Treatment of cerebellar infarction by decompressive suboccipital craniectomy.

BACKGROUND AND PURPOSE: We present an anecdotal series of 11 patients without past history of stroke with progressive neurological deterioration while on medical therapy for large cerebellar infarctions. Clinical signs of brain stem compression developed in these patients. Computerized tomography of the head confirmed mass effect from brain edema. It was the clinical judgment of the neurologists and neurosurgeons that each of these 11 patients would expire without surgical intervention. METHODS: All 11 patients (seven men, four women; mean age, 54 years) were treated with suboccipital craniec

Anterior Endoscopic Cervical Discectomy: Surgical Technique and Literature Review.

The conventional surgical technique for radiculopathy with cervical disc herniation (CDH) is anterior cervical discectomy and fusion, with a good clinical outcome and fusion rate. However, significant perioperative morbidity related to extensive surgical exposure has been reported. Therefore, anterior endoscopic cervical discectomy (AECD) using a working channel endoscope has been developed to reduce surgical complications and tissue damage. The objective of this study was to describe a cutting-edge technique for AECD of soft CDH. The primary indication is cervical radiculopathy with or withou

Preoperative transarterial embolization of spinal column neoplasms.

PURPOSE: To determine the safety and value of vertebral column embolization before surgical resection of vascular neoplastic disease. PATIENTS AND METHODS: Thirty preoperative embolization procedures were performed in 20 patients with vascular neoplasms of the vertebral column (C-2 to sacrum). Fourteen patients had metastatic renal cell carcinoma. Distal embolic agents were used in 27 cases and were coupled with more proximal agents in six. Gelatin pledgets alone were used in three cases. Twenty-six of the 27 surgical procedures involved partial to complete tumor resection. RESULTS: Seventy-

Five-year clinical outcomes with endoscopic transforaminal foraminoplasty for symptomatic degenerative conditions of the lumbar spine: a comparative study of inside-out versus outside-in techniques.

BACKGROUND: Foraminal stenosis is a condition that is underappreciated by traditionally trained surgeons because the entire foraminal zone is not adequately visualized with the translaminar approach unless extensive removal of the facet is performed to expose the extraforaminal zone. Its direct endoscopic visualization is feasible with the inside-out and outside-in endoscopic transforaminal technique. The authors analyzed the differences in long-term 5-year clinical outcomes of endoscopic transforaminal foraminoplasty for symptoms from lumbar foraminal stenosis to better establish clinical ind

Thoracic range of motion, stability, and correlation to imaging-determined degeneration.

OBJECT The degenerative process of the spinal column results in instability followed by a progressive loss of segmental motion. Segmental degeneration is associated with intervertebral disc and facet changes, which can be quantified. Correlating this degeneration with clinical segmental motion has not been investigated in the thoracic spine. The authors sought to determine if imaging-determined degeneration would correlate with native range of motion (ROM) or the change in ROM after decompressive procedures, potentially guiding clinical decision making in the setting of spine trauma or followi

Biportal endoscopic lumbosacral foraminotomy in dogs: technical feasibility and anatomical assessment in cadavers.

INTRODUCTION: Lumbosacral foraminal stenosis (LSFS) in dogs, a subtype of degenerative lumbosacral disease, leads to nerve root compression and clinical signs such as pain or pelvic limb dysfunction. Traditional decompression techniques-such as dorsal laminectomy or lateral foraminotomy-have limitations, including restricted access, tissue disruption, and prolonged recovery. To address these challenges, this study evaluated the technical feasibility and safety of biportal endoscopic lumbosacral foraminotomy (BELF), a minimally invasive technique adapted from human unilateral biportal endoscopy

Intractable seizures after cranioplasty, a dreadful post-operative complication managed efficiently from Nepal: A case series.

INTRODUCTION AND IMPORTANCE: Cranioplasty is an elective neurosurgical procedure following decompressive craniectomy, often associated with post-operative complications such as hemorrhage, seizures, infection, hydrocephalus, and bone resorption. While seizures post-cranioplasty is not uncommon, Intractable seizures are rare but a dreadful complication following cranioplasty. CASE SUMMARY: A 23 and 17-year-old male underwent decompression craniectomy for traumatic brain injury and subsequently underwent titanium mesh and acrylic cranioplasty respectively, During the post-operative period both

Percutaneous transforaminal endoscopic decompression with removal of the posterosuperior region underneath the slipping vertebral body for lumbar spinal stenosis with degenerative lumbar spondylolisthesis: a retrospective study.

BACKGROUND: Percutaneous transforaminal endoscopic decompression (PTED) is an ideal minimally invasive decompression technique for the treatment of lumbar spinal stenosis (LSS) with degenerative lumbar spondylolisthesis (DLS). The posterosuperior region underneath the slipping vertebral body (PRSVB) formed by DLS is an important factor exacerbating LSS in patients. Therefore, the necessity of removing the PRSVB during ventral decompression remains to be discussed. This study aimed to describe the procedure of PTED combined with the removal of the PRSVB and to evaluate the clinical outcomes. M

Volar Lunate Dislocation Causing Secondary Carpal Tunnel Syndrome: A Case Report.

Background  Failure of carpal tunnel release is an uncommon occurrence with unique pathologies that may impede proper diagnosis and treatment. Symptoms are most often attributed to an inadequate release of the transverse carpal ligament or pathologic scar tissue resultant of the primary decompression. Case Description  In this report, we describe the case of a 79-year-old male with a history of scaphoid lunate advanced collapse and a prior carpal tunnel decompression presenting with worsening right wrist function and new right palmar mass. The patient had no significant antecedent trauma, and

Basal Cisternostomy - A Microsurgical Cerebro Spinal Fluid Let Out Procedure and Treatment Option in the Management of Traumatic Brain Injury. Analysis of 40 Consecutive Head Injury Patients Operated with and without Bone Flap Replacement Following Cisternostomy in a Tertiary Care Centre in India.

BACKGROUND: Decompressive craniectomy (DC) is considered the gold standard blanket surgical procedure for all medically intractable cerebral oedema in Traumatic Brain Injury (TBI). It's only proven fact is that it reduces Intra Cerebral Pressure (ICP) by providing space for the oedematous brain. Attempts are being made to find additional or alternative procedures to improve outcomes in TBI. Basal Cisternostomy is one such technique proposed to bring such a change in world literature. AIM: To analyse the validity of Basal Cisternostomy in TBI patients. MATERIALS AND METHODS: A total of 40 pat

Biomedical Glue Sling Technique in Microvascular Decompression for Trigeminal Neuralgia Caused by Atherosclerotic Vertebrobasilar Artery: A Description of Operative Technique and Clinical Outcomes.

BACKGROUND: Microvascular decompression (MVD) is the most definitive surgical treatment for trigeminal neuralgia (TN). In the case in which an atherosclerotic vertebrobasilar artery (aVBA) offends the trigeminal nerve, the postoperative outcomes have been reported to be less satisfactory in terms of symptom recurrence and complications. In this study, the authors present their experience using a biomedical sling for MVD in patients with aVBA-associated TN. METHODS: A retrospective study of 22 consecutive patients who underwent the biomedical glue sling technique in MVD for TN with aVBA was co

Perils and pitfalls for clinicians embarking on qualitative research in physiotherapy.

AIM: To consider the pros and cons of focus groups versus interviews for studies interested in examining patient experiences of clinical interventions. The paper looks at the hazards of being a clinician when collecting qualitative data and shares these experiences to provide useful learning for other clinicians embarking on qualitative approaches. BACKGROUND: Sub-acromial decompression surgery (SAD) is the accepted, surgical intervention for shoulder impingement syndrome. Evidence suggests that outcomes from SAD are no more superior to conservative management. This raises questions as to whe

Intracanal pressure in compressive spinal cord injury: reduction with hypothermia.

Most cases of human spinal cord injury (SCI) are accompanied by continuing cord compression. Experimentally, compression results in rapid neurological decline over hours, suggesting a rise in intracanal pressure local to the site of injury. The aim of this study was to measure the rise in local intracanal pressure accompanying progressive canal occlusion and to determine the relationship between raised intracanal pressure and neurological outcome. We also aimed to establish whether hypothermia was able to reduce raised intracanal pressure. We demonstrate that, following SCI in F344 rats, local

Degenerative spondylolisthesis with an intact neural arch: a review of 60 cases with an analysis of clinical findings and the development of surgical management.

The treatment over the past 12 years of 60 patients with degenerative spondylolisthesis with an intact neural arch is reviewed. The patients averaged 65 years of age, with women outnumbering men by a ratio of 2:1. Symptoms in the lower extremities had been present for 3 months to 10 years, although varying back pain had existed for longer periods. Two-thirds showed signs of motor dysfunction. Sensory alterations and a positive Las ègue's sign could be demonstrated in only one-half of the patients studied. Four of 5 patients developed intermittent neurogenic claudication, with varying evidence

A comparative study of the treatment of cervical spondylotic myeloradiculopathy. Experience with 50 cases treated by means of extensive laminectomy, foraminotomy, and excision of osteophytes during the past 10 years.

This paper reviews management by means of the posterior approach of 50 patients with cervical myeloradiculopathy caused by spondylosis and stenosis of the spinal canal seen in the past 10 years. Careful selection of patients is an absolute necessity since a primary cause of failure occurred in individuals who subsequently proved to have motor neurone disease. Older individuals with long-standing neurological deficits, especially long tract signs indicative of fixed lesions, were benefited primarily by a lack of further progression of their disorder and occasional improvement in hand function a
