Loss of UBE3C-mediated PIM2 ubiquitination drives Helicobacter pylori-induced gastritis via JAK2/STAT3 activation.

Helicobacter pylori (H. pylori) infection is a major cause of chronic gastritis, yet its molecular mechanisms remain incompletely understood. In this study, we found that the proviral integration site for Moloney murine leukemia virus kinase 2 (PIM2) was significantly upregulated in H. pylori-infected gastric tissues and cells. Functionally, PIM2 knockdown not only attenuated inflammatory responses both in vivo and in vitro but also significantly reduced gastric H. pylori colonization in mice. Mechanistically, H. pylori infection downregulated the expression of the E3 ubiquitin ligase UBE3C, l…

Helicobacter pylori Eradication Rates among Patients Undergoing Endoscopy Remain Stagnant Despite Evolving Clinical Practice Patterns.

OBJECTIVES: Helicobacter pylori is the strongest risk factor for non-cardia gastric adenocarcinoma, the cancer with the greatest racial disparity in US cancer mortality. The 2017 American College of Gastroenterology guidelines recognized that local H pylori antibiotic resistance data are often unavailable in the United States, highlighted increasing clarithromycin resistance, and recommended that providers review previous antibiotic exposures before selection of an H pylori treatment regimen. METHODS: We analyzed a retrospective cohort of 477 patients with H pylori diagnosed on upper endoscop…

Proteomic remodeling of outer membrane vesicles in antibiotic-resistant Helicobacter pylori reveals adaptive stress responses.

BACKGROUND: Antibiotic resistance in Helicobacter pylori has become a major obstacle to successful eradication therapy worldwide. Although outer membrane vesicles (OMVs) are increasingly recognized as key mediators of bacterial adaptation and host interaction, their contribution to antibiotic resistance in H. pylori remains poorly understood. METHODS: Metronidazole and levofloxacin-resistant derivatives of the H. pylori G27 strain were generated by stepwise antibiotic exposure. OMVs isolated from susceptible and resistant strains were characterized by transmission electron microscopy and dyna…

Helicobacter pylori-induced cystathionine-γ-lyase drives redox imbalance and metabolic remodeling in gastric cancer.

BACKGROUND: Helicobacter pylori (H. pylori) infection is a major risk factor for gastric cancer (GC) and depends on nutrient and energy metabolism, particularly amino acids, for colonization. In host cells, cysteine is regulated by the transsulfuration (TSS) pathway, in which cystathionine-γ-lyase (CSE) generates cysteine, hydrogen sulfide (H2S), and reactive sulfur species (RSS). Dysregulation of this pathway may contribute to gastric carcinogenesis. This study aims to investigate the role of CSE in H. pylori-associated GC and to characterize the associated alterations in sulfur metabolism an…

Prevalence of Helicobacter pylori infection among gastritis and dyspeptic patients in Jos, Plateau State, Nigeria.

BACKGROUND: Helicobacter (H.) pylori infection remains a major cause of gastritis and dyspepsia globally, with disproportionately high prevalence in developing regions, including parts of Africa where infection rates often exceed 70%. In many of these settings, limited region-specific data on prevalence and antimicrobial resistance hinder effective diagnosis, treatment, and control strategies. OBJECTIVE: This study investigated the prevalence, molecular characteristics, and antibiotic susceptibility patterns of H. pylori among patients presenting with gastritis and dyspepsia in Jos, Plateau S…

Harnessing plant-derived bioactive compounds as LOX-1 inhibitors in Helicobacter pylori-driven gastric inflammation.

Helicobacter pylori persistence and rising antibiotic resistance have intensified interest in host-directed adjunctive strategies. Recent studies identify LOX-1 as a gastric epithelial receptor for H. pylori catalase and support its role in bacterial adhesion, making it a plausible host-directed target. In the current study, BI-0115, a structurally defined selective LOX-1 inhibitor, was used for functional validation in gastric epithelial cells infected with two clinical H. pylori isolates. Since LOX-1 is linked to inflammatory signalling pathways, we investigated the phosphorylation of p38-MA…

Helicobacter pylori-induced ADAMDEC1 overexpression in M2 macrophages drives preneoplastic progression in the gastric mucosa.

Helicobacter pylori ( H. pylori) infection is a major risk factor for gastric cancer, though the immune microenvironmental factors driving preneoplastic transformation remain unclear. In this study, single-cell RNA sequencing was used to characterize macrophage subsets and identify key genes associated with H. pylori-induced gastric lesions. Molecular, cellular, and in vivo assays were conducted to determine the mechanisms regulating the expression of a disintegrin and metalloproteinase-like decysin 1 ( ADAMDEC1) and its downstream effects. M2 macrophages were significantly enriched in H. pylo…

Role of Nod-like Receptors in Helicobacter pylori Infection: Insights into Innate Immune Signaling Pathways.

Helicobacter pylori is a prevalent gastric pathogen that establishes chronic infection and contributes to gastritis, peptic ulcer disease, and gastric cancer. Its persistence depends on immune evasion strategies that promote sustained low-grade inflammation in the gastric mucosa. Nucleotide-binding oligomerization domain-like receptors (NLRs) are cytosolic pattern recognition receptors that play key roles in innate immune responses against H. pylori. Nod1 and Nod2 detect bacterial peptidoglycan delivered via the type IV secretion system or outer membrane vesicles, activating NF-κB, MAPK, and i…

AI-assisted endoscopic detection of Helicobacter pylori infection in atrophic gastritis: a multicenter diagnostic study.

BACKGROUND: Convolutional neural network (CNN)-based artificial intelligence systems have demonstrated promise in detecting Helicobacter pylori (Hp) infection in patients with preserved gastric mucosa; however, their performance in atrophic gastritis (AG), a precancerous condition with complex mucosal changes that can obscure typical endoscopic features, remains unclear. METHODS: This retrospective multicenter cross-sectional study included 22,986 patients (464,480 endoscopic images) from four clinical centers. A CNN model was trained and optimized using a dedicated training and validation se…

A Broad Scope of Differentials: Helicobacter pylori-Associated Chronic Gastritis Mimicking a Gastrointestinal Stromal Tumor.

Chronic gastritis is a common manifestation of Helicobacter pylori infection. Clinical presentations can range from epigastric discomfort to gastrointestinal bleeding. The clinical course and gross endoscopic findings can mimic alternative diagnoses such as gastric adenocarcinoma and gastrointestinal stromal tumors (GISTs). We report a case of H. pylori-associated chronic gastritis that initially presented with frank melena. Subsequent blood tests demonstrated a drop in hemoglobin and a rise in serum urea. The case was managed according to the hospital's upper gastrointestinal bleed protocols.…

Vitamin C-deficient gulo-/- mice exhibit increased susceptibility to Helicobacter pylori colonization and gastric pathology.

Low dietary intake of vitamin C (ascorbic acid) has been associated with unfavorable clinical outcomes of Helicobacter pylori infection in human studies. However, experimental evidence regarding the influence of host vitamin C status on H. pylori colonization remains inconsistent across animal models. In this study, we generated vitamin C-deficient L-gulono-γ-lactone oxidase-deficient (gulo-/-) mice on an FVB genetic background to evaluate host susceptibility to H. pylori infection under defined vitamin C supplementation conditions. FVB gulo-/- mice were orally infected with H. pylori and prov…

New Insights into CRISPR-like Arrays in Helicobacter pylori: An Exploratory Analysis from Genomic Data.

Helicobacter pylori (H. pylori) is a highly adaptable gastric pathogen with marked genomic plasticity. Whilst functional CRISPR-Cas systems provide adaptive immunity in many bacteria, they have not been identified in H. pylori, unlike CRISPR-like sequences. In this study, eight H. pylori genomes were analysed using the bioinformatics tools CRISPRCasFinder, CRISPRCasTyper, and CRISPRloci. A total of 25 CRISPR-like arrays were identified, showing high conservation (88%) both between and within strains, suggesting that these arrays are not random remnants but rather organised structures possibly …

The quantitative proteomics analysis of multi-stage gastric mucosal lesions associated with Helicobacter pylori infection.

BACKGROUND: Helicobacter pylori (H. pylori) infection is the principal etiological factor for gastric cancer (GC), which ranks as the fourth leading cause of cancer-related mortality globally. Although the Correa cascade describes the stepwise progression from chronic gastritis to intestinal-type GC, the molecular events underlying this "inflammation-to-cancer" transition remain incompletely characterized. OBJECTIVE: This study aimed to delineate stage-specific proteomic alterations across H. pylori-associated gastric mucosal lesions and to identify candidate progression-associated biomarkers…

Integrated virulence-resistance profiling of Helicobacter pylori reveals context-dependent pathogenic signatures in gastric cancer.

BACKGROUND: Helicobacter pylori contributes to gastric carcinogenesis through diverse virulence mechanisms, transcriptional plasticity, and rising antimicrobial resistance. However, how these features integrate into coordinated bacterial states within the tumor microenvironment remains unclear. This study integrates virulence, expression, and antimicrobial signatures to define potentially meaningful H. pylori profiles. METHODS: We analyzed 159 histologically confirmed, H. pylori-positive gastric cancer tissues. Virulence gene presence, expression, and antimicrobial resistance-associated mutat…

Autophagy-driven modulation of stem cell dynamics in Helicobacter pylori-induced gastric diseases in mice.

Chronic Helicobacter pylori infection is a major contributor to gastric disease progression, with its involvement in autophagy and stem cell dynamics playing a critical role in disease mechanisms. This study investigated how H. pylori, particularly in combination with the carcinogen N-nitroso-N-methylurea (NMU), disrupted autophagy and stem cell function, driving gastric pathology. H. pylori infection significantly increased autophagy, promoted the epithelial-mesenchymal transition, suppressed Tff2 and Ghrelin expression in mouse gastric organoids, and enhanced stem cell proliferation (organoi…
