Olivocochlear efferent vs. middle-ear contributions to the alteration of otoacoustic emissions by contralateral noise.

The medial olivocochlear efferent bundle is the key element of a bilateral efferent reflex activated by sound in either ear and acting directly on cochlear outer hair cells (OHC) via numerous cholinergic synapses. It probably contributes to regulating the mechanical activity of the cochlea. Otoacoustic emissions, being sounds emitted by the cochlea as a reflection of its activity and suppressed by efferent activation, are increasingly considered to be the privileged tool for a noninvasive assessment of the efferent reflex. However, confounding effects on otoacoustic emissions can occur. A prim…

Relating middle-ear acoustic performance to body size in the cat family: measurements and models.

Is the acoustic performance of the mammalian middle ear dependent on body size? We focus on the cat family, because of its qualitatively uniform (and distinctive) middle-ear structure, large size range, and the extensive data available from domestic cats which provide a framework for relating middle-ear acoustics to structure. We report measurements of acoustic admittance in 17 live adult ears of 11 exotic species, ranging in size from sand cat (3 kg) to tiger (180 kg). For low frequencies, the middle-ear response is compliant for all species and generally increases with size. The compliance o…

MRI screening of the cerebellopontine angle and inner ear with fast spin-echo T2 technique.

In patients with unilateral hearing loss and dizziness it is important to rule out a cerebellopontine angle process. This is often done by audiological and otoneurological investigations. However, in many cases we must rely on the imaging of the temporal bone and the cerebello-brainstem area. The paper has presented the three dimensional (3D) Fast Spin-Echo (FSE) T2 weighted, 0.7 mm thick MR images, which in addition to being quick, does not require the use of expensive contrast material. Between September 1996 and November 1997, 152 patients with unilateral hearing loss and/or balance disorde…

Does ultrasound influence experimentally induced thrombus formation in the central artery of the rabbit ear?

BACKGROUND: Thrombosis is one of the most important causes of morbidity in the medical field. Several independent in vitro studies have shown that the fibrinolytic process may be enhanced by ultrasound, but the effect of ultrasound on thrombus formation in vivo is unexplored. The present study was designed to investigate this matter. METHODS: In a blind randomized study, standardized arteriotomies and intimectomies were performed on the central arteries of the ears of 25 rabbits. The rabbits were allocated to two groups, an untreated control group and a group treated with ultrasound (10 pulse…

Kinin and histamine stimulate Cl- secretion in gerbil middle ear epithelium: connection to otitis media.

The effects of bradykinin (BK) and histamine on transepithelial ion transport in primary cultures of gerbil middle ear epithelium were investigated. Lysyl-bradykinin (lys-BK) elicited a transient increase in short-circuit current (I(sc)) when added to apical or basolateral surfaces. Lys-BK had a larger effect than BK or des-arg9-BK on both epithelial surfaces. Histamine induced a transient increase in I(sc) only when added to the basolateral surface. Mepyramine, an H1 histamine antagonist, greatly reduced the histamine-induced I(sc). The H2 and H3 histamine antagonists were both ineffective fo…

Select types of supporting cell in the inner ear express aquaporin-4 water channel protein.

Aquaporins (AQPs) confer a high water permeability on cell membranes and play important parts in secretory and absorptive epithelia in kidney and other organs. Here we investigate whether AQPs are expressed in the sensory epithelia of the inner ear, where a precise volume regulation is crucial. By use of specific antibodies it was found that the inner ear contains AQP1 and 4 while being devoid of detectable levels of AQP2, 3 or 5. Immunofluorescence and postembedding immunogold labelling revealed a strictly non-epithelial distribution of AQP1, confirming previous data. In contrast, AQP4 protei…

A new form of complicated hereditary spastic paraplegia with cataracts, atretic ear canals and hypopigmentation.

A 16-year-old Hispanic boy born of consanguineous parents is described as having a history of cataracts, progressive lower-extremity spasticity and atrophy starting at 4 years of age, atretic ear canals with hearing dysfunction and diffuse patchy cutaneous hypopigmented areas. Clinical examination showed the typical signs of spastic paraplegia with increased tone, hyperreflexia, muscle atrophy and contractures. Sensation, autonomic and cerebellar functions were not disturbed. Neuroimaging studies were normal. Laboratory findings did not support a diagnosis of metabolic disturbance or infectiou…

Magnetic resonance imaging of the inner ear in patients with idiopathic sudden sensorineural hearing loss.

Although gadolinium-enhanced magnetic resonance imaging (Gd-MRI) has been used to indicate the presence of a subclinical labyrinthitis in patients with idiopathic sudden sensorineural hearing loss (ISSHL), its sensitivity in daily clinical practice is unknown. We describe Gd-MRI findings in 27 ISSHL patients taking part in a prospective multicenter clinical trial. MRI findings were related to the severity of the hearing loss, vestibular involvement and the time interval between the occurrence of ISSHL and imaging. Pathological enhancement of the cochlea indicating a labyrinthitis was found in …

Two modes of auditory hair cell loss following acoustic overstimulation in the avian inner ear.

To determine the type of cell death occurring and how the removal of damaged cells proceeds following overstimulation, we examined chick basilar papillae using an in situ DNA nick end labeling method and transmission electron microscopy. Two distinct modes of hair cell loss were identified. First, hair cells which had not progressed into typical cell death processes, apoptosis or necrosis, were deleted by extrusion from the epithelium just after sound exposure. Second, hair cells manifested degeneration through the process of apoptosis, then further deterioration within the epithelium after th

Acoustic effect of malleus head removal and tensor tympani muscle section on middle ear reconstruction.

In the reconstruction of ears with a missing incus, an incus replacement prosthesis (IRP) is commonly used to connect malleus and stapes. In some cases, it is necessary to resect the malleus head and/or section the tensor tympani muscle (TTM) tendon. The acoustic effects of these maneuvers have not been well studied. We performed experiments in a temporal bone model to measure the effect of these maneuvers on middle ear sound transmission. Measurements of umbo and stapes displacement were made before and after malleus head removal and TTM section plus incus replacement with an IRP. After malle…

Electrophysiological effects of multiple instillation of Haemophilus influenzae type b endotoxin on the inner ear.

An analysis of auditory brainstem response (ABR) thresholds and ABR-based frequency tuning curves was performed in 15 Sprague-Dawley rats exposed to Haemophilus influenzae type b endotoxin; 5 microg/50 microl toxin was instilled every second day, altogether five times, into the middle ear cavity through a small perforation in the tympanic membrane. ABR was measured 48 h after the second application and 24 h, 48 h, 5 days and 10 days after the fifth instillation. Five applications of toxin had no statistical effect on ABR thresholds and no changes in TC configuration were observed. It is conclu…

The influence of unilateral vertebral artery occlusion on bilateral inner ear blood flow in rats.

We investigated the influence of unilateral vertebral artery (VA) occlusion on bilateral inner ear blood flow in rats using laser Doppler flowmetry for the measurement of bilateral cochlear blood flow (CBF) and photochemically initiated thrombosis for VA occlusion. BP and CBF showed little change by unilateral VA occlusion alone. However, subsequent hypotension induced by venesection not only reduced CBF but also caused imbalance between bilateral CBF reduction rates. CBF changed independently of BP, indicating the existence of a local CBF regulatory system.

Case report: the first report of idiopathic hypereosinophilic syndrome involved with lung and middle ear.

It has been reported that various organs are involved in idiopathic hypereosinophilic syndrome. Frequently, the heart, lung, skin, and nervous system are involved. Involvement of the middle ear, however, has not yet been reported. In this article, the authors describe the first case of hypereosinophilic syndrome involving the lung and middle ear. A 39-year-old woman had a 4-month history of low grade fever, non-productive cough, and a feeling of fullness and hearing loss in both ears. Peripheral blood cell count showed eosinophilia. Bilateral tympanic cavities were obstructed with granulation …

[Deafness caused by malformation of the inner ear. Current contribution of x-ray computed tomography].

Hearing loss due to malformations of the auditory system may involve morphologic abnormalities of the external canal, the middle ear, or the inner ear. Various combined malformations are possible. Audiologic assessment and computed tomography make it possible to perform a precise diagnostic evaluation of inner ear malformations. In a series of 71 children with congenital sensorineural hearing loss, 31 had computed tomographic evidence of malformations. Since 19 children had bilateral lesions, a total of 50 ears were studied. The cochleovestibular abnormalities were typed and correlated to the …

Tympanic bulla osteotomy for treatment of middle-ear disease in cats: 19 cases (1984-1991).

Twenty-two tympanic bulla osteotomies were performed in 19 cats for the treatment of bacterial otitis media (n = 11), inflammatory polyps (n = 7), or middle-ear neoplasia (n = 4). Inflammatory polyps mostly affected young adult cats (mean age, 1.5 years), whereas otitis media affected middle-aged cats (mean age, 5.5 years) and neoplasia affected older cats (mean age, 10.25 years). Although not specific for the underlying disease process, fluid density within the tympanic bulla or thickening of the bulla was observed radiographically in 100% of cats with middle-ear disease. In cats for which re…
