Measuring psychological change after trauma: Psychometric properties of a new bi-directional scale.

OBJECTIVE: The current scientific measures of posttraumatic changes in the wake of major stressors have mostly been unidirectional. This study is an attempt to develop a scale that will capture the continuum of positive to negative psychological changes after trauma. METHOD: Forty-five statements were presented to a veteran sample (N = 4,053) with the request to report for each item their experiences of negative, positive, or no posttraumatic changes as a result of their deployment to Afghanistan. RESULTS: Principal component analysis brought out 4 dimensions; 26 nonoverlapping items that ha

Oveporexton, an Oral Orexin Receptor 2-Selective Agonist, in Narcolepsy Type 1.

BACKGROUND: Narcolepsy type 1 is a disorder of hypersomnolence caused by a loss of orexin neurons, which results in low orexin levels in the brain. METHODS: In this phase 2, randomized, placebo-controlled trial, participants with narcolepsy type 1 received once- or twice-daily oveporexton (TAK-861), an oral orexin receptor 2-selective agonist, or placebo. The primary end point was the mean change from baseline to week 8 in average sleep latency (the time it takes to fall asleep) on the Maintenance of Wakefulness Test (MWT) (range, 0 to 40 minutes; normal, ≥20). Secondary end points included t

Analysis of twenty-four hour heart rate variability in patients with panic disorder.

Growing evidence suggests that alterations in autonomic function contribute to the pathophysiology of panic disorder (PD). This retrospective study employed 24-h heart rate variability (HRV) analysis of Holter records to compare autonomic function in PD patients (n = 38) with healthy, age- and gender-matched controls. Both time and frequency domain measures were calculated, and a circadian rhythm analysis was performed. The SDNN index, 5-min total power, very low frequency (VLF) and low frequency (LF) power were significantly lower in panic patients relative to controls over the 24-h period. H

Elevated parkinsonism symptoms in autism during middle and older adulthood are linked with psychosocial, physical health, and mental health outcomes.

Evidence is growing for a link between parkinsonism, or the motor symptoms associated with Parkinson's Disease and autism. However, research to date has yet to examine whether the presence of these motoric symptoms impacts critical adult outcomes in autism. Therefore, the current study utilized a screening measure to bifurcate a relatively large (n = 379) sample of middle and older age autistic adults (40-83 years) into parkinsonism screen positive (n = 119) versus parkinsonism screen negative (n = 260) groups in order to compare them on broad metrics of daily living skills and subjective qual

Side effects induced by bright light treatment for seasonal affective disorder.

BACKGROUND: Bright light treatment has become an accepted treatment for seasonal affective disorder (SAD), but there have been few studies about adverse effects from using this treatment. We conducted a study to examine the frequency of adverse effects of bright light treatment for SAD. METHOD: Thirty patients meeting DSM-III-R criteria for major depression, recurrent, with seasonal pattern as determined by the Structured Clinical Interview for DSM-III-R were administered a structured interview, the 42-item Systematic Assessment for Treatment Emergent Events, to assess side effects from brigh

[Clinical characteristics of psychopathologic changes induced by medicinal anti-epileptic therapy].

On the basis of 48 clinical observations the authors describe some psychopathological changes due to certain anticonvulsive drugs used in the treatment of epilepsy (phenobarbital with diphenin or hexamindin, phenobarbital with finlepsin and bensodiazepine). The report contains an analysis of the causes leading to complications: a relative and absolute overdosage, a negative and positive interference of drugs.

Automated Large-Scale Extraction of Nanotoxicity Data from the Literature Using Multimodal Large Language Models.

Background: Sleep traits have been suggested to correlate with various diseases, but most evidence is based on subjective sleep measurement. We investigated the associations of accelerometer-derived objective sleep traits with diseases throughout physiological systems to ascertain whether the disease spectrum related to objective sleep traits differs from that related to subjective sleep traits. Methods: In 88,461 UK Biobank (UKB) adults wearing accelerometers, multiple dimensions of sleep were objectively derived: (a) nocturnal sleep duration and onset timing, (b) sleep rhythm (relative ampli

Risk of developing sleep disorders and psychologic comorbidity in children with inflammatory skin diseases-A population-based study.

BACKGROUND: Inflammatory skin diseases such as atopic dermatitis (AD), psoriasis, and urticaria are associated with impaired mental health and sleep. In children the risk of developing these comorbidities is still poorly evaluated. METHODS: Retrospective data analysis of a US Collaborative Network including children of 55 health care providers was performed. Data from electronic health records of patients with a diagnosis of either AD, psoriasis, or urticaria, as well as a control cohort were retrieved in an anonymized batch format and propensity score matched. RESULTS: Children with psorias

Sleep disorders in Parkinson's disease.

We sought to estimate the frequency and nature of sleep disturbances in Indian Parkinson's disease (PD) patients. One hundred forty nine consecutive PD patients attending the Movement Disorders Clinic of the All India Institute of Medical Sciences, New Delhi, India and 115 age-matched healthy controls participated. After clinical evaluation, sleep assessment was done using a 23-question, validated sleep questionnaire. Mean age of PD patients and the duration of illness were 58.37 (S.D. 10.45) years and 5.7 (S.D. 3.85) years, respectively. The mean age of the controls was 56.50 (S.D. 11.45) yea

Primary sleep disorders can cause long-term sleep disturbance in patients with autoimmune mediated limbic encephalitis.

OBJECTIVES: Antibody mediated limbic encephalitis causes a sub acute encephalopathy with an amnestic syndrome, seizures and often an affective prodrome. Sleep disturbance including abnormal dream sleep and insomnia are described in a percentage of long-term survivors but there are very few detailed assessments of sleep disturbance in patients beyond the acute phase of illness. The objectives of this study were to understand the causes of sleep disturbance in the long-term survivors of antibody mediated limbic encephalitis. PATIENTS AND METHODS: We screened twelve patients under long-term foll

Midlife sleep characteristics and Alzheimer's disease biomarkers 20 years later.

INTRODUCTION: Poor sleep characteristics are associated with Alzheimer's disease (AD) among older adults; however, this relationship remains underexplored earlier in life. We investigated how early midlife sleep relates to AD-related biomarkers in late midlife. METHODS: A total of 1325 adults (mean age = 40.3 years) self-reported sleep quality, sleep duration, daytime sleepiness, and insomnia symptoms. Twenty years later, we assessed plasma amyloid beta (Aβ)42/40, tau phosphorylated at threonine 217 (p-tau217), neurofilament light chain (NfL), and magnetic resonance imaging (MRI)-derived Spat

Association Between Subjective Sleep Quality and Future Risk of Falls in Older People: Results From LOHAS.

BACKGROUND: Inadequate sleep is correlated with morbidity and mortality among older adults. However, the longitudinal relationship between subjective sleep quality and risk of falls in the elderly population remains to be clarified. METHODS: Study participants were from Locomotive Syndrome and Health Outcome in Aizu Cohort Study (LOHAS) sites (1,071 community-dwelling people ≧65 years of age, mean: 71 years). Subjective sleep quality was measured by the Pittsburgh Sleep Quality Index (PSQI). Occurrence of falls (defined as experiencing at least one fall) during the subsequent year was ascerta

Measuring spike strength in patients with continuous spikes and waves during sleep: comparison of methods for prospective use as a clinical index.

OBJECTIVE: To compare methods of estimating spike strength as a potential index in the assessment of continuous spikes and waves during sleep (CSWS). METHODS: Spikes were searched and averaged automatically from pre- and postoperative EEGs of ten patients with CSWS who underwent corpus callosotomy (eight) or resective epilepsy surgery (two). From the most prominent spike, we measured peak amplitude and root mean square (RMS) over ±150ms window around the peak. In order to compensate for spatiotemporal instability of spikes, the cumulative amplitude and RMS were computed from the highest quart

The treatment of sleep disorders.

Narcolepsy is clinically associated with cataplexy, sleep paralysis and hypnagogic hallucinations. It is treated by reassurance (that there is no physical disease) and by stimulants such as ephedrine and amphetamine on an intermittent basis. The special tricyclic antidepressant clomipramine is also used, and mono-amine oxidase inhibitors (MAOIs) are useful in theory. Obstructive sleep apnoea is an important and often unrecognised cause of daytime somnolence. It is treated by weight reduction (pickwickian syndrome), hormones, or recently, with continuous positive pressure apparatus. Night terro

[Sleep and wake-cycle as circadian rhythm (author's transl)].

Sleep-wake cycles are closely tied with the circadian period of other bodily functions. Optimal synchronization is essential for troublefree alterations of sleep and wake periods. The neurophysiologic control areas are in various of the brain are far apart from each other for which reason a great variety of possible causes for sleep difficulties do exist. It has been shown recently that spezific mono-aminergic and cholinergic systems in the brain stem do control sleep. A case study will show that it is possible to improve sleeping difficulties with the serotonin precursor L-Troyptophan.
