Impact on beclometasone dipropionate pharmacokinetics when switching to a low global warming potential propellant in a pressurised metered-dose inhaler.

INTRODUCTION: Use of high global warming potential propellants (e.g., HFA-134a) for pressurised metered-dose inhalers (pMDIs) is being phased down. Beclometasone dipropionate (BDP) is approved for the treatment of asthma in several countries via an HFA-134a propellant pMDI. This is being reformulated using the low global warming potential propellant HFA-152a. Two studies compared BDP pharmacokinetics delivered via pMDI using HFA-152a vs HFA-134a. METHODS: Both studies (N = 71/study) were single-dose (four inhalations of BDP), randomised, double-blind, crossover (Study 1, four-way; Study 2, tw…

Spray-Drying Process Optimization and Modeling for an Inhaled Dry Powder of 5-Azacytidine for Treating Local and Metastatic Lung Cancer.

A spray-dried powder formulation of 5-azacytidine (5-AZA) recently showed significantly improved tumor reduction and systemic exposure for the potential treatment of metastatic lung cancer. To support clinical use, the target product profile required double the active loading. Here, a series of powders were spray dried to increase 5-AZA loading in the formulation, and process parameters were varied to identify variables controlling powder properties. Formulations were sprayed via in-line mixing, where separate dimethyl sulfoxide (DMSO) and aqueous solutions were mixed immediately upstream of a

Prediction of Local Drug Exposure of Dry Powder Inhaler CXG87 (budesonide/formoterol) Using Physiologically-Based Pharmacokinetic Model.

Inhaled formulations are the first choices for treating asthma and chronic obstructive pulmonary disease (COPD). Both local and systemic exposures need to be considered when assessing the efficacy and safety of inhaled drugs. Physiologically-based pharmacokinetic (PBPK) models were constructed and verified for budesonide and formoterol with different inhalation devices, CXG87 (test (T) formulation) and Symbicort® Turbuhaler® (reference (R) formulation). The models were then used to evaluate local exposures at different peak inspiratory flow rates (PIFRs) and dose conditions. Local exposures of…

Adherence to Inhaled Medication Guidelines in Patients with COPD: Evaluating the GOLD 2023 Strategy in a Real-World Secondary Care Setting.

INTRODUCTION: The 2023 Global Initiative for Chronic Obstructive Lung Disease (GOLD) strategy introduced updates to the prescription of inhaled medication, including withdrawing recommendation of long-acting beta-agonist/inhaled corticosteroids (LABA/ICS) as double therapy, restricting ICS use to patients with exacerbations, and recommending earlier escalation to triple therapy. These changes are expected to have a substantial impact on clinical practice. This study aims to describe how well clinicians adhere to the GOLD strategy in daily practice, and to explore the extent to which the 2023 u

Survival benefits of propofol-based versus inhalational anesthesia in non-metastatic breast cancer patients: a comprehensive meta-analysis.

Whether the anesthesia technique, inhalational general anesthesia (IGA) or propofol-based anesthesia (PBA), influences the long-term survival of non-metastatic breast cancer (eBC) remain unclear and controversial. We carried out a literature search on 16thJuly, 2022 for studies comparing IGA and PBA in eBC undergoing standard surgery, according to PRISMA 2020. The major endpoint in our study was overall survival (OS). Seventeen studies including four randomized clinical trials and thirteen retrospective cohort studies were included in the meta-analysis. Ten studies provided data for crude OS i…

Asthma prescribing trends, inhaler adherence and outcomes: a Real-World Data analysis of a multi-ethnic Asian Asthma population.

Inhaled corticosteroid (ICS) is the mainstay therapy for asthma, but general adherence is low. There is a paucity of real-world inhaler prescribing and adherence data from Asia and at the population level. To address these gaps, we performed a real-world data analysis of inhaler prescribing pattern and adherence in a multi-ethnic Asian asthma cohort and evaluated the association with asthma outcomes. We performed a retrospective analysis of adult asthma patients (aged ≥18 years) treated in the primary and specialist care settings in Singapore between 2015 to 2019. Medication adherence was meas
