Impact of practice patterns in shunt use during carotid endarterectomy with contralateral carotid occlusion.

PURPOSE: This study investigated the association between surgeon practice pattern in shunt placement and 30-day stroke/death in patients undergoing carotid endarterectomy (CEA) with contralateral carotid occlusion (CCO). METHODS: Among 6379 CEAs performed in the Vascular Study Group of New England (VSGNE) between 2002 and 2009, we identified 353 patients who underwent CEA with CCO and compared the 30-day stroke/death rate with 5279 patients who underwent primary, isolated CEA with a patent contralateral carotid artery. Within patients with CCO, we examined the 30-day stroke/death rate across

Effect of a Skin Self-monitoring Smartphone Application on Time to Physician Consultation Among Patients With Possible Melanoma: A Phase 2 Randomized Clinical Trial.

IMPORTANCE: Melanoma is among the most lethal skin cancers; it has become the fifth most common cancer in the United Kingdom, and incidence rates are rising. Population approaches to reducing incidence have focused on mass media campaigns to promote earlier presentation and potentially improve melanoma outcomes; however, interventions using smartphone applications targeting those with the greatest risk could promote earlier presentation to health care professionals for individuals with new or changing skin lesions. OBJECTIVE: To study the effect of a commercially available skin self-monitorin

Compliance patterns in adopting sustainability practices: A cluster analysis of oil palm producers in Colombia.

This study analyzes the adoption patterns of sustainability practices among oil palm producers in Colombia using cluster analysis of the Sustainability Index (SI). Employing advanced data mining algorithms, including K-means and Ward's method, producers were grouped according to their compliance with sustainable practices at national and regional levels. The analysis revealed ten distinct producer typologies, ranging from "Advanced adopters" to "Lagging adopters" in economic, environmental, and social sustainability dimensions. Key factors influencing cluster formation included palm-growing ar

Virtual mindfulness-based wellness coaching to support mental health during the COVID-19 pandemic: Thematic analysis of participants' experiences.

The COVID-19 pandemic and associated disruptions adversely impacted physical and mental health. The shift to remote delivery of medical and psychological services provided opportunities to disseminate effective community interventions, unimpeded by needs for local in-person meetings. This study focuses on the responses of older adults to the virtual community Mindfulness-based Wellness Coaching (MBWC) intervention that was delivered by the YMCA of Greater Toronto. We focus on the thematic analysis of semi-structured interviews of 23 participants who were regular attendees. Themes identified pe

Ethical Issues and Recommendations in Psychedelic Research and Practice: A Scoping Review.

The rapid growth in psychedelic research raises novel ethical challenges for both research and psychedelic-assisted therapy. Despite these challenges, there is no consensus among researchers, clinicians, patients, and regulators on how these ethical issues may be avoided or managed. This study aimed to identify key ethical issues in psychedelic research and practice in the literature. A scoping review was performed, identifying fifty-one relevant articles. Content analysis revealed five main ethical concerns in the literature: 1) standards of practice, 2) equity, 3) integrity, 4) cultural appr

Enteral approaches in malabsorption.

Enteral autonomy and freedom from parenteral nutrition dependency is the ultimate therapeutic goal in children with intestinal failure. This can be achieved following attainment of bowel adaptation in conditions such as short bowel syndrome. Enteral nutrition is a major therapeutic cornerstone in the management of children with intestinal failure. It promotes physiological development, bowel adaptation and enhances weaning from parenteral nutrition. The optimal method of delivery, type of nutrients, timing of initiation, promotion of feeds and transition to solid food in children with short bo

Motor learning of a bimanual task in children with unilateral cerebral palsy.

Children with unilateral cerebral palsy (CP) have been shown to improve their motor performance with sufficient practice. However, little is known about how they learn goal-oriented tasks. In the current study, 21 children with unilateral CP (age 4-10 years old) and 21 age-matched typically developed children (TDC) practiced a simple bimanual speed stack task over 15 days of practice. Both groups demonstrated their ability to learn the current bimanual task, but their rate of improvement and learning pattern differed. Children with unilateral CP overall were slower and improved ~10% less than

Practical recommendations for the use of ACE inhibitors, beta-blockers, aldosterone antagonists and angiotensin receptor blockers in heart failure: putting guidelines into practice.

Surveys of prescribing patterns in both hospitals and primary care have usually shown delays in translating the evidence from clinical trials of pharmacological agents into clinical practice, thereby denying patients with heart failure (HF) the benefits of drug treatments proven to improve well-being and prolong life. This may be due to unfamiliarity with the evidence-base for these therapies, the clinical guidelines recommending the use of these treatments or both, as well as concerns regarding adverse events. ACE inhibitors have long been the cornerstone of therapy for systolic HF irrespecti

AIDS education practices among Massachusetts physicians.

Physicians have a major role to play in the prevention of AIDS transmission. Promotion of health behaviors needs to be expanded beyond the traditional AIDS high risk populations; however, little information is available on the AIDS education practices of physicians. A survey of Massachusetts physicians in four specialty groups, Family Medicine Practitioners, Internists, Obstetricians/Gynecologists and Pediatricians, was conducted to determine the extent of their AIDS education practices. Three hundred and ninety one physicians returned the questionnaire for an overall response rate of 66%. Six

Histological approach and differentiation of Crohn's disease and gastrointestinal tuberculosis: Recommendations from the joint IAPM-ISG-CCFI Working Group.

BACKGROUND: The most common causes for ulcero-stricturing diseases of the ileo-cecal region and colon in Southeast Asia are Crohn's disease (CD) and gastrointestinal tuberculosis (GI TB). Diagnosing these conditions is challenging because they share several clinical, endoscopic, radiological and histological features on mucosal biopsies. Therefore, there is a need to standardize the sampling, processing and interpretation of mucosal biopsies to aid clinical decision-making. METHODS: Recognizing this challenge, core subject experts nominated by the Indian Association of Pathologists and Microb

Adolescent substance abuse treatment clinicians' self-help meeting referral practices and adolescent attendance rates.

Clinicians in adolescent substance abuse treatment programs often recommend attendance at 12-Step meetings; however, there has been no systematic study of their referral practices or possible influence on attendance rates. Quantitative and qualitative data were used to examine: (a) the self-help referral practices of clinicians employed in adolescent substance abuse treatment programs; and (b) the potential relationship between practices and self-help attendance. Data were analyzed from open-ended interviews with 28 clinicians at eight CSAT-funded SCY sites and from follow-up interviews with o
