The reciprocal interaction between obesity and obstructive sleep apnoea.

Obesity is a significant risk factor in the pathogenesis of obstructive sleep apnoea (OSA) altering airway anatomy and collapsibility, and respiratory control. The association between obesity and OSA has led to an increasing focus on the role of weight loss as a potential treatment for OSA. To date, most discussion of obesity and OSA assumes a one-way cause and effect relationship, with obesity contributing to the pathogenesis of OSA. However, OSA itself may contribute to the development of obesity. OSA has a potential role in the development and reinforcement of obesity via changes to energy …

Decision making is affected in obstructive sleep apnoea syndrome.

We assessed decision making in 20 patients newly diagnosed with obstructive sleep apnoea (OSA) and 20 healthy controls with the Iowa Gambling Task (IGT), which evaluates the ability to learn to sacrifice immediate rewards in favour of long-term gains. A standard neuropsychological battery was administered. Switching scores tended to be lower in patients. Patients persisted in selecting risky decks throughout the IGT, whereas controls behaved normally. Performance was correlated with hypoxaemia. Brain regions underlying decision making may be affected by OSA-related hypoxaemia.

[Non-CPAP therapies in obstructive sleep apnoea: an overview].

Optimal treatment of the obstructive sleep apnoea syndrome (OSAS) requires an individually designed and interdisciplinary approach. Continuous positive airway pressure (CPAP) is accepted as the first line therapy for patients with OSAS. However, non-CPAP therapies may be indicated as supportive therapeutical approach in CPAP failure or as an alternative approach in CPAP intolerance. Overall, the level of evidence for the majority of non-CPAP therapies is low. Mandibular advancement devices as a medical non-CPAP treatment have proven to reduce respiratory disturbances to a level which may be su…

Restless legs syndrome and periodic leg movements of sleep.

Women are more commonly affected than men by restless legs syndrome, and prevalence is highest amongst those of northern European heritage. The motor manifestations include nonvolitional myoclonus (periodic leg movements). Disinhibition of spinal sensorimotor circuits may underlie these primary features and can be affected by peripheral as well as supraspinal networks. Insufficient mobilizable iron stores increase expressivity in some individuals. The sensorimotor features are relieved by dopamine, especially dopamine agonists, gabapentin and its derivatives, and opioids. A diagnosis relies on

Orthostatic hypotension and subjective sleep quality in older people.

Poor sleep quality and orthostatic hypotension are common complaints in an older population, and both are related to factors such as polypharmacy and depression. However, it is not known whether there is a direct association between the two. Our objective is to investigate a potential association between orthostatic blood pressure response and subjective sleep quality in older people. A within-subjects, cross-sectional design embedded in a larger longitudinal study design. Participants were recruited from the community to visit the TRIL clinic at St James's Hospital, where they underwent a str…

[Circadian regulation of sleep-wake cycles and food anticipation].

The circadian clock is crucial for efficient physiological function and drives the temporal regulation of the sleep-wake state, metabolism, and behavior. The timing of food intake and the accompanying behavior are both controlled by the internal clock, which is located in the suprachiasmatic nucleus (SCN) of the anterior hypothalamus. The SCN is considered as the master clock because the circadian rhythms for most physiological and behavioral processes are terminated after SCN ablation. The molecular framework of circadian oscillations can be best studied in the SCN. A "core" set of circadian …

Rem sleep brady-arrhythmias: an indication to pacemaker implantation?

OBJECTIVES: Important adjustments in the autonomic nervous system occur during sleep. Bradycardia, due to increased vagal tone, and hypotension, caused by reduction of sympathetic activity, may occur during non rapid eye movement (REM) sleep (NREM). Increased sympathetic activity, causing increased heart rate, is conversely a feature of phasic REM sleep. During REM sleep, sinus arrests and atrioventricular (AV) blocks unrelated to apnea or hypopnea have been described. These arrhythmias are very rare and only a few cases have been reported in the literature. PATIENTS/METHODS: Following an ECG…

Low-grade albuminuria in children with obstructive sleep apnea.

Small urinary protein loss (low-grade albuminuria or microalbuminuria) may reflect altered permeability of the glomerular filtration barrier. In the present study, it was hypothesized that children with obstructive sleep apnea have an increased risk of microalbuminuria compared with control subjects without sleep-disordered breathing. Albumin-to-creatinine ratio was measured in morning spot urine specimens collected from consecutive children with or without snoring who were referred for polysomnography. Three groups were studied: (i) control subjects (no snoring, apnea-hypopnea index < 1 episo…

Short sleep duration and its association with energy metabolism.

A growing body of observational evidence suggests that short sleep duration is a risk factor for the development of obesity. Although increased energy intake is the most prevailing causal explanation for this association, we should also consider possible effects on energy metabolism to understand fully the potential impact of short sleep duration on the regulation of energy balance. We performed a search of the literature from 1970 to 2011, including original papers, investigating the relation between short sleep and energy metabolism in animals and humans. Although the limited number of exper…

Actigraphic assessment of sleep in chronic obstructive pulmonary disease.

PURPOSE: Previously, sleep in chronic obstructive pulmonary disease (COPD) has been objectively investigated only by lab-based polysomnography. The main purpose of this study was to evaluate sleep quality in COPD patients in their home environment using actigraphy. We also investigated the factors associated with sleep impairment and the relationship between objective and subjective sleep quality and daytime somnolence in these patients. METHODS: Twenty-six patients with moderate to very severe COPD and 15 controls were studied by actigraphy for at least 5 days. Subjective sleep quality was e…

Subjective sleep problems in patients with early Parkinson's disease.

BACKGROUND AND OBJECTIVE: Sleep problems are common in Parkinson's disease (PD) and increasingly so with disease progression. The frequency of these problems and the influence of dopaminergic treatment on sleep in early stages of PD remain unclear. We have therefore in this study examined the subjective experience of sleep problems in drug-naïve patients with early PD and how these problems developed after 1 year on dopaminergic treatment using the Parkinson's Disease Sleep Scale (PDSS). METHODS: In all, 138 drug-naïve patients with early PD derived from a population-based incident cohort and…

Sleep disordered breathing in patients with primary ciliary dyskinesia.

BACKGROUND: Upper airway manifestations of primary ciliary dyskinesia (PCD) can cause obstructive sleep apnea syndrome (OSAS). Also abnormalities of lung mechanics and gas exchange may lead to sleep abnormalities in these patients. OBJECTIVES: To determine the rate of OSAS and sleep quality in PCD patients, and whether these are related to upper respiratory system manifestations and severity of lung disease in these patients. METHODS: Twenty-nine PCD patients and healthy controls were included to the study. Respiratory symptoms within the previous month were separately scored with the severi…

[Paroxysmal feeling of suffocation during sleep--a diagnostic challenge].

Our patient suffered from nocturnal awakenings due to paroxysmal feelings of suffocation for three years. He was extensively examined at the community health centre and at the departments of neurology, psychiatry, oral diseases and respiratory medicine of the district hospital and the university hospital. The clinical hunch of the night nurses of the department of respiratory medicine and video EEG monitoring recommended by the sleep disorders team eventually revealed the real cause of the patient's symptoms.

Topography-specific spindle frequency changes in obstructive sleep apnea.

BACKGROUND: Sleep spindles, as detected on scalp electroencephalography (EEG), are considered to be markers of thalamo-cortical network integrity. Since obstructive sleep apnea (OSA) is a known cause of brain dysfunction, the aim of this study was to investigate sleep spindle frequency distribution in OSA. Seven non-OSA subjects and 21 patients with OSA (11 mild and 10 moderate) were studied. A matching pursuit procedure was used for automatic detection of fast (≥13 Hz) and slow (<13 Hz) spindles obtained from 30 min samples of NREM sleep stage 2 taken from initial, middle and final night thir…

Gender-related differences in the obstructive sleep apnea syndrome.

The obstructive sleep apnea syndrome (OSAS) is associated with increased morbidity and mortality. Epidemiological studies suggest a two-fold higher prevalence of OSAS in men than in women. Differences in pharyngeal collapsibility and central respiratory drive may play a significant role in the gender-specific pathogenesis. Compared with their male counterparts, female patients experience OSAS at an older age (postmenopausal) and with higher body-mass-index. Female OSAS patients furthermore more frequently report atypical symptoms, such as insomnia, depression, and/or restless legs, and thus ar…

Reversal of central sleep apnea following discontinuation of opioids.

This case report including polysomnography shows reversal of central sleep apnea after opioid withdrawal. A patient using opioids for pain was diagnosed with central sleep apnea (CSA), but was not compliant with therapy. Five years later he underwent detoxification. Subsequent polysomnography showed no sleep disordered breathing. This is the first report of correction of CSA following opioid withdrawal confirmed by polysomnogram.
