Knowing versus doing: education and training needs of staff in a chronic care hospital unit for individuals with dementia.

Hospital clinical staff routinely confront challenging behaviors in patients with dementia with limited training in prevention and management. The authors of the current article conducted a survey of staff on a chronic care hospital unit concerning knowledge about dementia, perceived educational needs, and the care environment. The overall mean score for a 27-item knowledge scale was 24.08 (SD = 2.61), reflecting high level of disease knowledge. However, staff indicated a need for more information and skills, specifically for managing behaviors nonpharmacologically (92.3%), enhancing patient s

Transition in high-acuity clinical learning environments: A qualitative meta-synthesis of newly graduated nurses in intensive care units and emergency departments.

AIM: To synthesise qualitative evidence on newly graduated nurses' transition experiences in intensive care units and emergency departments and identify implications for clinical education and learning-environment design. BACKGROUND: Transition to practice among newly graduated nurses is well documented, but evidence focused on high-acuity clinical learning environments remains fragmented. Intensive care units and emergency departments combine rapid decision-making, technological complexity and emotionally demanding care. DESIGN: A qualitative systematic review and meta-synthesis using Joann

Management of sleep disturbances in the ICU: a secondary analysis of the ESPRIT study.

BACKGROUND: Sleep disturbances are frequent in the intensive care unit (ICU) and are associated with delirium or delayed mechanical ventilation weaning. Although international guidelines recommend structured screening and non-pharmacological management, their implementation in daily practice is poorly reported. This study aimed to evaluate the prevalence, screening practices, and management strategies for sleep disturbances in French ICUs. METHODS: This was a secondary analysis of the ESPRIT study, a national, multicenter, cross-sectional study conducted in 128 ICUs. All adult patients admitt

Evaluation of nursing snippet-based intervention on parents' burden and self-efficacy towards caring for their children undergoing limb lengthening surgery.

BACKGROUND: The intricacy of pin-site care and fixator modification makes caring for children having limb lengthening surgery extremely taxing on parents. Their self-efficacy, which is essential for positive postoperative results, is frequently lowered by this stress. Traditional teaching approaches might not be able to offer the ongoing assistance required to preserve caregiver confidence and lower stress levels. AIM: To examine the nursing microlearning (snippet-based) intervention on parents' burden and self-efficacy in caring for their children undergoing limb lengthening surgery. METHOD

Understanding HIV Pre-Exposure Prophylaxis Questions of U.S. Health Care Providers: Unique Perspectives from the PrEPline Clinical Teleconsultation Service.

Introduction: Fewer than a quarter of people considered to have factors associated with HIV acquisition are prescribed pre-exposure prophylaxis (PrEP) in the United States. Prior studies demonstrate disparities in provider comfort and knowledge regarding PrEP, suggesting a need for provider capacity building to support widespread PrEP availability. This study examined real-world PrEP clinical questions/cases from providers to a teleconsultation service to identify knowledge gaps and improve PrEP-related training materials and clinical guidelines. Methods: The National Clinician Consultation Ce

Trauma and acute care surgeons report prescribing less opioids over time.

INTRODUCTION: Confronted with the opioid epidemic, surgeons must play a larger role to reduce risk of opioid abuse while managing acute pain. Having a better understanding of the beliefs and practices of trauma and acute care surgeons regarding discharge pain management may offer potential targets for interventions beyond fixed legal mandates. METHODS: An Institutional Review Board-approved electronic survey was sent to trauma and acute care surgeons who are members of the American Association for the Surgery of Trauma, and trauma and acute care surgeons and nurse practitioners at a Level 1 t

Infant-feeding practices and infant survival by familial wealth in London, 1752-1812.

Anecdotal evidence indicates that high-status women in England generally did not breastfeed their children in the seventeenth and early eighteenth centuries. Metropolitan families of varied social status also often sent their children out of London for wet-nursing. However, anecdotal sources and rural burial registers also suggest that these practices declined rapidly from the mid-eighteenth century, and were replaced by a culture of maternal breastfeeding in all social classes. These changes in infant-feeding practices have been argued to explain much of the dramatic improvement in infant mor

Bridging the AI-Literacy Gap in Health Care: Qualitative Analysis of the Flanders Case Study.

BACKGROUND: Building on the assertion that nearly every clinician will eventually use artificial intelligence (AI), this study provides a triangulated qualitative analysis of the requirements, challenges, and prospects for integrating AI into routine health care practice. This skills gap contributes to cautious and uneven adoption across clinical settings. Despite advancements, many health care professionals report a self-perceived lack of proficiency in comprehending, critically evaluating, and ethically deploying AI tools, which contributes to cautious adoption in clinical settings. What the American Journal of Critical Care Junior Peer Reviewers Were Reading During the First Year of the Program: Caring for Patients With COVID-19.

The Junior Peer Reviewer program of the American Journal of Critical Care provides mentorship in the peer review process to novice reviewers. The program includes discussion sessions in which participants review articles published in other journals to practice and improve their critical appraisal skills. The articles reviewed during the first year of the program focused on caring for patients with COVID-19. The global pandemic has placed a heavy burden on nursing practice. Prone positioning of patients with acute respiratory failure is likely to improve their outcomes. Hospitals caring for pat

The Compliance Conversation: Navigating Variations in Sterile Processing Practices.

Because of the variety of guidelines for instrument processing and sterilization (eg, government regulations on infection prevention and control [IPC], manufacturers' instructions for use, publications from professional organizations), practices may vary from one country or facility to another. The Joint Commission recommends that when creating or revising IPC-related policies, organizations should use a hierarchical model to address relevant IPC requirements. Joint Commission International accredits health care organizations outside the United States using standards and a survey process simil

Mayo Clinic Hematology Fellowship for Advanced Practice Providers.

Advanced practice providers (APPs), including nurse practitioners (NPs) and physician assistants (PAs), are part of a growing cancer care workforce. Current hematology-specific education provided by most graduate NP and PA school educations is limited. Mayo Clinic School of Health Sciences launched a hematology-specific fellowship in 2018 to provide APPs with the skills and knowledge required to deliver high-quality specialty care in hematology and blood and marrow transplant (BMT). The fellowship curriculum was developed based on a needs-based assessment study as well as the qualitative repor
