A two-year-old female alpaca suddenly became lame on its right hindlimb. Eight days later clinical and radiographic examinations showed that the patella had become temporarily fixed within the supratrochlear notch of the femur. Under general anaesthesia an abrasion trochleoplasty followed by fascial imbrication was carried out. After two weeks in supporting slings, the animal put full weight on the leg, and six months after the surgery it showed no signs of lameness or recurrence of the upward fixation.

A 71-year-old man was admitted to the hospital because of general fatigue. There were few reticulocytes in the peripheral blood and no erythroblasts in the bone marrow. Chest CT revealed an anterior mediastinal tumor. Under a diagnosis of thymoma with PRCA, extended thymothymectomy was performed. Histological diagnosis was mixed type thymoma with no invasive growth beyond the capsule. Administration of predonisolone following surgery was not effective for PRCA. Otherwise, peripheral blood counts were significantly improved following occasional onset of acute bronchitis.

An 81-year-old woman with unintentional salicylate intoxication presented with features of sepsis, abdominal pain, and tenderness. Laparotomy was performed to rule out acute cholecystitis. Anesthesia was complicated by severe hypercarbia despite hyperventilation, and progressive cardiovascular and neurologic deterioration postoperatively. The adverse neurologic, respiratory, and hepatic effects of abdominal surgery and general anesthesia probably potentiated salicylate toxicity and increased patient morbidity. There are a number of different approaches available to surgically remove the adrenal gland. These can be broadly classified into two general categories, open and laparoscopic. There is no one best method for all patients. Surgeons, skilled in all aspects of adrenal surgery, should choose an approach for adrenalectomy based on patient and tumor related factors. Once advantages and disadvantages of a specific procedure, with these factors in mind, are weighed, the best approach for the individual patient should be chosen.

A case of extramedullary plasmacytoma with its unusual appearance is reported. This is the second reported case in world literature affecting the paranasal sinuses with intracranial extension. The role of surgery is to obtain tissue for diagnosis and to excise residual disease. Radiotherapy is the treatment of choice and long-term follow-up is necessary for monitoring disease recurrence. The overall 10-year survival is about 50%. The case is discussed with a general review of the management of this pathology.

A case of spontaneous luxation of the globe and Crouzon's disease is described. The luxation was reduced under general anaesthesia. Bilateral permanent tarsorrhaphies were performed to prevent further luxations and to decrease the lagophthalmos and corneal exposure. There was also mild bilateral optic atrophy and a left superior oblique palsy. The patient exhibited turricephaly, maxillary hypoplasia and shallow orbits with proptosis. Major reconstructive surgery is not indicated as the patient is already 9 years old and has no mental impairment.

Bioresorbable barrier material, used in guided tissue regeneration, changes the conventional two-stage surgery to a single-step procedure. This article presents a general surgical protocol for application of a bioresorbable matrix barrier (Guidor AB, Huddinge, Sweden). Three case reports are used to illustrate the surgical treatment of a Class II furcation defect, an intrabony defect, and a recession type defect. The learning objective of this article is to enhance the knowledge of surgical procedures and bioresorbable barrier membranes available.

General anaesthesia influences the oxygen supply/demand ratio of the brain and may thus be able to improve neurological outcome after carotid artery surgery. Among the anaesthetic agents, barbiturates and isoflurane produce a marked reduction of the cerebral metabolic rate and activity. But the incidence of neurological deficits in the immediate postoperative period is low, while the possible causes of any neurological deficits are multiple. Consequently, to date, no particular anaesthetic agent or management has been shown to influence the neurological results.

Hip sonography provides a safe pathoanatomical assessment of a newborn hip joint at the earliest possible moment. Based on this safe diagnosis an adequate biomechanical treatment can be started instantly. The mean age of healing even of originally decentered hip joints is 7.5 months, if earliest sonographic diagnosis and adequate biomechanical treatment are performed correctly. Secondary hip surgery can be reduced to a minimum. Cost-benefit-analysis, too, supports the institution of a general sonographic screening of all newborn hip joints.

Nineteen prepubertal children with anogenital (AG) warts were treated by scissor excision plus electrocautery under general anesthesia. Median posttreatment follow up was nine months. Minor clinical recurrences were seen in five (26.3%) children, all within three months after treatment. Recurring warts responded in all cases to home application of 0.5% podophyllotoxin (Condyline). Surgery plus electrocautery was well tolerated with no notable side effects. It is simple, safe, and efficacious, and is a suitable second-line treatment for AG warts in children.

Most defects in dermatologic surgery can be repaired by primary closure. If this is not feasible, then local flaps should be employed. Where flaps are unfeasible or would result in an additional cosmetic deformity, then a full-thickness skin graft should be employed. If the defect is too large for a full-thickness skin graft, then a split-thickness skin graft is the appropriate choice. Individual surgical situations or the medical status of the patient may alter these general principles.

From January 1983 to December 1984 in the Division of Pediatric Surgery of Parma, 112 endoscopies of upper gastro-intestinal tract (UGT) were done. Besides the authors refer 49 endoscopies of the colon. Endoscopic indications of the UGT included evaluation of the esophagus in gastro-esophageal reflux, achalasia, lye ingestion, gastro-duodenal ulcer, bleedings of UGT, removal of foreign bodies. The indication of the colonoscopy included bleedings and all symptoms of colitis. The endoscopic procedures were done under sedation or in general anesthesia.

Although aneurysms of the common iliac artery are a rare occurrence they should be investigated routinely in patients with urinary or thrombo-embolic venous manifestations in the pelvis minor. Clinical signs are inconstant and diagnosis requires the use of abdominopelvic ultrasound tomography. Venous and urinary complications constitute warning signals suggesting the need for surgery to avoid sudden or untimely rupture. Operation in patients with a poor general condition should be performed as a function of the course of the lesion.

In Switzerland the amount of blood donations is determined by the demands for plasma; as a result there is a surplus of erythrocyte concentrates for which more uses should be found. Trials in a regional general hospital proved that these concentrates can be advantageously employed in the field of surgery and anasthesia: due to close cooperation between the clinical department and the blood donor sevice the demand for erythrocyte concentrates rose from 5 per cent to 80 per cent within 5 years.

Bronchiectasis is a clinical syndrome characterized by coughing and sputum production in the presence of abnormal thickening and dilatation of the bronchial walls.
