Treatment of patients with chronic idiopathic urticaria (CIU) involves reducing symptoms with the least invasive therapy and carefully balancing risk and benefit. The mainstay of therapy is the use of antihistamines with or without the use of intermittent pulses of corticosteroids. Reported is a patient with a clinical syndrome characteristic of scombroid fish poisoning after ingesting yellowfin tuna that may have been allowed to sit at room temperature for some time before preparation. The patient was treated with an intravenous infusion of cimetidine with prompt resolution of a diffuse, well demarcated, erythematous, pruritic rash. The treatment was without sequelae and permitted early discharge from the emergency department. A brief review of scombroid fish poisoning and its treatment is provided.

The authors studied ranitidine effects on the function of glucocorticoid receptors (GR) of gastric cytosol in patients with massive bleeding from gastric ulcer. To determine whether lack of healing with standard treatment with H2 antagonists might be due to abnormal gastric secretion, 30 patients who remained unhealed after 12 weeks' therapy with histamine H2 antagonists were compared with 20 patients who healed after 6 or 12 weeks' therapy. Serum immunoreactive gastrin is raised in some patients with rheumatoid arthritis (RA). In this paper paired samples taken within 30 minutes of each other show that this phenomenon is reproducible.
