A Case Report on Biliary Ascariasis.

Biliary Ascariasis occurs when Ascaris lumbricoides worms invade the biliary system. It may cause biliary obstruction, cholangitis, cholecystitis, or acute pancreatitis. We report a case of a 37-year-old female patient who presented with a history of upper abdominal pain, nausea, vomiting, and weight loss for two weeks. Ultrasound showed dilated common bile duct with linear tubular echogenic structure in the common bile duct and bowel loops. Endoscopic Retrograde Cholangio pancreatography (ERCP) revealed large adult worms confirming the diagnosis of Ascariasis.

Acute Liver Rejection in a Multiple Myeloma Patient Treated with Lenalidomide.

Herein we present a patient that underwent a liver transplant due to primary biliary cholangitis (PBC) and after 9 years developed multiple myeloma. Following the cessation of mycophenolate mofetil and 2 weeks after lenalidomide treatment was started, the patient experienced acute cellular rejection. The patient recovered after treatment with corticosteroids, resumption of mycophenolate mofetil, and cessation of lenalidomide. Lenalidomide-associated allograft rejection has been reported in other organs. However, this is the first case report of liver rejection induced by lenalidomide.

[Treatment of cholestatic liver disease].

Primary biliary cirrhosis (PBC) and primary sclerosing cholangitis (PSC) are chronic cholestatic liver diseases, biochemically characterized by an elevated serum bilirubin, alkaline phosphatase and gamma-glutamyl transferase. Although PBC and PSC have their own distinctive clinical, immunological, radiological and histological features, both diseases show a necro-inflammatory process, predominantly confined to the bile ducts within the portal tracts. In most patients the disease gradually progress and may lead to cirrhosis and liver failure. Transplantation may than be the only therapeutic opt…

Cholestatic syndromes.

New insights into the regulation of hepatobiliary transport proteins have provided the basis for a better understanding of the pathogenesis of cholestatic liver diseases. Mutations of transporter genes can cause hereditary cholestatic syndromes, the study of which has shed much light on the basic mechanisms of bile secretion and cholestasis. Important new studies have been published about the pathogenesis, clinical features, and treatment of primary biliary cirrhosis, primary sclerosing cholangitis, cholestasis of pregnancy, total parenteral nutrition-induced cholestasis, and drug-induced chol…

Intrahepatic cholestasis following liver transplantation.

Intrahepatic cholestasis following liver transplantation commonly occurs after liver transplantation and may be caused by infections, drugs such as cyclosporine and sulfonamides, and acute or chronic rejection. Less common causes such as fibrosing cholestatic hepatitis or recurrent primary biliary cirrhosis or primary sclerosing cholangitis may also be encountered. Biliary strictures may also be present. Although some disorders may be managed medically, others often require repeat liver transplantation. Prompt recognition and specific treatment can improve the outcome for liver transplant reci…

The Impact of Inflammatory Bowel Disease in Canada 2018: Extra-intestinal Diseases in IBD.

UNLABELLED: The burden of extra-intestinal disease is high in patients with IBD, some of whom respond to or are prevented by treating the bowel inflammation, whereas others require specific treatment because they are independent of the underlying bowel inflammation. Among the most common extra-intestinal manifestations are other chronic immune-mediated diseases such as erythema nodosum, ankylosing spondylitis and primary sclerosing cholangitis. Patients with IBD are at higher risk of complications in other organ systems such as osteoporosis, venous thromboembolism and cardiovascular disease. I…

Associations Between Quantitative MRI Metrics and Clinical Risk Scores in Children and Young Adults With Autoimmune Liver Disease.

BACKGROUND. The Mayo risk score and SCOPE (Sclerosing Cholangitis Outcomes in Pediatrics) index are clinical risk scores for monitoring the progression of primary sclerosing cholangitis (PSC) and predicting clinically important endpoints. OBJECTIVE. The purpose of this study was to evaluate relationships between quantitative MRI measures of liver disease and clinical risk scores in children and young adults with autoimmune liver disease (AILD). METHODS. This prospective study included 58 patients (35 male and 23 female patients; mean age, 15.1 ± 1.1 [SD] years [range, 6-24 years]) with AILD (1…

Acute Intraoperative Tympanic Membrane Rupture in Patient Anesthetized With Desflurane Without Nitrous Oxide: A Case Report.

We report a case of acute intraoperative tympanic membrane (TM) rupture in a patient anesthetized with desflurane without N2O. The patient was undergoing endoscopic retrograde cholangiopancreatography (ERCP) to treat ascending cholangitis. TM rupture is known to occur with N2O but has not been reported in the literature with the use of inhaled volatile anesthetics without N2O. We suspect that several factors contributed to this complication, including prone positioning, a remote history of ear trauma, and the selection of desflurane as the maintenance anesthetic as opposed to a vapor with a hi…

Successful bridging therapy with tirabrutinib before ASCT for relapsed primary DLBCL of the CNS complicated with PBC, cirrhosis, and pancytopenia.

The optimal therapy for relapsed primary diffuse large B-cell lymphoma (DLBCL) of the central nervous system (CNS) remains unclear. We herein report a case of relapsed primary DLBCL of the CNS complicated with primary biliary cholangitis, cirrhosis, and pancytopenia that was successfully treated with bridging therapy with tirabrutinib before autologous hematopoietic stem cell transplantation (ASCT). Tirabrutinib is well tolerated and effective for relapsed primary DLBCL of the CNS with comorbidities, including cirrhosis and pancytopenia. Tirabrutinib is a promising option as bridging therapy b…

One-year-old Boy with Gallbladder Stone: A Case Report and Literature Review.

Gallstone is a rare disease in children. It might cause complications such as cholecystitis, cholangitis, and gallstone pancreatitis due to obstruction of biliary duct or secondary infection. We report a case of a 1-year, 7-month-old boy with incidental finding of gallbladder stone, who received ceftriaxone treatment after diagnosing with Salmonella enterocolitis. Herein, we review the incidence, etiology, risk factors, and management of ceftriaxone-related gallstones in children. In conclusion, these patients are suggested to receive expectant management with close follow-up instead of surgic…

[Massive biliary hemorrhage due to rupture of pseudoaneurysm after endoscopic biliary plastic stenting for pancreatic cancer].

A 51-year-old woman underwent endoscopic biliary drainage with a plastic stent for obstructive jaundice due to unresectable pancreatic head cancer. During chemotherapy following heavy ion therapy for the cancer, she presented with acute cholangitis and massive rectal bleeding. The massive hemorrhage was caused by rupture of the biliary duct due to a pseudoaneurysm in the right hepatic artery. The position of the aneurysm coincided with the liver-side end of the plastic stent. Sustained irritation of the arterial and bile duct walls by the stent was considered to have contributed to formation o…

[PRINCIPLES OF THE HEPATICO-JEJUNOANASTOMOSIS FORMATION, USING METHOD OF THE SOFT-TISSUES HF-ELECTRIC WELDING IN CLINICAL PRACTICE].

Hepatico-jejunoanastomosis (HJA) was formed in accordance to the high-frequency electric welding method: in 14 patients – for the main bile outflow disorders, in 8 of them – as a consequence of the periampullar zone malignances, in 6 – stricture of a common hepatic duct, HJA earlier formatted, purulent cholangitis, iatrogenic damage of biliary ducts. In all the patients the welded averting one-layered termino-lateral or latero-lateral HJA were formatted. The welded anastomoses have appeared hermetic, sufficiently hard, immediately after the formation and further.

Making Sense of Autoantibodies in Cholestatic Liver Diseases.

Primary biliary cirrhosis (PBC) and primary sclerosing cholangitis (PSC) are the most common chronic cholestatic liver diseases (CLD) in adults and are associated with immune mechanisms. PBC is considered a model autoimmune disease, and more than 90% of patients present very specific autoantibodies against mitochondrial antigens. Whether PSC should be considered an autoimmune or merely immune-mediated disease is still under debate. This review addresses the clinical relevance of autoantibodies in CLD and their pathogenic mechanisms and illustrates the technology available for appropriate autoa…

[Diagnosis of liver diseases by classification of laboratory signal factor pattern findings with the Mahalanobis·Taguchi Adjoint method].

There are many autoimmune liver diseases in which diagnosis is difficult so that overlap is accepted, and this negatively affects treatment. The initial diagnosis is therefore important for later treatment and convalescence. We distinguished autoimmune cholangitis, autoimmune hepatitis and primary biliary cirrhosis by the Mahalanobis·Taguchi Adjoint (MTA) method in the Mahalanobis·Taguchi system and analyzed the pattern of factor effects by the MTA method. As a result, the characteristic factor effect pattern of each disease was classified, enabling the qualitative evaluation of cases includin…

[Acute necrotizing pancreatitis and complete atrioventricular block complicating the course of ascaris infection in an adult patient].

Ascaris lumbricoides, a large round nematode, which causes human ascariasis, is the most prevalent helminth in the world. Ascariasis is usually asymptomatic but can cause serious complications, with a mortality rate of 5%. We report a 55-year-old woman from Comoros who presented with ascariasis complicated by occult cholangitis, severe acute pancreatitis, and transient complete heart-block. Cardiac damage due to migrating ascaris larvae was the likely explanation of the transient heart-block in this patient, although such a complication had never been described previously.
