A 1 yr old female spayed dog presented for evaluation of lethargy, hyporexia, and left hind limb lameness. On presentation, she was tachycardic and febrile. Echocardiogram revealed pericardial effusion and evidence of pericarditis and myocarditis, and electrocardiogram showed underlying sinus rhythm with complex arrhythmias. Cardiac troponin-I level was markedly elevated. The dog was hospitalized for 6 days on IV fluid therapy and treated with ampicillin/sulbactam and enrofloxacin. She was discharged on amoxicillin-clavulanic acid and maropitant citrate. Within 24 hr, the dog returned because

A 75-year-old man with a fever, shoulder pain, and lower leg edema was diagnosed with polymyalgia rheumatica and started on glucocorticoid therapy. Eighteen months later, he was admitted with rapidly progressive renal failure. Glucocorticoid therapy had been discontinued one month prior to admission. Serum anti-glomerular basement membrane antibodies were elevated, and a kidney biopsy revealed fibrinoid necrosis of the medium-sized renal arteries, tubulointerstitial nephritis, and collapsed glomeruli. An immunofluorescence study showed mild immunoglobulin G linear deposition. Polyarteritis nod

BACKGROUND: Primary intraosseous carcinoma(IOC)is a rare malignancy originating from the epithelial lining of odontogenic cysts. We present a case of IOC arising from an odontogenic cyst in the mandible.
CASE: A 51-year-old man presented to a general hospital with limited mouth opening. Initial anti-inflammatory treatment was administered for a presumed secondary infected mandibular odontogenic cyst;however, symptoms persisted. Subsequent biopsy revealed squamous cell carcinoma, prompting referred to our department for multidisciplinary management. Various examinations were performed, and the

BACKGROUND Vulvar melanoma during pregnancy is exceptionally rare. Hormonal and immunological changes in pregnancy have raised concerns about the potential for accelerated melanoma progression and poorer maternal outcomes. This case report describes an unusual presentation of vulvar melanoma in a pregnant patient, which rapidly progressed despite previous treatments, but resulted in a favorable fetal outcome. CASE REPORT A 40-year-old G3P2A0 woman at 28 weeks of gestation, with a history of vulvar malignant melanoma diagnosed 3 years prior, presented with sudden abdominal pain and hematuria. S

Leptomeningeal spread (LMS) of glioblastoma multiforme (GBM) is a rare event, presenting challenges in both diagnosis and treatment. Here, we describe the case of a 59-year-old GBM patient who experienced LMS during his treatment. Despite multimodal therapy, including surgery, chemotherapy, and radiotherapy, the patient's condition rapidly deteriorated. The case illustrates the difficulty in treating GBM and the restricted ability of available therapies to stop LMS. This manifestation is uncommon, which highlights the need for increased awareness, ongoing study, and development of tailored tre

Calciphylaxis is a rare, life-threatening condition in end-stage renal disease, often presenting with skin ulcers and potentially involving the internal organs. A 62-year-old man undergoing hemodialysis developed non-healing leg ulcers that were initially treated for cellulitis. The diagnosis was confirmed through a biopsy after ulceration and necrosis progressed. Despite multidisciplinary treatment, the patient died because of sepsis and intestinal perforation. Autopsy revealed extensive visceral calciphylaxis, highlighting the under-recognized systemic nature of the disease. This case emphas

We report a rare case of pulmonary tumor thrombotic microangiopathy (PTTM) in a patient with metastatic neurotrophic tropomyosin receptor kinase (NTRK) fusion-positive transverse colon cancer who exhibited a marked radiologic and biochemical response to entrectinib. Despite significant tumor shrinkage, progressive dyspnea and hypoxemia developed approximately four weeks after therapy initiation. Chest CT revealed diffuse interstitial infiltrates, initially interpreted as drug-induced pneumonitis or infection. Entrectinib was discontinued, but respiratory failure progressed, and the patient die

BACKGROUND: Sarcina ventriculi is a bacterium predominantly reported in the stomach and associated with emphysematous gastritis, delayed gastric emptying, gastroparesis, or gastric outlet obstruction. Its prevalence is increasing among patients with a history of organ transplants, immunosuppression, and graft-versus-host disease (GVHD). This bacterium can be detected on histology with characteristic tetrad packet morphology; however, confirmation requires PCR and molecular studies. The role of Sarcina ventriculi in human diseases is not fully understood and has unclear clinical significance. W

Pathogenic variants in DNM1L, encoding dynamin-like protein-1 (DRP1), cause a lethal encephalopathy. DRP1 defective function results in altered mitochondrial networks, characterized by elongated/spaghetti-like, highly interconnected mitochondria. We validated in yeast the pathogenicity of a de novo DNM1L variant identified by whole exome sequencing performed more than 10 years after the patient's death. Meanwhile, we reviewed the broadness and specificities of DNM1L-related phenotype. The patient, who exhibited developmental delay in her third year, developed a therapy-refractory myoclonic sta

An 11-year-old, Damaraland mole-rat queen died after several months of abdominal distension with no other clinical signs, and a necropsy was performed. Grossly, numerous red-white, smooth masses, ranging from a few millimeters to 1 cm in diameter, were widely scattered across the surfaces of the diaphragm, mesentery, and peritoneum. The pulmonary hilar, mediastinal, mesenteric, and renal lymph nodes were enlarged. Histologically, the masses and the enlarged lymph nodes consisted of atypical cells with abundant eosinophilic cytoplasm and large, eccentrically positioned nuclei. Multinucleated gi

BACKGROUND: Lysozyme-induced nephropathy (LyN) is a rare cause of acute kidney injury (AKI) in patients with chronic myelomonocytic leukemia (CMML). Diagnosing LyN is challenging due to the numerous potential and concurrent aetiologies. Previous case reports have demonstrated the diagnostic utility of serum and urinary lysozyme measurements, which may correlate with renal dysfunction; however, the role of renal imaging remains unclear and renal biopsy remains essential for diagnosis.

Some cases of obesity are thought to be associated with hypo-leptinemia. This may cause decreased appetite suppression resulting in increased appetite, leading to weight gain. Replacement therapy with leptin might be theoretically useful, but verification by reporting more cases is required. Here, we first investigated the serum leptin levels and their correlation with body mass index (BMI) in 107 patients with obesity to identify the subjects with hypo-leptinemia. Among them, one patient with congenital hypopituitarism was further investigated by comparison of his clinical and pathological ch

This case report describes a male in his early 40s presenting with severe ethylene glycol (EG) poisoning from antifreeze ingestion, marked by profound metabolic acidosis, hyperkalaemia and multi-organ failure. The patient was found unconscious in the garage with shallow breathing and hypothermia, progressing to critical illness despite aggressive resuscitative measures. Diagnostic evaluations revealed high anion gap metabolic acidosis, elevated lactate, hyperkalaemia and a high osmolal gap, confirmed to be due to EG toxicity. Immediate interventions included continuous renal replacement therap

Acute necrotising encephalopathy (ANE) of childhood is a rare, rapidly progressive and often fatal form of encephalopathy characterised by distinctive clinical and neuroradiological features. It typically presents with a sudden and progressive alteration of consciousness, along with the characteristic finding of multifocal, symmetric lesions involving the thalami, brainstem, cerebellum and cerebral white matter. Although its exact pathogenesis remains unclear, it is thought to result from a cytokine storm triggered by viral infections, leading to blood-brain barrier disruption and brain injury

Gastrointestinal parasites have been reported in wild Sunda pangolins (Manis javanica), but the clinical significance of such detections is often unknown. This report describes pathological lesions in the stomachs of two dead wild Sunda pangolins caused by Gendrespirura sp.. Gross and histopathological examination showed gastritis and hyperkeratosis in infected animals. Morphological identification confirmed the presence of Gendrespirura sp., and molecular analysis revealed a close phylogenetic relationship with the genus Gendrespirura, supporting placement within the family Habronematidae. BACKGROUND: Naegleria fowleri, a pathogenic free-living amoeba, causes primary amoebic meningoencephalitis (PAM), a rare but devastating disease with acute onset, rapid progression, and > 95% mortality. Despite its rarity, the catastrophic outcomes associated with this infection underscore the critical importance of prevention. In this report, we present a rare pediatric fatality caused by PAM in China, highlighting the challenges of diagnosis and treatment.

Patients with dermatomyositis (DM) are particularly susceptible to opportunistic infections due to immunosuppression induced by the disease itself and its treatment. We describe three patients who met the diagnostic criteria for DM and developed tuberculous myositis. The first case, a 54-year-old woman, had a positive polymerase chain reaction (PCR) for Mycobacterium tuberculosis detected in a post-mortem muscle biopsy. A second patient was diagnosed with a positive Ziehl-Neelsen stain in bronchoalveolar lavage, and a third patient, with multiple collections in the thorax and lower limbs, had

BACKGROUND: Johne's disease (JD), paratuberculosis, is a chronic and debilitating intestinal disease affecting ruminants, caused by Mycobacterium avium subspecies paratuberculosis (MAP). Although JD has been extensively studied in domestic livestock, its prevalence and impact on wildlife in Iran, particularly among wild sheep, remain underexplored.
CASE DESCRIPTION: This study presents the first documented cases of JD in two free-ranging, 3-year-old Isfahan wild sheep (Ovis orientalis isphahanica), which exhibited weight loss, hyperthermia and neurological symptoms, including paddling movemen

Mpox, a zoonotic disease caused by the monkeypox virus (MPXV), has reemerged as a significant global public health threat, particularly in sub-Saharan Africa, where more than 50,000 cases have been reported since 2022. Although HIV-related immunosuppression is a known risk factor for severe MPXV infection, the intersection of mpox, HIV, and critical illness has been largely overlooked in ongoing outbreaks in East and Central Africa. We describe a case of a 45-year-old woman in Uganda, living with WHO stage 3 HIV and active pulmonary tuberculosis (TB), who developed mpox-associated sepsis and a
