Partial-thickness articular surface rotator cuff tears in patients over the age of 35: Etiology and intra-articular associations.

PURPOSE: Partial-thickness articular-sided rotator cuff tears have a multifactorial etiology and are associated with degeneration of the tendon. They are often described as an injury of the young athlete, although they are also found in the older population. The aim of this study was to investigate the frequency and associations of partial-thickness articular-sided tears in patients over the age of 35 years. DESIGN: Retrospective MATERIALS AND METHODS: A retrospective study of all arthroscopic procedures for rotator cuff pathology in patients over the age of 35 years over a 2-year period by

Defining the danger zone for peroneal nerve safety in all-inside lateral meniscus repair: an MRI-based study.

BACKGROUND: The peroneal (fibular) nerve (PN) traverses the posterolateral aspect of the knee joint, placing it at risk during lateral meniscus (LM) repair, given its proximity to the popliteus tendon (PoT). OBJECTIVE: This study aimed to evaluate the risk of PN injury and define a safe anchor penetration angle during all-inside LM repair. MATERIALS AND METHODS: A retrospective analysis was conducted using magnetic resonance imaging (MRI) scans of 327 individuals aged 18-60 years. The patellar tendon (PaT) and PoT widths were measured. The PN location relative to the PoT was determined. Dist

Anatomical quantitative evaluation of popliteal artery visualization during medial approach in supine position.

PURPOSE: The medial approach to the popliteal artery has been less commonly used than the posterior approach in surgical repair of traumatic popliteal injury. This study was performed to quantitatively evaluate the visual field of the popliteal artery obtained by staged myotendotomy in the medial approach to the popliteal artery. METHODS: Twenty legs of fresh-frozen adult cadavers were dissected using the medial approach to the popliteal artery. In stage 1, the popliteal artery was exposed between the vastus medialis and sartorius muscles without myotendotomy. In stage 2, the medial head of t

Open knee dislocation, triple intra-articular fractures and patellar tendon rupture: case report of a knee disaster treated with aggressive irrigation/debridement, early anatomic reduction and internal fixation.

BACKGROUND: Open knee fracture-dislocation is a rare orthopedic injury. However, the importance of its correct management could not be overstated. To the best of our knowledge, this is the fifth study reporting a case with simultaneous Hoffa fracture and knee dislocation and the 1st study describing a patient with open plateau fracture-dislocation accompanied with Hoffa fracture, patella fracture, and patellar tendon tear. In addition, this report is noticeable as our case had no gross ligament injury unlike frequent association of knee dislocation with knee collateral ligament damage. CASE P

Suture-button Fixation and Arthroscopic Dorsal Ligamento-capsulodesis in Chronic Scapholunate Dissociation.

The treatment choice in scapholunate (SL) injury depends on the extent of the SL ligament tear, chronicity of injury, quality of the ligament remnants, reducibility of carpal malalignment, and cartilage status of the radiocarpal and midcarpal joints. In the absence of degenerative changes with chronic reducible dissociation, the optimal treatment would be the reconstruction of the SL interosseous ligament. Various SL reconstruction techniques via open or arthroscopic approaches have been described over the years; they include tendon reconstructions, volar/dorsal capsulodesis, SL allografts, bo

The Learning Curve and Pitfalls of Arthroscopic Four-Corner Arthrodesis.

Background  Midcarpal "four-corner" wrist arthrodesis may be done from an open arthrotomy or arthroscopically. Purpose  This study aimed to examine the results of the recently described arthroscopic four-corner arthrodesis and whether the procedure seems to have any merit compared with the open technique. Patients and Methods  We retrospectively identified eight patients with nine cases of arthroscopic four-corner arthrodesis performed at our institution, 2014 to 2017. The underlying pathologies were scapholunate advanced collapse ( n  = 6), Preiser's disease ( n  = 1), radioscaphoid ( n  = 1)

Achilles tendon stiffness is unchanged one hour after a marathon.

Overuse-induced injuries have been proposed as a predisposing factor for Achilles tendon (AT) ruptures. If tendons can be overloaded, their mechanical properties should change during exercise. Because there data are lacking on the effects of a single bout of long-lasting exercise on AT mechanical properties, the present study measured AT stiffness before and after a marathon. AT stiffness was determined as the slope of the force-elongation curve between 10 and 80% of maximum voluntary force. AT force-elongation characteristics were measured in an ankle dynamometer using simultaneous motion-cap

Quadriceps muscle contraction protects the anterior cruciate ligament during anterior tibial translation.

The proposed skiing injury mechanism that suggests a quadriceps muscle contraction can contribute to anterior cruciate ligament rupture was biomechanically investigated. The effect of quadriceps muscle force on a knee specimen loaded to anterior cruciate ligament failure during anterior tibial translation was studied in a human cadaveric model. In both knees from six donors, average age 41 years (range, 31 to 65), the joint capsule and ligaments, except the anterior cruciate ligament, were cut. The quadriceps tendon, patella, patellar tendon, and menisci were left intact. One knee from each pa

Imaging Traumatic Hand and Finger Injuries.

The hands and fingers play a vital role in everyday activities, which increases the risk for accidental injury. Imaging these potentially complex injuries is vital to successful diagnosis and treatment. Imaging techniques and best practices vary; multiple modalities might be needed to achieve an accurate diagnosis. This article discusses anatomy and common injuries of the hand and fingers. Methods for successful imaging also are described. A broad understanding of anatomy and imaging techniques enables radiologic technologists to better assist with hand trauma management.

Evaluation and management of hamstring injuries.

Muscle injuries are the most common injuries in sports, with hamstring injuries accounting for 29% of all injuries in athletes. These injuries lead to prolonged impairment and have a reinjury risk of 12% to 31%. They range from mild muscle damage without loss of structural integrity to complete muscle tearing with fiber disruption. Novel MRI scores are increasingly being used and allow a more precise prediction of return to sport. In this article, the authors review the history, mechanisms of injury, and classification systems for hamstring injuries as well as present the latest evidence relat

Mallet deformity in sport.

During a four month period 851 patients presented to the Edinburgh Orthopaedic Trauma Unit with an acute sporting injury. Eighteen (2%) patients had either a soft tissue or bony mallet deformity. Six different sports were identified with rugby accounting for eight of the 18 mallet deformities. All patients were treated with splintage. Fourteen patients returned questionnaires regarding subjective digit function following treatment and nine patients claimed excellent function. The average splinting time for this group was 6 weeks and sports were avoided for an average of 5 weeks. Mallet deformi

Injection of Amniotic Membrane and Umbilical Cord Particulate for Muscle and Ligament Tears in Collegiate Football Athletes: A Single-Center, Retrospective Study.

BACKGROUND: Football is a leading cause of sports-related injuries, accounting for more than half of time-loss injuries in men's collegiate sports. Amniotic membrane and umbilical cord (AM/UC) powder injection is a novel treatment that may aid in functional recovery and return to play in collegiate athletes due to its anti-inflammatory, anti-scarring, and pro-regenerative properties. METHODS: A single-center, retrospective chart review was performed on consecutive college football players who sustained acute muscle or ligament tear and were subsequently treated with 50 or 100 mg injection of

The reanimation of the elbow functions in avulsive injuries of the upper brachial plexus using the medial cord transfer: nuances of the technique and update.

OBJECTIVES: Report on a new transfer for the reinnervation of biceps and brachialis muscles after multilevel avulsive injuries of brachial plexus provided at least T1 is viable: the Medial Cord to Musculocutaneous (MCMc) and its variant, the Medial Cord to anterior Upper Trunk (MC-aUT). The MC-aUT is indicated in agenesia of musculocutaneous nerve and when a residual function in the biceps is present. The MCMc transfer would be unfeasible in the former and contraindicated in the latter. METHODS: Three hundred and five consecutive patients, classified according to the quality of hand function,

[Avulsion of the proximal hamstring origin - report of 6 cases].

Proximal hamstring origin avulsions are rare injuries. A common cause for this kind of injury is a trauma with the hamstring in overextension and simultaneously forced hip flexion and knee extension. We report on 6 cases, 5 with an acute rupture of the hamstring origin and one case with a delayed presentation in our emergency room. In 3 cases the injury was related to sport activity, the other 3 are related to accidents during work. None of these patients took part in competitive sports. One case was reported 8 weeks after trauma with an MRI performed one week before. Due to the low functional
