An unusual oesophageal foreign body in neonate - case report.

BACKGROUND: Foreign bodies in the esophagus in children are usually swallowed accidentally during play. An oesophageal foreign body in neonate is rare and thus the diagnostic work-up may be difficult. CASE REPORT: We present a case of an 8-day-old baby with symptoms of oesophageal obstruction. An unusual foreign body was found. CONCLUSIONS: Foreign bodies in neonate's esophagus are rare; non-accidental action should be suspected. Atypical clinical and radiological symptoms may cause diagnostic difficulties.

[Novel methods for the assessment of sensory, biomechanical and motor functions of the gastrointestinal tract].

The article gives a review of new methods in gastroenterology. The >>functional lumen imaging probe<< is a method used to evaluate the competence of sphincter regions during distension. The axial force probe can measure forces in the oesophagus during swallowing and give new information about patients with motor disorders of the oesophagus. The >>multimodal probe<< measures the sensory response to electrical, mechanical, thermal and chemical stimuli. Together with advanced modeling of the brain activity, these methods have increased our knowledge about the pathogenesis of sensory symptoms.

[Usefulness of video-assisted cervicotomy on the extraction of foreign bodies].

Foreign bodies located in the upper aerodigestive tract, if not diagnosed and taken out speedily, may be the cause of great morbimortality. We present a female patient who arrived at the emergency department reporting she had swallowed a fish bone; it was finally located in the pre-vertebral muscle at the level of C3-C4, where it had entered from the oesophagus at 2 cm from the superior oesophageal sphincter. We used a cervicotomy assisted by endoscopy, showing a new application for videoendoscopy.

Freeman-Sheldon syndrome. A case report and review of the literature.

The Authors describe a case of Freeman-Sheldon Syndrome, a rare congenital autosomal dominant disorder (gene mapped on chromosome 11p15.5) characterized by microstomia with crinkled lips, camptodactyly with ulnar deviation of the fingers and equinus-varus-supine clubfoot. The autosomal recessive form, even rarer and difficult to recognize, has a more severe clinical manifestation. The symptomatology is worsened by breathing and swallowing disorders due to the small orifices of the mouth and nose, which sometimes require tracheotomy to avoid obstruction of the airways.

[The case of gastric and ileal bezoar in a 3 year old boy].

The authors report a case of a 3-year-old boy with a gastric and ileal bezoar, in whom severe anaemia and tumor in the epigastrium were the main symptoms. Bezoars are rare foreign bodies of the gut. They contain swallowed hair, different fibres, seeds, nutshells, sweets or drugs. The group of risk factors for bezoar formation includes anatomical defects, motoric disorders of the digestive tract, type of prescribed drugs and other coexisting chronic diseases. Most cases need surgical treatment.

Myasthenia gravis masquerading as post-poliomyelitis syndrome.

A 79-year-old man with previous bulbar poliomyelitis developed dysphagia and was diagnosed as having post-polio syndrome. Over 2 years, his swallowing deteriorated and he suffered an aspiration pneumonia. Only after the subsequent development of fatigue and facial weakness was myasthenia gravis diagnosed. Diagnostic criteria for post-polio syndrome include the exclusion of all other neurological conditions such as myasthenia gravis. Moreover, in any instance where a patient develops new symptoms, it is advisable to reconsider the original diagnosis.

Pharyngo oesophageal strictures and its reconstruction by delto pectoral flaps.

Delto pectoral flap is a thin and pliable cutaneous axial flap suitable for the reconstruction of pharynx and upper oesophagus. Corrosive stricture of the pharyngo oesophageal area can very well be reconstructed by deltopectoral flap. Oesophageal reconstruction by viscera like stomach or colon often produces anastomic stricture at the upper anastomic site with hypopharynx or upper oesophagus. Delto pectoral flap can also be used to reconstruct this anastomotic stricture. Postoperative swallowing is satisfactory when reconstruction is done with this flap.

Inhalation of tobramycin in a patient with aspiration pneumonia as a result of medullary stroke.

A 78-year-old man suffered from refractory aspiration pneumonia as a result of a minor medullary stroke. The only neurological symptom observed in this patient was difficulty in swallowing. He was managed with i.v. hyperalimentation with termination of oral intake, including water. However, he still experienced several episodes of aspiration pneumonia. As he was considered to have a bacterial infection because of silent aspiration of colonized oropharyngeal material, inhalation of tobramycin was introduced and successful control of airway infection was attained.

An unusual presentation of congenital bronchoesophageal fistula.

We present the case of a 5-week-old neonate with multiple congenital abnormalities including a broncho-oesophageal fistula, which showed radiological features suggestive of congenital diaphragmatic hernia. Emergency limited barium swallow done was initially reported as a case of diaphragmatic hernia. Autopsy revealed pus within the right lung, and a fistulous connection between the oesophagus and an intralobar sequestrated lung. No diaphragmatic hernia or intra-abdominal organ abnormality were seen, and an occipital meningomyelocoele was also confirmed.

Oral involvement in a case of AA amyloidosis.

A case of oral involvement of amyloid associated (AA) amyloidosis (secondary systemic amyloidosis) in a patient who is suffering from psoriatic arthritis and ankylosing spondylitis, is described. The patient was complaining of pain in the oral cavity (burning mouth), especially on the tongue, and difficulty in chewing and swallowing foods. Oral examination revealed a few papules on the dorsum of the tongue associated with xerostomia. In addition, two slightly painful, small ulcers, localized on the vestibule of the mouth were observed. Biopsy of the tongue demonstrated amyloid deposits.

Oesophageal leiomyoma presenting as a mediastinal mass.

A 28 year old male presented with complaints of retrosternal pain, discomfort and dysphagia of 4 years duration. Barium swallow and oesophagoscopy were suggestive of extrinsic compression of thoracic oesophagus. CT scan of chest was suggestive of a large mediastinal lymph node mass. Thoracotomy and open biopsy showed a benign mesenchymal tumor on frozen section. A transthoracic oesophagectomy with gastric pull up and cervical oesophago-gastric anastomosis was performed. The postoperative course was uneventful and the patient discharged on the tenth postoperative day.

The management of dysphasia in skeletal hyperostosis.

Diffuse idiopathic skeletal hyperostosis (DISH), or Forestier's disease, is an ossifying condition frequently encountered in otolaryngology as it affects 12-28 per cent of the adult population. This form of hyperostosis can manifest clinically with dysphagia, food impaction, hoarseness, stridor, myelopathies and other neurological problems. Judicious management of severe dysphagia proves challenging. The failure of conservative care often leaves surgery as the only option. In this report an anterolateral transcervical surgical approach to the confluent osteophytes is discussed and the value of

Primary osteogenic sarcoma of the tongue.

A 56-year-old man presented with the difficulty of swallowing and respiratory distress due to a large tumour arising from the tongue and occupying the entire oral cavity. Histological examination revealed it to be an extraskeletal osteogenic sarcoma. The tumour was excised. After six weeks, he came back with massive local recurrence and bleeding from the tumour, but died despite chemotherapy. Review of the literature revealed only four other such cases of this rare tumour. A brief review of these four cases is also made.

Leiomyoma of the esophagus and bronchus in a child.

Leiomyomas of the esophagus and/or the bronchus have rarely been reported in children. To our knowledge, the simultaneous presence of this tumor in both the esophagus and a bronchus in a child has not been previously reported. A 7-year old boy presented with respiratory and esophageal symptoms and was found to have a leiomyoma of the esophagus and right main bronchus. The esophageal leiomyoma was treated with limited myotomy, but bronchoscopic resection was possible for the bronchial lesion. The postoperative result was excellent, with normal swallowing and no residual respiratory problems at

Heterogeneity in adducted thumbs sequence.

We report on a boy with adducted thumbs, microcephaly, swallowing difficulties, hypotonia, and severe mental retardation, but without craniostenosis or arthrogryposis. An MRI scan showed myelinization according to age and mild ventricular enlargement. A muscle biopsy documented irregular-shaped and swollen mitochondriae, but results of mitochondrial function tests were normal. The clinical findings were consistent with a developmental defect of the central nervous system. We include a brief review of the 9 reported cases with adducted thumbs sequence.

Fracture of the thoracic spine with paralysis and esophageal perforation.

A 17-year-old young man presented with a highly unstable fracture dislocation of the third and fourth thoracic vertebrae with neurological deficit, in which the fractured spine had perforated the thoracic esophagus. Open reduction and internal fixation of the spinal fractures in combination with aggressive treatment of the mediastinitis caused by esophageal perforation, consisting of two re-thoracotomies, was performed. Two years after the accident, the patient had recovered well. The neurological deficit had recovered, and there were no difficulties with swallowing.

Identification of Blastomyces dermatitidis in the stool of a dog with acute pulmonary blastomycosis.

We report the identification of Blastomyces dermatitidis by microscopic examination of a direct faecal smear from a dog with pulmonary blastomycosis. A simultaneously obtained faecal culture grew Blastomyces dermatitidis. The fungus was also cultured from a transtracheal sample from this same dog. This report suggests that yeast-phase cells of B. dermatitidis may be recovered in the stool of dogs with pulmonary blastomycosis following transit through the gastrointestinal tract of swallowed infected sputum. Implications regarding the ecology of Blastomyces dermatitidis are discussed.

Ukrain treatment in carcinoma of the oesophagus (case report).

A poorly differentiated squamous cell oesophageal carcinoma was diagnosed in a patient after an 11 month history of difficulty in swallowing. The condition was inoperable according to clinical and X-ray contrast examinations. Radiation therapy with three courses of chemotherapy was carried out, but no amelioration was obtained. Ukrain therapy (course of 46 ampoules) was undertaken; this changed the situation entirely: all the subjective problems disappeared and no residual cancer was seen on the X-rays. Complete remission is seen nearly four years after Ukrain therapy.

[Conservation surgery for supraglottic carcinoma. Oncological and functional results].

Conservative surgical treatment of carcinomas limited to the supraglottic larynx has been shown to be an oncologically and functionally effective therapeutic approach. A retrospective study was made of the medical records of 792 patients who underwent conservative surgery for supraglottic laryngeal carcinoma at the Gregorio Marañón University Hospital of Madrid, Spain, in 1962-1991. Our theoretical protocol for treating supraglottic tumors is presented. The oncological results (survival, local control, recurrences) and functional results (breathing, swallowing, voice quality) were analyzed.

Toothpick perforation of the intestine diagnosed by a small bowel series.

We report the case of a 50-yr-old man who unknowingly swallowed a double-pointed toothpick that ultimately perforated the distal small bowel over a period of several weeks. Appendicitis and ileitis were sequentially considered as diagnosis. A foreign body was diagnosed on a second small bowel series and removed. It has been noted that, because toothpicks are not radiopaque, radiographic studies are not useful in the diagnostic workup. In the case reported, the toothpick was seen on the contrast radiograph of the small bowel and the correct diagnosis ultimately made.
