Oral care in people with dementia living in the community.

Oral health is recognised as an essential component of a person's overall wellbeing, increasingly so in people with dementia. As dementia progresses, a person diagnosed with the condition may become more dependent on the support of the family carer to meet their personal care needs. Similarly, community nurses may be one of the first health professionals that a family carer may call on when difficulties in maintaining good oral care become a concern. An understanding of some of the main concerns and resources available can be helpful for community nurses in helping their patients and their car

Comparing global and community indicators of safety and peace.

UNLABELLED: Indicators measuring the progress of safety and peace have been pursued at a macro (global) and meso level (community). While global indicators focus on the country-level indicators, it does not consider the community-level contextually relevant needs of the indicators. While the global indicators present a general framework to measure safety and peace, these constructs are dynamic and fluid, and thus understood differently within and across countries, provinces and communities. Drawing on data collected through community participatory processes, this study aims to compare existing

Cost to play community-level sport in Australia.

Participation in community club-based sport delivers significant physical, social, and psychological health benefits, yet the cost to play is consistently identified as one of the most common barriers to participation across all age groups. Existing data have largely been limited to registration fees paid to sport organisations, leaving the true financial burden on individuals and families poorly understood. This study investigated the comprehensive costs associated with community sport participation across Australia. An online survey was conducted in collaboration with the Australian Sports C

Caring for Trauma in Underserved Communities: Enhancing Psychotherapy.

Underserved communities-especially communities of color in the United States-face disproportionate exposure to trauma because of social and structural inequalities. These factors give rise to intergenerational transmission of trauma and inform cultural beliefs about psychotherapy and mental health care more broadly. Increasingly, individual psychotherapies developed in diverse settings, as well as community-based psychosocial interventions, show evidence of client engagement, acceptability, and efficacy. The authors describe upstream and immediate factors that increase vulnerability to trauma

Mangrove health shapes lignocellulolytic bacterial communities.

The process of lignocellulosic biofuel production needs enzymes that are resistant to high temperatures and low pH. The mangrove sediments, which are typified by variable conditions, can contain bacteria that synthesize intrinsically steady enzymes. We selected 193 bacterial isolates of 12 mangrove sites in Goa, India and tested them to produce lignocellulolytic enzymes (cellulase, laccase, xylanase, xylose isomerase) under the conditions of neutral (37°C, pH 7), acidic (37°C, pH 5), thermophilic (50°C, pH 7), and combined stress (50°C, pH 5). Bacillus and Vibrio dominated, with 22 genera iden

Groundwater microbial communities across the terrestrial subsurface.

The terrestrial subsurface hosts one of Earth's largest biomes, with groundwater systems representing major habitats for microbial life. In these environments, microbes persist under energy scarcity caused by the absence of photosynthesis, physical isolation and slow geochemical turnover, but the ecological principles underpinning their survival are poorly understood. Here we propose a framework of 'microbial frugality' to describe how evolutionary and physiological traits enable life to persist near the energetic minimum in groundwater. Key traits include low replication rates, genome streaml

Eosinopenia and prognosis in severe community-acquired pneumonia.

BACKGROUND: Eosinopenia has been reported as a marker of severity in infections, but its prognostic value in patients with severe community-acquired pneumonia (CAP) admitted to the intensive care unit (ICU) is unclear. METHODS: We retrospectively analyzed 496 patients with severe CAP admitted to the ICU and stratified them according to the presence of eosinopenia on admission. We compared clinical characteristics, laboratory data, microbiological etiology, treatment, complications, and outcomes between groups. Independent factors associated with 30-day mortality were identified using multivari

Community Characteristics Associated With Health System Penetration.

Health systems have become a dominant form of health care organization in the United States, yet little is known about how their presence varies across communities. This study examined the association between the community-level characteristics and health system penetration, defined as the percentage of general acute care hospital beds affiliated with a health system in a geographic market. We found that communities with higher health system penetration tended to have lower levels of competition. In rural areas, greater insurance coverage was associated with greater penetration. In urban areas

Translation of alcohol research to the academic medicine community.

The goal of alcohol research is to improve overall health and well-being. Alcohol misuse causes an estimated 178,000 deaths annually in the United States, involves over 4 million emergency room visits annually, and contributes to or exacerbates 200 medical disorders and conditions. Reducing and preventing the pathology and other consequences of alcohol misuse can significantly reduce morbidity and mortality. The National Institute on Alcohol Abuse and Alcoholism (NIAAA) was created in 1970 in response to national interest in fundamental research to address these challenges. The mission of NIAA

Prioritizing the Health of Underserved Communities in Prenatal Genetics.

Prenatal cell-free genetic screening (cfDNA) for fetal chromosomal conditions, including aneuploidy and microdeletion syndromes, is increasingly offered in diverse patient populations and clinical settings, including in safety-net clinics and other public health settings. Companies testing for cfDNA have argued for first-tier cfDNA by state Medicaid programs, arguing that differences in coverage of these screens between public and private insurance constitute a health disparity. We contend that this framing is unhelpful considering the ongoing maternal and perinatal health crisis affecting low

Unique microbial communities and phylosymbiosis signals in herpetofauna.

Microbial symbionts are closely related to the internal and external factors of their host. However, the prevalence of phylosymbiosis (the presence of host phylogenetic signal in microbial community composition) remains controversial, especially in animals collectively referred to as herpetofauna. To expand our understanding of host-microbiota interactions, we analysed 11 697 symbiotic microbiota samples from of 337 herpetofaunal species, covering skin, oral cavity, gut, cloaca, feces, and other body sites. The composition of the microbial communities gradually changes along the digestive trac

Health literacy of the Deaf community: a scoping review.

This scoping review examined health literacy among Deaf populations and identified gaps in current evidence. A systemic search of PubMed, Ex … ll, Deaf people demonstrated poorer health literacy levels than hearing populations, largely due to communication barriers and inaccessible health information. While educational interventions improved knowledge, evidence on their sustained impact remains limited. Findings underscore the need for culturally-competent, sign-language-accessible health education to …

Disseminated Blastomycosis Misdiagnosed as Community-Acquired Pneumonia.

Blastomycosis, caused by Blastomyces species, is a fungal infection that can manifest with a wide range of clinical presentations, from asymptomatic cases to severe, multi-organ involvement. Here, we describe a 33-year-old Black woman from an endemic region who experienced progressive, antibiotic-refractory respiratory symptoms and developed a cutaneous lesion, initially misdiagnosed as community-acquired pneumonia. This case highlights the need to include blastomycosis in the differential diagnosis of persistent pneumonia, particularly in at-risk populations, to facilitate timely diagnosis an

Frequency of viral etiology in community-acquired pneumonia.

The identification of etiology is very important when managing patients with community-acquired pneumonia (CAP). In Pakistan, studies regarding the viral etiology in CAP are scarce. The main objective of this study was to evaluate the frequency of viral etiology in CAP patients and analyze the clinical features and their impact on prognosis. Medical records of CAP patients admitted to Aga Khan University Hospital (Karachi, Pakistan) from March 2022 to February 2023 were retrospectively reviewed; patients who had microbiological tests performed within 48 hours of hospital admission were include

Lung ultrasonography for community-acquired pneumonia diagnosis.

Over the past two decades, several studies have assessed the performance of lung ultrasonography in the diagnosis of community-acquired pneumonia (CAP). The performance of this diagnostic tool, when used by clinicians other than radiologists, is excellent, with an area under the curve of 0.97, 92% sensitivity, and 93% specificity according to a recent meta-analysis including 5,108 patients. When ultrasonography is compared with chest CT-scan, sensitivity ranged from 68% to 98% and specificity from 61% to 98%. Compared with chest x-ray, ultrasonography seems more sensitive and specific.
