Orbital cellulitis: a review of 21 cases from Ibadan, Nigeria.

This study was set up to review cases of orbital cellulitis admitted to the eye ward of our hospital between 1994 and 1998, and to compare the findings with similar studies. Twenty-one (14 males, 7 females, age range 45 days to 50 years [mean 13 years]) of 35 patients with an initial diagnosis of orbital cellulitis were reviewed (14 patients were excluded from the final analysis). Most of the cases were associated with pansinusitis, the commonest organism isolated being Staphylococcus aureus. All the patients were co-managed with ortorhinolaryngologists, and sometimes also with bacteriologists…

Growing skull fracture of the posterior cranial fossa and of the orbital roof.

BACKGROUND: Growing Skull Fractures (GSF) are rare complications of head trauma, primarily reported in infancy and early childhood. GSF are commonly located on calvaria, and rarely in other locations, including the skull base. METHOD: In this study, we report two cases of GSF occurring in unusual locations. The first, a 8-month old girl, with a GSF of the suboccipital posterior fossa region, and the second, a 4-year old boy with a GSF of the right orbital roof. Both cases underwent operative treatment of the GSF, with microsurgical dissection and excision of the protruding gliotic brain tissu…

Fronto-orbital remodeling without orbito-naso-frontal bandeau.

INTRODUCTION: Fronto-orbital bilateral advance is the procedure of choice for the treatment of craniosynostosis affecting most of the anterior area of the skull and orbitomalar regions. The aim of the technique is to achieve a supra-orbital bilateral bar and a frontal bone. We have introduced a modification in order to simplify the technique. PATIENTS AND METHODS: From November 1998 to January 2002, 18 patients with craniosynostosis have been surgically treated using our technique. The mean age when the treatment was performed was 6.93 months (range 3 to 22 months). Brain computed tomography …

Penetrating trauma-induced orbital floor fracture. Case report.

Orbital floor fractures are common in midface trauma. While the mechanism which produces them is subject to debate, they are acknowledged to be difficult to diagnose solely through clinical means. In presenting the case of an orbital blow-out fracture caused by penetrating trauma, the author describes the successful diagnosis and surgical treatment of a "pure" fracture in which no globe trauma occurred.

Orbital apex injury: trauma at the junction between the face and the cranium.

Orbital apex injury is usually seen in multiply and severely injured patients who are subject to high-energy trauma. Orbital apex injury rarely occurs in isolation. By proximity, the face, the skull base, or their combination are the most likely regions to be injured in association with orbital apex trauma. The vast majority of these injuries occur as an extension of orbital, LeFort, naso-orbito-ethmoid, panfacial, sphenoid, or temporal bone fractures of the skull. Complex osseous anatomic structures with intimately related multiple neurovascular organs make injuries to the orbital apex diagno…

Endoscopic endonasal reduction for blowout fracture of the medial orbital wall.

Endoscopic endonasal reductions have been addressed in 63 patients with blowout fracture of the medial orbital wall since 1992. The operations were carried out under general anesthesia with a magnified operative space projected on a television monitor by a charge coupled device video camera attached to the endoscope. The middle nasal turbinate was fractured toward the nasal septum, the uncinate process was cut off, and the bulla was opened. The ethmoidal bony partition and the mucous membrane were removed; however, the fractured bone chips of the medial orbital wall were preserved. The herniat…

[Triplex scanning of orbital vessels in patients with normal pressure glaucoma].

Bloodflow in the orbital artery and central retinal artery was studied by triplex scanning in 35 patients with normal pressure glaucoma, and 14 subjects without signs of glaucoma. Significant differences in the studied parameters were detected in the patients in comparison with the controls: decreased linear bloodflow velocity in the orbital (p < 0.01) and central retinal arteries (p < 0.01), increased peripheral resistance indexes in both arteries (p < 0.05) in patients with normal pressure glaucoma, and decreased bloodflow velocity in the central retinal artery in primary open-angle glaucoma…

Computer-assisted secondary reconstruction of unilateral posttraumatic orbital deformity.

Until now, computer-assisted surgery has not been practiced as part of the surgical routine of posttraumatic orbital reconstruction. The purpose of this study was to investigate the use of a navigation system for computer-assisted preoperative planning with virtual reconstruction to obtain symmetry of the orbits and intraoperative control of virtual contours in comparison with the clinically achieved surgical results. A further objective of the computer-assisted orbital analysis was to use an ideal measurement for the two-dimensional and three-dimensional changes following orbital reconstructi…

Orbital inflammation in a patient with extrascleral spread of choroidal malignant melanoma.

An elderly woman presented with fever, dehydration, orbital inflammation, total external and internal ophthalmoplegia and blindness, resembling the clinical appearance at presentation of severe orbital inflammatory disease or mucormycosis. Orbital computed tomography scanning demonstrated a retrobulbar orbital mass. Subsequent B-scan ultrasound examination confirmed the orbital mass but also demonstrated a mass within the eye. At lateral orbitotomy, extrascleral spread of an entirely necrotic intraocular melanoma was demonstrated. As computed tomography scanning may not be able to delineate an…

A case of atypical idiopathic orbital myositis mimicking neurological disease.

A 24 year old patient with acute double vision and headache was admitted to our clinic. A diagnosis of orbital myositis was confirmed after magnetic resonance imaging and orbital echogram. No other clinical manifestations were observed on general examination. Since orbital myositis cases with only neurological symptoms and without typical visual impairment are not frequent, we decided to present this case.

Spontaneous subconjunctival orbital fat prolapse: report of three cases.

Spontaneous subconjunctival orbital fat prolapse is an uncommon but benign entity. It is seldom discussed in the literature, and is sometimes confused with other ocular surface tumors, such as dermolipoma. We present 3 cases, two of which are bilateral, the other being unilateral. Two cases underwent surgical excision due to ocular irritation, and the pathology proved mature adipose tissue compatible with the diagnosis. Computed tomography (CT) was available for 1 case and revealed a fat-compatible radiolucent mass continuous with the intraconal fat. Some other reports have presumed that the i…
