From teratogens to trajectories: Integrating teratology and Developmental Origins of Health and Disease (DOHaD) frameworks for a new take on fetal alcohol spectrum disorders (FASD).

Research on the effects of prenatal alcohol exposure (PAE) developed within two research frameworks that progressed largely in parallel. Classical teratology was the dominant framework for many years, focusing on prenatal exposure to toxic agents that disrupted developmental processes, resulting in birth defects and neurobehavioral deficits. By contrast, recent studies have positioned PAE within the Developmental Origins of Health and Disease (DOHaD) framework, which suggests that low birth weight and other early life exposures reprogram developmental trajectories, increasing susceptibility to

[Expert consensus on home management of colorectal cancer patients with enterostomy (2026 version)].

Enterostomy constitutes a crucial intervention in the comprehensive treatment of colorectal cancer, often resulting in significant alterations in patients' body image and defecation patterns post-surgery. During the home-based rehabilitation phase, individuals with an enterostomy encounter not only technical challenges associated with stoma care and the prevention of complications but also psychosocial difficulties such as psychological stress, role adaptation, and barriers to social reintegration. The adoption of evidence-based home management strategies has the potential to enhance patients'

A multidisciplinary clinic model for alcohol use disorder after liver transplantation: Implementation and early outcomes of the LIFT Clinic.

Return to alcohol use after liver transplantation is a major clinical challenge, with limited structured approaches for prevention and management in the post-transplant setting. To address gaps in post-transplant alcohol use disorder (AUD) care, we established the Long-term Individualized Follow-up after Transplant (LIFT) Clinic, a co-located multidisciplinary program integrating hepatology, psychiatry, social work, and health navigation. We conducted a retrospective cohort study of 113 patients referred between December 2022 and December 2024. Fifty patients attended at least one clinic visit

Gut microbiota reshapes host energy metabolism to modulate depressive behaviors.

Disturbances in energy metabolism are a key pathophysiological feature of major depressive disorder (MDD). The gut microbiota, as a critical regulator of host metabolism, may influence systemic energy homeostasis and contribute to depression. To investigate this, we performed a multi-omics analysis integrating targeted metabolomics and shotgun metagenomics on samples from 100 MDD patients and 68 healthy controls. MDD patients exhibited significant disruptions in central energy pathways (glycolysis, TCA cycle, and ornithine cycle), which correlated with symptom severity and cognitive impairment

HIV/HCV prevalence and the retention paradox in marginalized populations in Madrid (2019-2023): A large-scale cross-sectional study.

OBJECTIVES: Marginalized urban populations experience a high burden of HIV and HCV. We assessed the prevalence and risk factors for both infections and characterized the care cascades in a cohort of vulnerable individuals in Madrid, Spain. METHODS: We conducted a cross-sectional study (2019-2023) of 4582 individuals via mobile units in high-risk hotspots, offering integrated rapid HIV/HCV testing with point-of-care HCV-RNA confirmation. Multivariable logistic regression identified independent HIV risk factors. RESULTS: HIV prevalence was 6.3% (95% CI: 5.6-7.0). Among people with HIV (PWH), 1

Investment in family medicine to improve health outcomes in sub-Saharan Africa.

Family physicians (FPs), as educators, capacity builders and clinical governance leaders of primary care (PC) teams, work to make them more effective and responsive to the needs of their patients. Various strategies are required to raise the profile of Family Medicine (FM) to ensure stronger representation in health sector planning and policy development for advocacy on behalf of the communities they serve. An illustration is given of the need for FP leaders to become equal partners in the National Surgical, Obstetric and Anaesthesia Planning process to ensure safer surgery at district hospita

Disrupting integrator complex subunit INTS6 causes neurodevelopmental disorders and impairs neurogenesis and synapse development.

The Integrator complex plays essential roles in RNA polymerase II (RNAPII) transcription termination and RNA processing. Here, we identify INTS6, a subunit of the Integrator complex, as a novel gene associated with neurodevelopmental disorders (NDDs). Through analysis of large NDD cohorts and international collaborations, we identified 23 families harboring monoallelic likely gene-disruptive or de novo missense variants in INTS6. Phenotypic characterization revealed shared features, including language and motor delays, autism, intellectual disability, and sleep disturbances. Using a nervous-sy

A conceptual and computational framework for modeling the complex, adaptive dynamics of epidemics: The case of the SARS-CoV-2 pandemic in Mexico.

In the quest to ensure adequate preparedness for health emergencies caused by infectious disease pandemics, there is a need for tools that can address the myriad relevant questions related to the spread and trajectory of pandemics. A hybrid intelligence model that combines human and artificial intelligence may provide a viable solution, as it can process data from models that comprehensively integrate contextual and direct factors, effectively mimicking the social processes surrounding transmission while incorporating human interpretation to enhance our understanding of pandemics. Using data f

Equity in epidemic response: an action-oriented framework for guiding public health in equitable responses to major infectious disease emergencies.

BACKGROUND: A rapid and equitable response is paramount to mitigating the spread and impact of an infectious disease public health emergency. Unfortunately, public health responses often integrate equity as a secondary component rather than a foundational one-a decision that can result in disproportionate effects of the epidemic on vulnerable populations and that may further fuel or worsen the ongoing health emergency. This paper introduces a framework grounded in health equity principles to guide the design and implementation of response efforts during infectious disease emergencies. METHODS

A probability integral method modified model for accurately characterizing subsidence at the boundary of a mining area.

The "West-East Coal Transmission" project is an important resource allocation project in China, aiming to alleviate the shortage of coal resources in China's economically developed eastern cities. Ensuring coal transportation railway safety is an important part for the smooth running of this project. However, China is a large coal mining country, and some railways inevitably pass through the coal mining influence areas, seriously threatening the structural health of the railways. The probability integral method (PIM) is an official prediction model for mining subsidence in China, while it is d

Differential Association of Preadolescent Risk Factors Across Developmental Patterns of Adolescent Concurrent Gambling Participation and Substance Use.

Early risk factors for gambling participation (GP) and substance use (SU) in adolescents have usually been studied separately, although these disorders were integrated into the same clinical category over a decade ago. This exploratory study aimed to investigate the early individual, parental, familial and social risk factors associated with developmental patterns of adolescent GP and SU in a population-representative cohort (N = 1594, 51.2% boys). Using a person-centered strategy and multiple assessments from age 12 to 17, six developmental patterns describing joint GP and SU courses were rev

Supplementation with stigma maydis polysaccharide attenuates autism-like behaviors and improves gut function in valproic acid-induced autism model male rats.

Stigma maydis polysaccharide (SMPS) has regulatory effect on the intestinal microflora and promotes gastrointestinal peristalsis. Children with autism spectrum disorder (ASD) often experience gastrointestinal problems and dysbiosis in their gut microbiota. Our previous study revealed that SMPS interventions had an impact on the gut microbiota of valproic acid (VPA)-induced autism model rats. However, the effects of SMPS on the behavior and gut function of autism model rats remain poorly understood. Therefore, we gave different doses of SMPS intervention in the early stage of autism model rats
