Occupant injuries in light passenger vehicles-A NASS study to enable priorities for development of injury prediction capabilities of human body models.

To prioritize how the development of mathematical human body models for injury prediction in crash safety analysis should be made, the most frequent injuries in the NASS CDS data from 2000 to 2015 were analyzed. The crashes were divided into seven types, from front to side. Non-minor injuries (AIS2+) were analyzed in two steps. In the first step, a grouping was made according to the AIS definition of body regions: head, face, neck, thorax, abdomen and pelvic contents, spine, upper extremities (including shoulder girdle) and lower extremities (including pelvis). In a second step, the body regio

Monteggia-like lesions: preliminary reports and mid-term results of a single center.

Background and aim of the work Monteggia-like lesions are complex and rare elbow injuries. A detailed classification and a proper surgical treatment can provide quite good functional and radiological outcomes. The aim of this retrospective study was to evaluate the mid-term results of Monteggia-like lesions surgical treatment. The other purpose was to investigate whether the complexity of ulna and radius fractures could be prognostic factors of insufficient functional results. Methods Seven women and five men, with a mean age of 59,25 years who had sustained a Monteggia-like lesion were follow

New Reference PMHS Tests to Assess Whole-Body Pedestrian Impact Using a Simplified Generic Vehicle Front-End.

This study aims to provide a set of reference post-mortem human subject tests which can be used, with easily reproducible test conditions, for developing and/or validating pedestrian dummies and computational human body models against a road vehicle. An adjustable generic buck was first developed to represent vehicle front-ends. It was composed of four components: two steel cylindrical tubes screwed on rigid supports in V-form represent the bumper and spoiler respectively, a quarter of a steel cylindrical tube represents the bonnet leading edge, and a steel plate represents the bonnet. These c

Urinary steroid metabolites in a case of florid Ectopic Cushing's syndrome and clinical correlations.

A 51-year old woman was admitted with multiple cerebral, pulmonary and intra-abdominal abscesses. The combination of apparent immunosuppression, obesity, diabetes mellitus, hypertension, hypokalaemia, osteoporotic fractures and bilateral shoulder avascular necrosis led to a clinical diagnosis of Cushing's syndrome (CS). This was biochemically confirmed as follows: midnight serum cortisol 4275 nmol/L (60-250), non-suppressed overnight dexamethasone suppression test, raised salivary cortisol 716 nmol/L (5-46) and ACTH 639 ng/L (0-46). Urinary free cortisol was elevated >75,000 nmol/L (<165). Uri

Rotator cuff injuries in adolescent athletes.

The cause of rotator cuff injuries in the young athlete has been described as an overuse injury related to internal impingement. Abduction coupled with external rotation is believed to impinge on the rotator cuff, specifically the supraspinatus, and lead to undersurface tears that can progress to full-thickness tears. This impingement is believed to be worsened with increased range of motion and instability in overhead athletes. A retrospective review of seven patients diagnosed with rotator cuff injuries was performed to better understand this shoulder injury pattern. The type of sport played

[Current status of spinal injuries in winter sports].

Parallel with a marked escalation in the number of injuries of the skull, shoulder girdle and upper extremities in recent years, the incidence of spinal injuries has also intensified. This is due to an increase in speed and deceleration traumas, such as are particular to collision accidents. Of all patients with winter sports injuries at the Department of Traumatology at Innsbruck University Hospital, 4.9% have a spinal trauma. A retrospective ten-year study was undertaken to analyze the surgically treated spinal injuries out of overall winter sports injuries. Between 1982 and 1992 862 spinal

Minimally invasive plate osteosynthesis with a locking compression plate is superior to open reduction and internal fixation in the management of the proximal humerus fractures.

BACKGROUND: The use of minimally invasive plate osteosynthesis (MIPO) via anterolateral deltoid splitting has good outcomes in the management of proximal humerus fractures. While using this approach has several advantages, including minimal soft tissue disruption, preservation of natural biology and minimal blood loss, there is an increased risk for axillary nerve damage. This study compared the advantages and clinical and radiological outcomes of MIPO or open reduction and internal fixation (ORIF) in patients with proximal humerus fractures. METHODS: A matched-pair analysis was performed, an

[Unstable shoulder dislocation].

Shoulder dislocations are a frequent event and the severity is often underestimated. A thorough clinical investigation and adapted imaging diagnostics are able to detect age-dependent injury patterns. A biphasic conservative treatment keeping the arm in a neutral to mild external rotation for 3 weeks has proven to be most effective and should be followed by an intensive physiotherapy. Nevertheless, conservative treatment protocols show unacceptable recurrence rates particularly in young men active in sport, therefore, surgical stabilisation is recommended. In the past open procedures were acce

Higher Surgeon Volume is Associated With a Lower Rate of Subsequent Revision Procedures After Total Shoulder Arthroplasty: A National Analysis.

BACKGROUND: Studies assessing the relationship between surgeon volume and outcomes have shown mixed results, depending on the specific procedure analyzed. This volume relationship has not been well studied in patients undergoing total shoulder arthroplasty (TSA), but it should be, because this procedure is common, expensive, and potentially morbid. QUESTIONS/PURPOSES: We performed this study to assess the association between increasing surgeon volume and decreasing rate of revision at 2 years for (1) anatomic TSA (aTSA) and (2) reverse TSA (rTSA) in the United States. METHODS: In this retros

Does intraoperative fluoroscopic 3D imaging provide extra information for fracture surgery?

Fracture surgery of the extremities using 2D fluoroscopy frequently fails to detect the suboptimal positioning of implants and joint incongruities. The use of intraoperative 3D-rotational X-ray (3D-RX) imaging with a new X-ray device potentially reveals these failures. We compared 50 intraoperative (2D) results of surgery and certainty about the effectiveness of different aspects of fracture reduction as interpreted from conventional (2D) methods versus intraoperative 3D-RX in 42 distal extremity fractures by means of a surgery questionnaire. In addition, we investigated the need for revision

Unexpected Positive Cultures After Failed Proximal Humerus Osteosynthesis: Why a Two-Stage Procedure Could Be Safer.

Background: Treatment of failed osteosynthesis of fractures of the proximal humerus with one-stage or two-stage surgery is difficult and clinical results are poor. The aim of this work is to evaluate the microbiological positivity of devices removed due to osteosynthesis failure. Furthermore, the clinical outcomes of these patients were evaluated at a follow-up of minimum 6 months, to assess the recovery of range of motion and the reduction in pain. Methods: A retrospective analysis was performed on 15 patients treated from September 2021 to September 2023 for failure of previous proximal hume

Tendon Transfer Versus Tendon Graft Reconstruction of Flexor Pollicis Longus Attritional Ruptures Secondary to Volar Plating of Distal Radius Fractures.

BACKGROUND: Attritional rupture of the flexor pollicis longus (FPL) after volar plating of distal radius fractures is an uncommon but morbid complication, secondary to plate prominence at or distal to the watershed zone. No consensus exists regarding the optimal reconstruction, as both tendon grafting and tendon transfer are routinely utilized. This study evaluated clinical outcomes of tendon transfer versus tendon grafting for FPL rupture following volar plate fixation and compared presenting features, radiographic prominence, and complications between cohorts. METHODS: A retrospective revie

Ambulatory Catheter-based Interscalene Block for Proximal Humerus Fracture Rehabilitation: Safety, Efficacy and Lessons from a Pilot Study.

INTRODUCTION: Proximal humerus fractures (PHFs) are associated with morbidity/functional impairment. Rehabilitation adherence is crucial to regain independent function yet is often hindered by pain. This pilot study aims to analyse the safety and efficacy of ambulatory catheter-based interscalene blocks (CISBs) as analgesia in post-surgical PHF patients and summarise learning points to guide further implementation/study of ambulatory CISB. MATERIALS AND METHODS: This pilot study selected PHF patients who were >18yo, surgically treated and received ambulatory CISB (CISB ≥72 hours). Data was de
