Failure of immobilization of the cervical spine by the halo vest. A report of five cases.

The halo vest is considered by us, as well as by the majority of orthopaedic surgeons, to be the best commercially available orthosis for control of the cervical spine. However, we have seen five patients with posterior ligament injury of the neck who sustained a recurrence of facet dislocation or subluxation while in a halo vest.

Timing of operative care in cervical spinal cord injury.

The emphasis of this review has been on bone pathology and not on neurologic syndromes. We have not discussed those rare conditions in which the neurologic deficit progresses after injury and past authors have recommended decompressive procedures. When neurologic deficits do increase, we think this is more often an indication for further diagnostic studies to see if the alignment is proper, to rule out further compression, or increase medical support if the cardiovascular status has deteriorated. When there has been a dramatic and sudden worsening of the neurologic deficit, as occurs in 3% of

Perthes' disease treated by prolonged recumbency and femoral head containment: a long-term appraisal.

The results of a long-term review of 102 hips in eighty-seven patients with Perthes' disease are described, the mean follow-up interval being seventeen years. All had been treated by an extremely rigorous conservative regime in which the patients were kept in hospital for an average period of twenty-six months, during which time they were confined to bed with the legs in wide abduction, first in traction and later in "broomstick" plasters to ensure "containment" of the femoral head. The patients were assessed by the joint clincial and radiological method described by Ratliff (1956). The result…

Injuries of the cervical spine in football.

The neck is a vulnerable area in contact sports in general and in football in particular. The type of injuries encountered often vary with the age and development of the players. Five well-established mechanisms of injury have been identified. Most injuries appear to occur during the act of tackling when the well protected head sustains violent trauma which is transferred directly to the neck. The most dangerous single mechanism is that of flexion, but lateral deviation, extension and impaction also have been identified as mechanisms of injury. The pattern of injuries would suggest that severa

Capital necrosis, metaphyseal cyst and subluxation in coxa plana.

Three specific elements associated with a poor prognosis in Legg-Calvé-Perthes disease, i.e., capital necrosis, metaphyseal cyst and subluxation, have been selected for study to determine the effect of each on the end result. A retrospective analysis was made of 337 children who received non-weight-bearing treatment. Half of the patients were treated in abduction splints, over a third with bed rest and traction and the remainder with a variety of routines including sling and crutches, wheelchair or bed rest alone. The group was treated as representing a common pool of therapy and was analyzed …

Predictors of failed closed reduction in pediatric Gartland type III and flexion-type supracondylar humerus fractures.

BACKGROUND: Pediatric supracondylar humerus fractures (SCHFs) are among the most common childhood injuries, often requiring surgical fixatio … rameters, and radiographic characteristics were analyzed. For the Gartland type 3 fractures, gentle traction was performed while elbow is in 200 of flexion to achieve soft tissue realignment. Reduction was done in the coronal plane, and alignment was checked manually, and the elbow was flexed with forearm supination or pronation, depending on the direction …

Insulin analogues may accelerate progression of diabetic retinopathy after impairment of inner blood-retinal barrier.

Diabetic retinopathy regresses after spontaneous infarction or surgical ablation of pituitary gland. Growth hormone deficiency seems to be a … G) cell was identified to be stimulated and proliferate by IGF-1. IGF has been reported to generate traction force in retinal pigment epitelium (RPE) and mullerian cells. Mullerian cells also support inner blood-retinal barrier. Insulin analogues may cause proliferation in glial cells and generate traction force in RPE and mullerian cells by stimulating IG …

Selected clinical approaches for the treatment of classic Class II, division 1 malocclusion.

INTRODUCTION: Class II, division 1 malocclusion is the most common sagittal discrepancy, marked by distal positioning of the lower teeth, up … headgear therapy, maxillary molar distalization with temporary anchorage devices (TADs), premolar extractions, clear aligner therapy, and orthodontic-surgical approaches for severe skeletal cases. Advances in biomechanics and skeletal anchorage have expanded non-extraction and less invasive options. However, careful case selection remains essential. Diagno …

The effectiveness of novel 3D printed customized facemask appliance for treatment of class III malocclusion: a randomized clinical trial.

BACKGROUND: Utilizing 3D facial scans with the aid of digital modeling tools and additive manufacturing is a promising technique for customization of facemask (FM) appliance for correction of Class III malocclusion. OBJECTIVES: To evaluate the efficacy of a modified 3D-printed maxillary protraction FM compared with a commercially available adjustable FM for the correction of Class III malocclusion due to maxillary retrognathism. METHODS: This prospective randomized controlled clinical trial enrolled a total of 34 patients aged 7-12 years with skeletal Class III malocclusion requiring maxilla…

Is maxillary protraction the earlier the better? A retrospective study on early orthodontic treatment of Class III malocclusion with maxillary deficiency.

To investigate the optimal timing of maxillary protraction in children with Class III malocclusion to aid comprehension of this still non-consensual topic. In all, the data of 97 children with Class III malocclusion treated by using the Delaire facemask with maxillary expansion were collected retrospectively and divided into three groups according to their dentition stages; those subjects in the mixed dentition group were further divided into three subgroups. All patients were regrouped by the cervical vertebral maturation index (CVMI) and observed closely by cephalograms at the beginning of t…

Nonsurgical orthodontic treatment using bone-anchored maxillary protraction in a patient with unilateral cleft lip and palate.

Class III malocclusion due to a retrognathic maxilla is common in patients with cleft lip and palate. Skeletally anchored maxillary protraction using screw-anchored mini-plates combined with intraoral elastics has shown promising results in achieving orthopedic changes and maintaining the outcome until the completion of the growth. This case report presents the course of treatment in a patient with unilateral cleft lip and palate and multiple congenitally missing teeth treated with bone-anchored maxillary protraction until the end of growth. Four mini-plates (Bollard plates) were used during c…

Closed reduction of laterally displaced fracture/dislocation of the mandibular condylar process in a child.

This is a unique case of isolated lateral fracture/dislocation of the condylar process in a 3-year-old child injured on an escalator. Immediate closed reduction under general anesthesia successfully restored condylar height and premorbid dental occlusion. Maxillomandibular fixation was not used because mandibular mobilization was encouraged. Clinical follow-up confirmed maintenance of normal dental occlusion and temporomandibular joint function.

The intra-operative radius joystick test to diagnose complete disruption of the interosseous membrane.

Disruption of the interosseous membrane is easily missed in patients with Essex-Lopresti syndrome. None of the imaging techniques available … the interosseous membrane. On each forearm, we performed the radius joystick test: moderate lateral traction was applied to the radial neck with the forearm in maximal pronation, to look for lateral displacement of the proximal radius indicating that the interosseous membrane had been disrupted. Each of six surgeons (three junior and three senior) performe …

Two case reports of gastric ulcer from pressure necrosis related to a rigid and taut percutaneous endoscopic gastrostomy bumper.

Two complications are reported from excessively taut application of percutaneous endoscopic gastrostomy (PEG) external bumpers against the abdominal wall skin. First, a 55-year-old woman status post PEG developed a gastric ulcer, complicated by acute gastric bleeding, directly underneath the internal gastric PEG bumper. This complication was associated with replacement by an unknown healthcare worker of the standard flexible external (cutaneous) PEG bumper with an unauthorized rigid external clamp (bumper) and with excessively taut application of this clamp against the abdominal wall skin.
