Long-term results of dorsally displaced distal radius fractures treated with the pi-plate: is hardware removal necessary?

The purpose of this study was to evaluate the outcome of patients treated with open reduction and internal fixation (ORIF) using dorsal plates and screws (AO/ASIF pi-plate) for dorsally displaced fractures of the distal radius. Although extensor tendon rupture is a recognized complication of all distal radial fractures, there appears to be an increased risk of this using dorsal plating. In addition, there is the added complication of extensor tendon irritation and dorsal wrist pain, which may necessitate plate removal. The low-profile pi-plates intended to overcome this problem have not done s

Innovative approach to intramedullary nailing of the fibula: a technical note.

Traditionally unstable ankle fractures are surgically managed using open reduction and internal fixation (ORIF) with plate and screws. However, the operative management has gained an innovative technique. In the last decade, intramedullary (IM) nailing was introduced in local guidelines as a treatment for a selective group of elderly patients with compromised soft-tissues, as this technique is minimally invasive and less prone to wound complications including infections. Based on the authors' experience with IM nailing of the fibula using an intramedullary locking fibula nail, common technical

Posterolateral elbow dislocation with entrapment of the medial epicondyle in children: a case report.

INTRODUCTION: Elbow dislocations in children are uncommon injuries. Dislocations with associated fractures or so-called complex dislocations of the elbow can be challenging to diagnose and treat. CASE PRESENTATION: A 14-year-old male had a posterolateral elbow dislocation after a fall. Closed reduction with traction was performed. Radiographs after initial reduction showed a fragment entrapped into the humero-cubital joint. Computerized tomography scan showed the fragment belonging to the medial epicondyle. Open reduction and internal fixation with a 3.0 millimeter cannulated screw was perfor

Should nails be locked dynamically or statically in atypical femoral fractures? - A radiological analysis of time to union and reoperations in 236 displaced fractures with 4 years average follow-up.

INTRODUCTION: Atypical femoral fractures (AFFs) are associated with delayed union and higher reoperation rates. Axial dynamization of intramedullary nails is hypothesized to reduce time-to-union (TTU) and fixation failure as compared to static locking. METHODS: Consecutive acutely displaced AFFs fixed with long intramedullary nails across five centres between 2006 and 2021 with a minimum postoperative follow-up of three months were retrospectively reviewed. The primary outcome was TTU, compared between AFFs treated with dynamically or statically locked intramedullary nails. Fracture union was

Triangular Fixation Technique for Bicolumn Restoration in Treatment of Distal Humerus Intercondylar Fracture.

BACKGROUND: Distal humerus intercondylar fractures are intra-articular and comminuted fractures involving soft tissue injury. As distal humerus is triangle-shaped, parallel plating coupled with articular fixation would be suitable for bicolumn restoration in treatment of distal humerus intercondylar fracture. METHODS: This study included 38 patients (15 males and 23 females) who underwent olecranon osteotomy, open reduction and internal fixation with the triangle-shaped cannulated screw and parallel locking plates (triangular fixation technique). Functional results were assessed with the visu

Determining the Outcomes of Pilon Fractures Treated by Minimally Invasive Plate Osteosynthesis (MIPO): A Retrospective Study.

BACKGROUND: Pilon fractures, affecting the distal tibia, pose significant challenges due to their complexity and the delicate balance required between adequate fracture reduction and soft tissue preservation. Minimally invasive plate osteosynthesis (MIPO) has emerged as a viable alternative to traditional open reduction and internal fixation (ORIF), aiming to reduce soft tissue complications while maintaining biomechanical stability. OBJECTIVE: This study evaluates the functional and radiological outcomes of pilon fractures treated via MIPO, analyzes correlations between radiographic union an

Acute Total Hip Arthroplasty for Acetabular Fractures.

Acetabular fractures are becoming more common, especially in the elderly population. Standard goals of management include restoring stability and articular congruity to the hip joint in an effort to minimize the risk of posttraumatic arthritis. This is most commonly achieved through open reduction and internal fixation, from which outcomes are generally positive. However, several preoperative risk factors have been identified that correlate with poor outcomes, such as severe acetabular impaction, femoral-sided lesions, and older age. Furthermore, goals of management in the increasingly encount

Non-osteoporotic L4 vertebral compression fracture after low-voltage electrical injury without secondary trauma: A rare case report.

Electrical injury is a common occupational hazard; however, vertebral compression fracture caused by low-voltage electric shock in the absence of secondary fall- or impact-related trauma is extremely rare. We report the case of a man in his early 40s who developed severe lower back pain and inability to walk immediately after a 220-volt alternating-current electrical injury at work. He remained conscious and sustained no fall or blunt trauma. Imaging examinations, including radiography, computed tomography, and magnetic resonance imaging, revealed an acute L4 vertebral compression fracture. Du

Fixation of Displaced Lateral Column and Spine of Scapula using Modified Judet Approach.

INTRODUCTION: Since traumatic injuries to the scapula and scapulothoracic articulation are rare, they have not been extensively covered in the literature. According to reports, arthrosis, rotator cuff dysfunction, scapulothoracic dyskinesis, and impingement pain all contributed to decreased shoulder function after nonsurgical therapy. In addition, it was noted that surgical intervention for scapular fractures produced better results and a higher functional recovery. CASE REPORT: A 51-year-old female presented to our emergency department with a history of a fall of a heavy object over her righ

Two-View Gravity Stress Imaging Protocol for Nondisplaced Type II Supination External Rotation Ankle Fractures: Introducing the Gravity Stress Cross-Table Lateral View.

Assessing ankle stability in nondisplaced Lauge-Hansen supination external rotation type II injuries requires stress imaging. Gravity stress mortise imaging is routinely used as an alternative to manual stress imaging to assess deltoid integrity with the goal of differentiating type II from type IV injuries in cases without a posterior or medial fracture. A type II injury with a nondisplaced fibula fracture is typically treated with cast immobilization, and a type IV injury is considered unstable and often requires operative repair. The present case series (two patients) highlights a standardi

Transcapho perilunate dislocation with palmar extrusion of the scaphoid proximal pole.

Perilunate fracture-dislocations usually combine ligament ruptures, bone avulsions, and fractures in different patterns. Rarely a displaced fracture of the scaphoid can coexist with a scapho-lunate dissociation and can result in enucleation of the proximal pole. We report about a case of trans-scaphoid perilunate dislocation with palmar extrusion/enucleation of the scaphoid proximal pole, treated with scaphoid fracture open reduction and internal fixation with screw, scapho-lunate ligament repair with an anchor and vascularization of the scaphoid proximal pole with the 2nd intermetacarpal arte

Risk Factors for Thromboembolic Events After Surgery for Ankle Fractures.

We conducted a retrospective national-cohort study to determine the incidence of and independent risk factors for venous thromboembolic events (VTEs) after open reduction and internal fixation (ORIF) of ankle fractures. Using the American College of Surgeons National Surgical Quality Improvement Program (ACS-NSQIP) database, we identified patients who underwent ORIF for ankle fracture between 2005 and 2012. VTE was defined as the occurrence of a deep vein thrombosis or a pulmonary embolism within the first 30 postoperative days. Of the 4412 ankle fracture patients who met the inclusion criteri

Total hip arthroplasty after operatively treated acetabular fracture: a concise follow-up, at a mean of twenty years, of a previous report.

Acetabular fractures increase the risk of posttraumatic arthritis and thus the risk of total hip arthroplasty (THA). We previously presented the ten-year results of THA performed for posttraumatic arthritis after an acetabular fracture; we now present the twenty-year outcomes. The original publication included sixty-six patients who underwent THA between 1970 and 1993 for posttraumatic arthritis after open reduction and internal fixation (ORIF) of an acetabular fracture. Clinical outcomes, implant survivorship, radiographic results, and complications were evaluated. A total of thirty-six patie
