has set a new standard for the assessment of foot and ankle alignment in patients with progressive collapsing foot deformity (PCFD) under physiological loading conditions compared with conventional CT. Principal component analysis (PCA) models are currently used for a detailed 3D shape analysis, but are not able to take into account non-linear (e.g. rotational) anatomical variance, which is particularly relevant in PCFD. Innovative advances in geometrical morphometrics by principal polynomial shape analysis (PPSA) are now able to overcome this challenge. Therefore…
