Early controlled motion with dynamic splinting versus static splinting for zones III and IV extensor tendon lacerations: a preliminary report.

Recent developments in the understanding of tendon healing and the effects of early motion have led to early controlled motion programs (ECM) for lacerated extensor tendons. The purpose of this study was to determine whether there were differences in length of treatment and final results when patients who had been treated with ECM with dynamic splinting were compared with patients who had been treated with traditional static extension splinting (SES) for zone III and zone IV extensor tendon lacerations. A retrospective study of patient charts from June 1984 through January 1990 was conducted. …

A comparison of post-operative mobilization of flexor tendon repairs with "passive flexion-active extension" and "controlled active motion" techniques.

We report a comparative study of the outcome of flexor tendon repairs mobilized by either a "passive flexion-active extension" or a "controlled active motion" regimen. We show that the controlled active motion regimen conferred significant benefits on the final range of motion and extensor lag. The rupture rate was raised with "controlled active motion" but this was not greater than levels reported by other authors using "passive flexion-active extension" regimens.

Rectus femoris surgery in children with cerebral palsy. Part I: The effect of rectus femoris transfer location on knee motion.

Rectus femoris transfer was performed in 78 children (105 sides) with cerebral palsy (CP) at the same time as other surgical procedures as appropriate. The transfer was either medial to the sartorius (62 sides), semitendinosus (19 sides), or the gracilis (14 sides) muscles, or laterally to the iliotibial band (10 sides). Gait analysis performed before and 1 year after operation demonstrated increased knee range of motion (ROM) with increased extension at initial contact and in midstance and maintained knee flexion in swing. There were no statistically significant differences between the four t…

High tibial osteotomy. Use of an osteotomy jig, rigid fixation, and early motion versus conventional surgical technique and cast immobilization.

High tibial osteotomy (HTO) using conventional surgical technique and cast immobilization was compared to HTO using an osteotomy jig, rigid internal fixation, and early motion. Fifteen patients (19 knees) had conventional HTO. The mean preoperative femoral-tibial angle was 2.5 degrees of varus, and the mean postoperative angle was 6.5 degrees of valgus. Two knees were undercorrected and eight knees (42%) had associated complications. Twenty patients (21 knees) had HTO utilizing the new surgical technique and postoperative management. The mean preoperative femoral-tibial angle was 2.3 degrees v…

Relationship of regional wall motion abnormalities to hemodynamic indices of myocardial oxygen supply and demand in patients undergoing CABG surgery.

To investigate the hemodynamic correlates of perioperative regional wall motion abnormalities (RWMA), we measured wall motion continuously via transesophageal echocardiography (TEE), and related RWMA to continuously measured hemodynamic indices of myocardial oxygen supply and demand (heart rate [HR] and systemic and pulmonary arterial blood pressures). Fifty patients undergoing coronary artery bypass graft (CABG) surgery were studied throughout the prebypass postbypass, and intensive care unit (ICU) periods. Only 28% of TEE episodes (RWMA suggestive of ischemia) were preceded by acute changes …

Role of resting thallium201 perfusion in predicting coronary anatomy, left ventricular wall motion, and hospital outcome in unstable angina pectoris.

We performed quantitative thallium scintigraphy in 66 unstable angina patients, 5.6 +/- 5.1 hours after rest pain, to predict coronary anatomy, left ventricular wall motion, and hospital outcome. Thallium defects and/or washout abnormalities were present in 5 of 10 (50%) patients with coronary stenoses less than 50%, 27 of 33 (82%) patients with coronary stenosis greater than or equal to 50% and no history of previous myocardial infarction, and in 23 of 23 patients (100%) with histories of previous infarction. Defects were uncommon in the territory of vessels with less than 50% (13 of 61, 21%)…

Usefulness of echocardiographically determined mitral leaflet motion for diagnosis of mitral valve prolapse in 17- and 18-year-old men.

Mitral leaflet motion during systole was studied by echocardiography in 102 healthy young men. Mean posterior maximal leaflet motion was 2 +/- 1 mm behind the CD line on 2-dimensionally (2-D) directed M-mode examination. On the apical 4-chamber cross-sectional view a mean area of 0.34 +/- 0.24 mm2 was contained by the mitral leaflets above the plane of the mitral anulus. Elevated values on the M-mode view (greater than or equal to 4 mm) compared with the 2-D 4-chamber view (greater than or equal to 0.70 mm2) were discordant, with 90% (18 of 20) of the elevated values found in 1 view only. Thus…

Simultaneous full- and half-energy Stark polarization measurements using a modified background polychromator motional Stark effect system.

The radial electric field (Er) is important for turbulence suppression and transport barrier formation in magnetically confined plasmas. To establish the diagnostic capability required for its future determination, the Korea Superconducting Tokamak Advanced Research (KSTAR) Motional Stark Effect Background Polychromator was modified to measure the polarization angles of the full-energy π and half-energy σ Stark components simultaneously along a common line of sight (LOS). Interference filters and operating temperatures were selected using the calculated Stark spectra and the existing filter in…

Towards Clinical Application of Enhanced Timed up and Go With Markerless Motion Capture and Machine Learning for Balance and Gait Assessment.

Balance and gait impairments play a key role in falls among the elderly. Traditional clinical scales such as the Berg Balance Scale (BBS) to assess fall risk are often subjective, time consuming, and do not assess gait performance. Shorter assessments such as Timed Up and Go (TUG) are available, but most clinicians only look into the completion time. This study aimed to develop a fast, low-cost, and automated framework for balance function assessment and comprehensive gait analysis by enhancing the traditional TUG test with a markerless motion capture (MoCap) system and machine learning models…

An ab initio study and machine learning framework to capture the motional effects in solid-state NMR of lithium-ion conductors.

Solid-state NMR spectroscopy, when combined with first-principles density functional theory (DFT) calculations, offers a highly sensitive pr … resentative of Li-ion diffusion from machine-learning molecular dynamics. Compared with vibrational motion, Li-ion hopping around Cl is shown to dominate the motional averaging through reorientation of the NMR tensors. This study therefore provides an efficient computational method to resolve the complexities of the NMR spectra of Li6PS5Cl, which can be …

The flash-lag effect and motion perception timing: a framework grounded in a proposed biophysical mechanism for perception as a first-person property.

The flash-lag effect (FLE), a dissociation between physical events and their perceived time and location, has an unresolved neural basis. Here, the FLE is re-examined within the semblance hypothesis, in which perception, a first-person property, arises from the rapid formation and reversal of inter-postsynaptic functional LINKs (IPLs). A novel, unpredictable stimulus such as a flash is hypothesized to require de novo IPL formation, with perceptual latency Tpercept(flash) ≈ 70-100 ms. Predictable, continuously tracked motion is instead hypothesized to reactivate IPLs formed at motion onset, wit…

Decoupling Photochemical and Photothermal Effects Using Molecular Motors Enables Fast, Intelligent, and Life-Like Motions in Soft Materials.

The development of synthetic smart materials that perform on-demand tasks in complex environments, while triggered noninvasively, is a major goal to develop the next generation of soft robotics. However, fast and complex locomotion with high spatial-temporal precision using external stimuli controlling shape in such systems remains a major challenge. Here, we describe a light-responsive soft material based on liquid crystal polymer networks (LCPNs) with variable stiffness comprising highly efficient light-driven molecular rotary motors and dye molecules. This system selectively responds to lig…
