Modern management of pediatric nocturnal enuresis: Evidence-based treatment and practical guidance.

Nocturnal enuresis (NE) is a common and multifactorial pediatric condition characterized by abnormalities in bladder storage, renal water regulation, and central arousal mechanisms. According to the International Children's Continence Society, NE is defined as intermittent urinary incontinence that occurs during sleep in children aged 5 years or older, in the absence of congenital or acquired neurologic or urologic disease. Although often self-limiting, NE imposes a substantial psychosocial burden, causing embarrassment, low self-esteem, sleep disruption, and family stress. Advances in neuroph

Early-Onset Trichotillomania With Predominantly Other-Directed Trichophagia in a 23-Month-Old Girl: A CARE-Compliant Case Report.

BACKGROUND: Trichotillomania (TTM) and trichophagia are rare in toddlers and usually involve self-directed hair pulling that leads to alopecia. Atypical patterns, such as pulling and ingesting others' hair, may delay recognition. Trichophagia carries a risk of trichobezoar formation and serious gastrointestinal complications. CASE PRESENTATION: A case is described involving a 23-month-old girl with a three-month history of persistently plucking and ingesting her mother's and sister's hair, including during their sleep. She also ingested loose hairs found on floors and furniture. Although pred

Instantaneous self-recovery and ultra-low detection limit hydrogel electronic sensor for temporomandibular disorders intelligent diagnosis.

Temporomandibular disorders (TMD) intelligent diagnosis promises to elevate clinical efficiency and facilitate timely TMD management for patients. However, development of TMD intelligent diagnostic tools with high accuracy and sensitivity presents challenges, particularly in sensing minute deformations and ensuring rapid self-recovery. Here we report a biocompatible hydrogel electronic sensor with instantaneous self-recovery (within 2.1 s) and ultra-low detection limit (0.005% strain). It could efficiently diagnose disc displacement with reduction (DDwR) with satisfactory accuracy of 90.00%, a

Utilization of Chiropractic Services among Older Adults with Spine Pain at a Publicly Funded Canadian Healthcare Facility: A Retrospective Study.

There are limited data on chiropractic care for older adults, specifically from medically underserved communities. This study describes the characteristics, clinical management, and patient-reported outcomes of older adults with spinal pain who present for chiropractic care at a publicly funded community health centre serving marginalized populations. This retrospective analysis utilized quality assurance data from chiropractic encounters at Mount Carmel Clinic between January 2011 and June 2020 of adults aged 45 and older. Descriptive statistics summarized the study population and their self-

[EPIPEN INSTRUCTIONS FOR ANAPHYLAXIS PATIENTS DURING DISPENSING: FACTORS RELATED TO THE QUALITY OF PHARMACISTS' INSTRUCTIONS].

BACKGROUND AND AIM: We previously reported that pharmacists working in pharmacies don't have enough knowledge and enough experience teaching anaphylaxis (An) and EpiPen use. We administered a questionnaire survey to pharmacists with experience handling EpiPen prescriptions. We investigated the relationship between the questionnaire results and the factors in the pharmacists' background regarding the explanation and guidance to patients. RESULTS: The percentage of pharmacists working in pharmacies who provided guidance using visual information and demonstrations was insufficient. Moreover, thi

Perspective: The Promise of Virtual Reality as an Immersive Therapeutic.

In this perspective, we discuss the promise of virtual reality (VR) as an immersive therapeutic (ITx) delivery device. We review the neurobiology of learning and show that VR broadly engages multiple learning systems in the brain in synchrony thus having the unique potential to increase the effectiveness and speed of therapeutic change. We examine one application of VR in the delivery of chronic pain management therapy. We discuss the strengths and weaknesses of current therapies such as pain education and cognitive behavior therapy and suggest that VR has the potential to deliver self-adminis

Breaking down statin myopathy: understanding the self-limited and autoimmune subtypes.

Statins are widely used crucial drugs for the primary and secondary prevention of atherosclerotic cardiovascular disease (ASCVD). Although generally well tolerated, statin intolerance can unfortunately limit statin use, with statin-associated muscle symptoms (SAMS) being the most common side effect associated with its discontinuation. Statin intolerance is an inability to tolerate a dose of statin required to sufficiently reduce an individual's cardiovascular risk, limiting the effective treatment of patients at risk of or with cardiovascular disease (CVD). Statin myopathy is a broad entity en

The Link Between Victimization and Misconduct in Prison Inmates: The Influence of Importation, Deprivation, and Low Self-Control Factors.

There has been limited study of the link between victimization and the criminal offense in the correctional context, despite the extensive literature supporting the overlap of victims and offenders in the general population. In this study, 2,880 inmates in 12 institutional correction facilities in Taiwan were examined to explore the common factors of the victim-offender overlap, guided by the importation, deprivation, and low self-control theories. The results of bivariate probit regression analysis revealed the presence of the victim-offender phenomenon in Taiwanese inmates. Specifically, the

Uveitis: Molecular Pathogenesis and Emerging Therapies.

The profound impact that vision loss has on human activities and quality of life necessitates understanding the etiology of potentially blinding diseases and their clinical management. The unique anatomic features of the eye and its sequestration from peripheral immune system also provides a framework for studying other diseases in immune privileged sites and validating basic immunological principles. Thus, early studies of intraocular inflammatory diseases (uveitis) were at the forefront of research on organ transplantation. These studies laid the groundwork for foundational discoveries on ho

Recurrent macroglossia requiring tracheostomy after haemorrhagic basal ganglia stroke.

A 50-year-old African American woman with hypertension, congestive heart failure, chronic kidney disease and prior cerebral vascular accident was transferred from an outside hospital after being found unresponsive and subsequently intubated for severe orolingual swelling. Imaging showed left thalamic haemorrhagic stroke, and the lingual swelling was clinically concerning for angio-oedema, with which a lingual biopsy was consistent. Work-up was negative for hereditary or acquired angio-oedema, and imaging was negative for structural causes. Of note, the patient had an episode of severe orolingu

Psychiatric morbidity and socio-occupational dysfunction in residents of a drug rehabilitation centre: challenges of substance misuse management in a Bruneian context.

Aims and method In 2011, a psychiatric clinic was started in Pusat Al-Islah, a drug rehabilitation centre. Our aim was to record self-reported socio-occupational dysfunction and patterns of drug misuse and to evaluate the usefulness of a psychiatric screening tool. A two-phased approach using the Self-Reporting Questionnaire (SRQ) and the Mini International Neuropsychiatric Interview (MINI) was used to examine the rates of psychiatric diagnoses. Results Methamphetamine was the most commonly misused substance in 94.5% of residents. High levels of socio-occupational dysfunction were reported. In

A pilot cross-sectional study of patients presenting with cellulitis to emergency departments.

To characterise the Emergency Department (ED) prevalence of cellulitis, factors predicting oral antibiotic therapy and the utility of the Clinical Resource Efficiency Support Team (CREST) guideline in predicting patient management in the ED setting, a prospective, cross-sectional study of consecutive adult patients presenting to 3 Irish EDs was performed. The overall prevalence of cellulitis was 12 per 1,000 ED visits. Of 59 patients enrolled, 45.8% were discharged. Predictors of treatment with oral antibiotics were: CREST, Class 1 allocation (odds ratio (OR) 6.81, 95% Cl =1.5-30.1, p=0.012),

Medication adherence interventions for older adults: literature review.

This literature review explores the range and nature of medication adherence interventions tested with older adults. The unique needs of older adults require specifically designed and tailored interventions. Low medication adherence rates among some elderly contribute to inadequate pharmacological management of illnesses. Searches were conducted to identify randomized controlled trials of medication adherence; computerized databases, journal hand searches, and ancestry searches yielded 63 studies published between 1977 and 2005 where participants' mean age was > 60 years. Interventions were ca

Deliberate self-poisoning with tiagabine: an unusual toxidrome.

Tiagabine is an anticonvulsant acting by selective inhibition of neuronal and glial gamma-aminobutyric acid uptake, resulting in increased gamma-aminobutyric acid-mediated inhibition in the brain. Few reports in the literature describe the clinical course of severe tiagabine intoxication. A 44-year-old woman presented after deliberate self-poisoning with 100 tiagabine 15 mg tablets (1,500 mg; 25 mg/kg). Serum tiagabine level was 4,600 microg/L (1,725 mmol/L) at presentation, 20 times levels associated with therapeutic dosing. Intoxication was manifested by profuse vomiting, coma, myoclonus, ge

Sleep in overweight adolescents: shorter sleep, poorer sleep quality, sleepiness, and sleep-disordered breathing.

OBJECTIVE: To document the sleep of overweight adolescents and to explore the degree to which weight-related sleep pathology might account for diminished psychosocial outcome. METHODS: Sixty children aged 10-16.9 from a weight-management clinic were compared to 22 healthy controls using comprehensive actigraphic, polysomnographic, and parent- and self-report questionnaire assessments. RESULTS: Overweight participants averaged more symptoms of sleep-disordered breathing, later sleep onset, shorter sleep time, and more disrupted sleep than controls. Although the groups did not differ in self-r

Herpes zoster-associated acute urinary retention: a case report.

An 87-year-old woman presents with a 4-week history of urinary incontinence during which she had been treated for disseminated herpes zoster virus (HZV). On physical exam painful vesicles involving the entire vulvar region with mainly right sacral distribution were found. A catheterized volume exceeded 600 ml of retained urine after the patient failed to void spontaneously. Multichannel voiding-pressure urodynamic studies revealed an acontractile neurogenic bladder with overflow incontinence. The patient was discharged on a conservative regimen with arrangement for visiting nurse services to p

Fecal incontinence in anorectal malformations, neuropathy, and miscellaneous conditions.

In the majority cases, fecal soiling in children is functional and usually associated with severe constipation. Fortunately, functional soiling is a self-limiting problem and usually disappears at puberty. Organic fecal incontinence is a consequence of congenital malformations affecting the anorectum, anal sphincters, or the spinal cord. Inability to control bowel function may be permanent, as in patients with myelodysplasia; self-limiting, as in patients who have fecal soiling after a pull-through operation for Hirschsprung's disease; or partial, as in many patients who have undergone repair
