Development, feasibility and potential effectiveness of community-based continuous mass dog vaccination delivery strategies: Lessons for optimization and replication.

OBJECTIVES: Dog vaccination can eliminate rabies in dogs, but annual delivery strategies do not sustain vaccination coverage between campaigns. We describe the development of a community-based continuous mass dog vaccination (CBC-MDV) approach designed to improve and maintain vaccination coverage in Tanzania and examine the feasibility of delivering this approach as well as lessons for its optimization. METHODS: We developed three delivery strategies of CBC-MDV and tested them against the current annual vaccination strategy following the UK Medical Research Council's guidance: i) developing a

Development and Use of Gene Therapy Orphan Drugs-Ethical Needs for a Broader Cooperation Between the Pharmaceutical Industry and Society.

Gene therapy orphan medicinal products constitute a unique group of new drugs which in case of hereditary diseases are usually administered only once at an early age, in the hope to provide sufficient gene product to last for the entire life of the patients. The combination of an exceptionally large single payment and the life-long clinical follow-up needed for understanding the long-term benefits and safety of gene therapy, represent new types of scientific, financial, social and ethical challenges for the pharmaceutical industry, regulators and society. With special consideration of the uniq

Frailty for Perioperative Clinicians: A Narrative Review.

Frailty is a multidimensional syndrome characterized by decreased reserve and diminished resistance to stressors. People with frailty are vulnerable to stressors, and exposure to the stress of surgery is associated with increased risk of adverse outcomes and higher levels of resource use. As Western populations age rapidly, older people with frailty are presenting for surgery with increasing frequency. This means that anesthesiologists and other perioperative clinicians need to be familiar with frailty, its assessment, manifestations, and strategies for optimization. We present a narrative rev

Screening for chlamydia and/or gonorrhea in primary health care: protocol for systematic review.

BACKGROUND: Chlamydia trachomatis and Neisseria gonorrhoeae are the most commonly reported sexually transmitted infections in Canada. Existing national guidance on screening for these infections was not based on a systematic review, and recommendations as well as implementation considerations (e.g., population groups, testing and case management) should be explicit and reflect the quality of evidence. The aim of this systematic review is to synthesize research on screening for these infections in sexually active individuals within primary care. We will also review evidence on how people weigh

Lives saved from malaria prevention in Africa--evidence to sustain cost-effective gains.

Lives saved have become a standard metric to express health benefits across interventions and diseases. Recent estimates of malaria-attributable under-five deaths prevented using the Lives Saved tool (LiST), extrapolating effectiveness estimates from community-randomized trials of scale-up of insecticide-treated nets (ITNs) in the 1990s, confirm the substantial impact and good cost-effectiveness that ITNs have achieved in high-endemic sub-Saharan Africa. An even higher cost-effectiveness would likely have been found if the modelling had included the additional indirect mortality impact of ITNs

Cost of production of live attenuated dengue vaccines: a case study of the Instituto Butantan, Sao Paulo, Brazil.

BACKGROUND: A vaccine to prevent dengue disease is urgently needed. Fortunately, a few tetravalent candidate vaccines are in the later stages of development and show promise. But, if the cost of these candidates is too high, their beneficial potential will not be realized. The price of a vaccine is one of the most important factors affecting its ultimate application in developing countries. In recent years, new vaccines such as those for human papilloma virus and pneumococcal disease (conjugate vaccine) have been introduced with prices in developed countries exceeding $50 per dose. These price

Burden of cervical cancer in Europe: estimates for 2004.

The European Council recommends that organised cervical cancer screening be offered in all member states. In order to evaluate the impact of existing and new prevention methods, regularly updated information on the burden of cervical cancer is needed. The best estimates of mortality and incidence rates were applied to the 2004 projected population of 40 European countries using methods developed by the International Agency for Research on Cancer. Using the absolute number of cases and deaths, the standardised and cumulative rates (up to age of 74 years) were computed for individual countries,

National recommendations for donation after cardiocirculatory death in Canada: Donation after cardiocirculatory death in Canada.

These recommendations are the result of a national, multidisciplinary, year-long process to discuss whether and how to proceed with organ donation after cardiocirculatory death (DCD) in Canada. A national forum was held in February 2005 to discuss and develop recommendations on the principles, procedures and practice related to DCD, including ethical and legal considerations. At the forum's conclusion, a strong majority of participants supported proceeding with DCD programs in Canada. The forum also recognized the need to formulate and emphasize core values to guide the development of programs

Effect of Lifestyle Modification Through Web-Based Telerehabilitation Monitoring Combined With Supervised Sensorimotor Training After Total Knee Arthroplasty: Randomized Controlled Trial.

BACKGROUND: Total knee arthroplasty (TKA) is commonly performed to manage end-stage knee osteoarthritis, yet postsurgical recovery varies significantly among patients. Lifestyle modification and rehabilitation interventions play a critical role in optimizing outcomes. While telerehabilitation has shown promise in enhancing accessibility and compliance, its role in supporting lifestyle behavior change alongside supervised sensorimotor training remains underexplored. OBJECTIVE: This study aimed to evaluate the effects of a home-based lifestyle modification program delivered through web-based te

Large Language Model-Based Behavioral Activation Chatbot for Young People With Depression Using Artificial Users and Clinical Experts: Mixed Methods Evaluation.

BACKGROUND: Mental health chatbots are increasingly used to support people with depressive symptoms, and large language models make these systems more flexible than rule-based chatbots. However, it remains unclear how well large language model-based chatbots deliver structured psychological interventions. OBJECTIVE: This study examined how well a GPT-4o-based chatbot delivered a behavioral activation intervention for young people with depression using sessions with artificial users and clinical expert assessment. It also identified limitations and potential refinements. METHODS: We implement

Co-development, initial evaluation and implementation of a digital health intervention (SmartADHD) for young people (aged 16-30) with attention deficit hyperactivity disorder, including optimisation for sustainable delivery via health and social care settings: protocol for a mixed-methods study.

INTRODUCTION: Attention deficit hyperactivity disorder (ADHD) is a neurodevelopmental condition affecting 5–7% of children/adolescents and 2–5% of adults. It is characterised by inattention, hyperactivity and impulsivity, impacting functioning across multiple domains with linked social and economic burden. Despite effective treatments that reduce the risk of negative outcomes, the UK is currently failing to provide equitable access to healthcare for people with ADHD. Young people with ADHD are at a vulnerable life-stage, facing multiple transitions such as moving educational settings, starting

The Digital Diabetes Patient Reminder Tool for Adults With Type 2 Diabetes: Protocol for a Proof-of-Concept Randomized Controlled Trial.

BACKGROUND: About 828 million adults were estimated to be living with diabetes, and this is projected to affect 1 in 9 adults by 2045. In Belgium, 7.6% of adults have been diagnosed with the disease; however, approximately 1 in 3 people remain unaware of their condition. Advances in information and communication technology (ICT) have improved care quality and health management, and facilitated communication between health care providers and patients. The Digital Diabetes Patient Reminder (DIPAR) tool was developed as an extension of the existing patient portal to support diabetes management th

MINMON-J: a hybrid implementation pilot study evaluating a low-barrier hepatitis C treatment model in a jail setting.

INTRODUCTION: Hepatitis C virus (HCV) remains a leading cause of infectious disease-related morbidity in the USA, disproportionately affecting people who inject drugs and people who are incarcerated. Despite the availability of highly effective, highly tolerated direct-acting antivirals, treatment uptake in jails remains limited due to short stays, unpredictable release dates and system-level barriers. The original MINMON trial demonstrated that a low barrier 'minimal monitoring"' model can achieve high cure rates in community settings. This study, MINMON-J, aims to adapt and evaluate a modifi

Comprehensive review of the evidence regarding the effectiveness of community-based primary health care in improving maternal, neonatal and child health: 1. rationale, methods and database description.

BACKGROUND: Community-based primary health care (CBPHC) is an approach used by health programs to extend preventive and curative health services beyond health facilities into communities and even down to households. Evidence of the effectiveness of CBPHC in improving maternal, neonatal and child health (MNCH) has been summarized by others, but our review gives particular attention to not only the effectiveness of specific interventions but also their delivery strategies at the community level along with their equity effects. This is the first article in a series that summarizes and analyzes th
