The occurrence of a fusion of three primary incisors is rare. A two-year-old Japanese girl was brought to the pediatric dental outpatient clinic, Tokyo Dental College, to receive a caries-prevention treatment. The eruption of the canine is known to be a factor for diastema closure. In this study the relative changes in the orientation of the unerupted canine were assessed using the orthopantomographs of 9854 patients who sought consultation between April 1984 and March 1993. Obstructive sleep apnoea (OSA) has been associated with many life-threatening conditions but has only recently appeared in the dental literature. Dental appliances and orthognathic surgery are two strategies which are currently used in the treatment of sleep apnoea. This article provides a background on OSA and these treatment approaches, and discusses the potential risks and benefits of each. A case is reported to illustrate the use of a dental appliance in the treatment of OSA.

A case is described of resorption of the mesial aspect of a maxillary permanent first molar in a 13-year-old boy whose maxillary second premolar was impacted. The premolar was partially erupted in the line of the arch and radiography revealed a radiolucent area involving enamel, dentine and cementum on the mesial aspect of the first molar. The first molar was extracted and the premolar erupted normally. The case emphasizes the importance of early diagnosis of developmental abnormalities.

Two cases of hypoglossia congenita with anterior maxillo-mandibular fusion are reported. One is a case of hypoglossia with anterior maxillo-mandibular fusion and the other is a case of hypoglossia-hypodactylia with anterior maxillo-mandibular fusion. This condition presents the anaesthetist with the problem of airway management during the surgery. A simple technique using a nasopharyngeal tube was used for these two cases, with satisfactory results.
