[Clinical experience about manipulative reduction of the radialis and ulnar fractures].

OBJECTIVE: To observe the clinical efficacy about manipulative reduction of the radialis and ulnar fractures. METHODS: From June 2005 to June 2009, 85 patients with radialis and ulnar fractures, 50 males and 35 females, ranging in age from 1 to 20 years with an average of 10.8 years, were treated with manipulative reduction and external fixation by small splint. There were 53 cases of direct violence, 30 cases of transmission violence, 2 cases of reversing violence; and there were 6 cases of comminuted fracture, 8 cases of spiral fracture, 21 cases of oblique fracture, 50 cases of serrated or

Current concepts in the management of complex elbow trauma.

Complex instability of the elbow, also known as fracture dislocation of the elbow, is one of the most challenging injuries of the musculoskeletal system. A clear understanding of the biomechanics of the joint with special focus on the relative contributions of the articular components and the ligamentous constraints is critical to the proper management of these injuries. The accurate recognition and implications of associated injuries offers a rationale for approaching these difficult problems by appropriately addressing the elements of the trauma. Further, doing so in a timely fashion enhance

Extensor tendon injury to the index and middle finger secondary to Kirschner wire insertion for a distal radial fracture.

We describe a case with the highly unusual complication of attritional ruptures to both the extensor tendons to the index and middle finger following Kirschner wire placements for a distal radial fracture. This particular complication has never been previously described in the literature. We also review the literature, identifying certain techniques in the management of distal radial fractures with Kirschner wires which are considered to increase the risk of tendonous or neurovascular injury. Finally, recommendations are made that should reduce the complications of iatrogenic soft tissue injur

Fractures of the olecranon and radial neck in children.

Over one half of radial neck fractures are associated with other injuries about the elbow. Most radial neck fractures can be treated with closed reduction or with percutaneous probing, even if the other associated injuries require open reduction. The results are good, although radiologic imperfections may be seen after healing. In the older child, olecranon fractures that have significant displacement usually require open reduction with internal fixation.

Tophaceous gout simulating infection about a stainless steel plate.

A 55-year-old man developed acute symptoms suggestive of a late, deep infection 8 years after uncomplicated open reduction and internal fixation of fractures to the left ulnar and radial shafts. At the time of surgical debridement, tophaceous material was found surrounding the radial plate. Acute tophaceous gout surrounding fracture fixation devices has not been previously reported and is difficult to distinguish from a deep infection.

Carpal tunnel pressure in the acute phase after Colles' fracture.

In order to analyze the pressure on the carpal tunnel in the acute phase after Colles' fractures 13 patients were studied. The initial pressure and the variation after injection of a local anesthetic into the fracture hematoma, as well as the effects of increased volar flexion of the wrist, were measured using a wick catheter technique. Comparisons were made with a control group of ten healthy volunteers. The pressure was significantly higher in the fracture group than in the control group, and it increased significantly after injection of a local anesthetic into the fracture hematoma. A corre

Fracture of the distal radial physis complicated by compartment syndrome and premature physeal closure.

Complications after injury to the epiphyseal growth plate of the distal radius are uncommon. An 11-year-8-month-old boy developed two complications--compartment syndrome of the forearm and premature closure of the physis--after a Salter-Harris Type I injury. Treatment included closed reduction, decompression, skin grafting, excision of the physeal bar, and epiphysiodesis of the distal ulna and the remaining open physis of the distal radius.

Bone mass of the axial and the appendicular skeleton in women with Colles' fracture: its relation to physical activity.

The aim of this investigation was to elucidate whether Colles' fractures in middle-aged women were indicative of generalized bone loss (osteopenia) and whether bone mass of the axial and the appendicular skeleton was related to the extent of daily physical activity. Thirty-six patients (aged 50-73 years) with previous Colles' fracture were studied. The median time elapse after the injury was 18 months (range 9-20.5 months). Bone mineral content of the 2nd, 3rd, and 4th lumbar vertebrae (lumbar BMC) and both distal forearms (forearm BMC) were determined to dual-photon (153Gd) absorptiometry. Th

[Pathogenesis of Sudeck's syndrom and a new possibility for its treatment (author's transl)].

Authors describe the microcircular haemodynamics of Sudeck's syndrom together with the characteristic vitious circle of pain, rigidity, oedema and osteoporosis. Based on theoretical considerations and own results the infusion of dextran (Rheomacrodex 10%) as a pathogenetic base therapy is recommended. For increasing efficiency according to their experiences early treatment is suggested.

[Dislocation fractures of the forearm. Galeazzi, Monteggia, and Essex-Lopresti injuries].

Combined forearm fractures are identified according to their location as Galeazzi, Monteggia, or Essex-Lopresti injuries. The feature common to these three forms is the combination of a forearm fracture with instability of the distal or proximal radioulnar joint. Appropriate management of the injury at an early stage is indispensable to achieve good functional results. Galeazzi fractures should initially be treated by open reduction and correct anatomy restored by plate osteosynthesis. Fixation of the distal radioulnar joint with Kirschner wires should be performed in cases of persistent dislo

Electric scooter injuries: Incidence and injury patterns at a level I trauma center.

PURPOSE: Electric scooters (e-scooters) have become an increasingly popular mode of public transportation in recent years. As the incidence of related injuries rises, it is important to understand specific fracture patterns unique to e-scooters and electric bikes (e-bikes) to help guide management. The purpose of this study was to review the prevalence and describe specific fracture patterns of e-scooter and e-bike related injuries at the busiest level 1 trauma center in the borough of Manhattan. METHODS: Chart review to determine mechanism of injury was performed on all patients for whom an

Complex Monteggia Fractures in the Adult Cohort: Injury and Management.

Monteggia fractures involve proximal ulna fracture associated with a radiocapitellar joint dislocation. The Bado classification is primarily based on the direction of the radial head dislocation. The Jupiter subtype classification of Bado II fractures further characterizes the severity of proximal ulna comminution and the involvement of the coronoid fragment. This latter classification can better prognosticate the challenges of surgical reconstruction and clinical outcomes. Surgery for all adult Monteggia fractures is required to restore the anatomic alignment of the ulna, which indirectly red

Estrogen Replacement Therapy and Postoperative Complications After Distal Radius Open Reduction and Internal Fixation.

BACKGROUND: To our knowledge, there are no studies assessing associations between patients on preoperative estrogen replacement therapy (ERT) undergoing distal radius open reduction and internal fixation (DRORIF) and postoperative outcomes. Thus, the purpose of this study was to compare surgical and medical complications in patients on ERT undergoing DR-ORIF with a matched control cohort. MATERIALS AND METHODS: The PearlDiver Mariner database was searched for female patients who took ERT within 3 months of the index procedure. Patients were matched 1:4 with non-ERT controls using propensity s

Epidemiology and Treatment Outcomes in Adult Peri-Implant Forearm Fractures: A Longitudinal Observational Study.

BACKGROUND: Although forearm fractures in adults are commonly managed operatively, there is a paucity of studies on peri-implant fractures of the forearm. METHODS: We performed a single-centre retrospective cohort study. Consecutive patients presenting with peri-implant fractures of the forearm between 2010 and 2024 were included. In this study, we described the epidemiology of peri-implant forearm fractures, fracture configuration, treatment received, and outcomes following this injury. RESULTS: Thirty-five patients were identified from our database. Median age at the time of injury was 39.
