[Comparison of the effects of child-centered playful companionship and pharmacological sedation in alleviating preoperative anxiety in preschool children].

Objective: To investigate the effectiveness of child-centered playful companionship and sedative medication in alleviating preoperative anxiety in preschool children. Methods: A retrospective analysis was conducted on 3 825 preschool children (3-6 years) who underwent elective surgery at Shanghai Children's Medical Center from April 2021 to March 2022. The children were divided into three groups based on the preoperative anxiolytic intervention: child-centered playful companionship group (n=2 198), pharmacological sedation group (n=1 281), and no intervention group (n=346). The pharmacological

Association of Jail Decarceration and Anticontagion Policies With COVID-19 Case Growth Rates in US Counties.

IMPORTANCE: Mass incarceration is known to foster infectious disease outbreaks, amplification of infectious diseases in surrounding communities, and exacerbation of health disparities in disproportionately policed communities. To date, however, policy interventions intended to achieve epidemic mitigation in US communities have neglected to account for decarceration as a possible means of protecting public health and safety. OBJECTIVE: To evaluate the association of jail decarceration and government anticontagion policies with reductions in the spread of SARS-CoV-2. DESIGN, SETTING, AND PARTI

Prevention of Allergy to a Major Cow's Milk Allergen by Breastfeeding in Mice Depends on Maternal Immune Status and Oral Exposure During Lactation.

Background: The high incidence of food allergy in childhood points to the need of elucidating early life factors dictating allergy susceptibility. Here, we aim to address in a mouse model how the exposure to a major cow's milk allergen through breastmilk of mothers with different immune status influences food allergy outcome in offspring. Methods: BALB/cJ future dams were either kept naïve, or sensitized through the oral route using cholera toxin ("orally sensitized") or through the i.p. route using alum ("i.p. sensitized"), or rendered fully tolerant (oral gavage without any adjuvant) to bovi

From Aid Dependency to Self-Reliance: Lessons From Sierra Leone and Rwanda's Strategies for Eliminating Preventable Blindness in Africa: A Perspective Study.

BACKGROUND AND AIMS: Preventable blindness remains a leading cause of disability in low-income countries (LICs), with 91.5% of cases in Sierra Leone and 83.9% in Rwanda considered avoidable. Reliance on foreign aid has supported eye care development in Africa, yet has also fostered systemic dependency. As donor fundg declines, the need for sustainable, locally led eye health systems becomes urgent. This perspective examines how Rwanda and Sierra Leone, both postconflict LICs reliant on foreign aid, have pursued divergent approaches to eliminate preventable blindness. Rwanda via a nationally le

Childhood vaccine refusal in Türkiye: a dual perspective qualitative study of parents and healthcare workers.

BACKGROUND: Childhood vaccine refusal is a growing public health challenge worldwide. In Türkiye, formal refusal, defined as a parental decision to decline one or more scheduled vaccines that is documented in primary care immunization records, rose from 183 families in 2011 to more than 12,000 in 2016, yet the structural, cultural, and ethical dimensions of parental decision-making remain poorly understood. This study explored these tensions from the perspectives of both vaccine-refusing parents and vaccine-administering healthcare workers. METHODS: Semi-structured interviews were conducted b

[Discussions Concerning the Generalist-Specialist Combination Management Model of Chronic Kidney Disease].

In recent years, the effective management of patients with chronic kidney disease (CKD) is gaining growing attention. In 2014, our hospital established the CKD generalist-specialist combination management model, which incorporates a set of CKD management processes. The generalist component incorporates the following, general practitioners from 6 community health centers in the surrounding areas (with about 650 000 permanent residents in the region) joining hands, setting up a management team composed of doctors and nurses, and formulating management protocols for patient follow-up, patient rec

Potential of prehabilitation in hepatocellular carcinoma: a narrative review of available evidence.

BACKGROUND AND OBJECTIVE: Hepatocellular carcinoma (HCC) is a leading cause of cancer deaths. Patients with HCC are often significantly affected by sarcopenia and cancer cachexia. Prehabilitation, a multimodal pre-operative exercise and nutritional intervention, has been implemented with varying degrees of success in enhancing outcomes among other gastrointestinal (GI) malignancies. However, remarkably little is described how prehabilitation may be beneficial in improving outcomes among those with HCC. Thus, a narrative review is warranted to examine previously developed prehabilitation models

The Effects of Group Prenatal Care on Infant Birthweight and Maternal Well-Being: A Randomized Controlled Trial.

INTRODUCTION: Group prenatal care models have been in use in the USA for over 20 years and have shown benefits in reducing rates of preterm birth and low birth weight infants in high-risk civilian populations. Group prenatal care has been widely implemented at military treatment facilities, despite a lack of high-quality evidence for improved perinatal outcomes in this population. MATERIALS AND METHODS: In this randomized, controlled trial, 129 patients at a military treatment facility received either traditional one-on-one prenatal care or group prenatal care using the CenteringPregnancy mod

From pediatric to adult care: strategic evaluation of a transition program for patients with osteogenesis imperfecta.

BACKGROUND: Achieving a successful transition from pediatric to adult care for young adults with special needs, especially rare genetic diseases such as osteogenesis imperfecta (OI), is a prominent issue in healthcare research. This transition represents a challenge for patients with OI, their families, clinicians and healthcare managers because of the complex nature of the process and the lack of evaluation of existing transition programs. We evaluated a transition program for adolescents and young adults with OI from a pediatric orthopedic hospital to adult care. METHODS: Data were collecte

Post-conflict transition and sustainability in Kosovo: establishing primary healthcare-based antenatal care.

INTRODUCTION: Kosovo is a post-conflict nation with an extensively damaged infrastructure, a weak primary care base, and poor maternal-child health outcomes. The Kosovo-Dartmouth Alliance for Healthy Newborns (the Alliance) sought to improve maternal and neonatal health in Kosovo by providing family medicine-based antenatal care (ANC). METHODS: The ANC Program used a modification of the World Health Organization's four-visit, prenatal care model. The program is based in family medicine and requires minimal medical equipment, such as a blood pressure cuff, fetal doppler, measuring tape, urine

Nano-Enabled Microfluidic Platforms for Functional Immunomonitoring in Pediatric Sepsis.

Pediatric sepsis involves dynamic, developmentally conditioned immune dysfunction that a single cytokine concentration or one-time severity label cannot represent. This structured narrative Review examines how nano-enabled microfluidic systems could support blood-sparing, serial assessment of functional immune states in the pediatric intensive care unit (PICU). We separate soluble-protein concentration, cell phenotype, ex vivo stimulation response, cellular effector function, immunometabolic function, and physiology, linking each measurement class to its evidence level and permitted inference.

First-in-human pilot study of broadband optical spectroscopy (BOS) as noninvasive surveillance for necrotizing enterocolitis (NEC).

BACKGROUND: Necrotizing enterocolitis (NEC) lacks predictive biomarkers and objective early diagnostic markers. Broadband optical spectroscopy (BOS), a transcutaneous noninvasive tool, has previously demonstrated diagnostic specificity and early predictive power for NEC in a mouse model. Here, in continuation of the translational development, we report a first-in-human observational study in premature infants. METHODS: An apparatus for BOS in infants was assembled comprising a handheld probe with broad-spectrum light source coupled to a laboratory-grade spectrometer (ASD LabSpec 4, Malvern Pa

Denning Phenology Mediates Sea-Ice Loss Impacts on Early Reproductive Success in Polar Bears.

Climate change is disrupting myriad biological processes, including the ability of species to synchronize life-history events with optimal environmental conditions, with potential fitness impacts. Changes in migratory and breeding phenology and their consequences on fitness have been well studied in birds and some mammals. By contrast, changes in the phenology of overwinter sheltering and their influence on fitness have rarely been investigated. Pregnant female polar bears (Ursus maritimus) spend the winter fasting in a maternity den where they give birth and nurse their cubs until emergence i

Integrating Palliative Care into the Indonesia Health System: A Policy Brief to Enhance Accessibility, Quality, and Sustainability.

BACKGROUND: Indonesia faces a growing double burden of non-communicable diseases, particularly cancer. The latest data from the Global Cancer Observatory (Globocan) indicates over 408,661 new cases and 242,099 cancer-related deaths in 2022, with a projected 63% increase in the case burden between 2025 and 2040 without strategic intervention. Although a new legal framework through Health Law No. 17 of 2023 and the Minister of Health Decree (KMK) No. HK.01.07/MENKES/2180/2023 has mandated palliative care as an integral component of health services, its implementation still faces significant syst
