Deletion of a branch-point consensus sequence in the LMX1B gene causes exon skipping in a family with nail patella syndrome.

Nail patella syndrome (NPS) has been shown to result from loss of function mutations within the transcription factor LMX1B. In a large NPS family a 17 bp intronic deletion encompassing a consensus branchpoint sequence was observed to segregate with the NPS phenotype. RNA analysis demonstrated that deletion of the branchpoint sequence resulted in skipping of the downstream exon. A mechanism to explain this phenomenon is presented.

Elastic stable intramedullary nailing (ESIN) provides good long-term results in pediatric long-bone fractures.

The present study evaluates the long-term outcome of elastic stable intramedullary nailing (ESIN) in children with long-bone shaft fractures. 70 patients were examined, the present mean age of patients was 12.7 (+/- 3.26) years, mean time interval since surgery was 3.1 (+/- 1.49) years. Following ESIN at the lower extremity (23 femur fractures, 14 tibia fractures), keloid formation was remarkable in one patient, range of motion (ROM) was adequate in all but one boy (extension deficit of the hip of 10 degrees). Overall leg length discrepancy was +4.1 mm (+/- 5.2 mm) following ESIN for femur fra

[Postero-medial elastic stable intramedullary nailing for anteriorly displaced distal diaphyso-metaphyseal fractures of the radius in children].

PURPOSE: To suggest a mechanically satisfying method to stabilize the unfrequent distal diaphyso-metaphyseal fractures of the radius, when they are anteriorly displaced, avoiding the classical use of an anterior osteosynthesis by plate and screws. MATERIAL AND METHODS: Ten cases of postero medial Elastic Stable Intramedullary Nailing are presented. Technically, a small postero medial approach with gentle retraction of extensors tendons is used. Then a hole is drilled through the postero medial metaphysis of the distal radius. A present wire is introduced intramedullary, directing its elastic

[Intramedullary nailing osteosynthesis with epiphyseal fixation of fractures of the proximal humerus in adults. 1 to 3-year follow-up].

The authors used a medullary pinning with epiphyseal fixation in the treatment of superior metaphyseal fractures of the humerus for 41 patients aged 22 to 82 years. There were 28 cases with epiphyseal fractures and 12 with metaphyseal fractures. The follow-up was 1 to 3 years. The pins were introduced through the triceps brachii after perforation of the diaphysis, 1 cm above the ulnar fossa. Results were very good. Strength returned within 8.5 weeks. There was only one case of nonunion of a proximal diaphyseal fracture due to a technical fault (diastasis between fragments). Rehabilitation bega

The Gamma nail, sliding-compression plate. A comparison between the long-term results obtained in two similar series.

The authors report the results they obtained in a retrospective comparative study conducted on two systems of osteosynthesis used to treat trochanteric fractures: the sliding-compression screw-plate (CHS), and the Gamma nail. Two series were included in the study, each of which was made up of 50 cases; these were comparable in terms of fracture type, age of the patients, associated pathologies, and minimum follow-ups of 6 months. The parameters compared were: surgical trauma, average amount of time before a standing position could be resumed, resumption of motor activity previous to trauma, me

[Initial clinical experience with osteosynthesis of femoral shaft fractures with a newly developed intramedullary implant (claw interlocking nail)].

The implantation of distal interlocking screws in interlocking nailing can be difficult and time-consuming. With the AO Universal Femoral Nail as the basis, an implant was designed with which distal interlocking is accomplished from the inside of the nail by means of a simple claw mechanism. Instrumentation and interlocking are possible from the proximal standard approach, so that an additional operation adjacent to the knee (which is necessary when interlocking bolts are used) is superfluous; neither for the insertion nor for the removal of the interlocking mechanism is an image intensifier n

Undisplaced fracture of the neck of the femur: results of treatment of 100 patients treated by single Watson-Jones nail fixation.

In a review of 100 patients with clinical and radiological evidence of undisplaced Garden stage I and II fractures of the neck of the femur treated by fixation with a single Watson-Jones nail, the early mortality was 4 per cent and in 93 cases with complete radiological data important displacement of the fracture during surgery occurred on one occasion. Of 72 patients with a mean follow-up of 22.6 months (range 3-83 months) 68 patients (94.4 per cent) achieved union and non-union occurred in 4 patients (5.6 per cent) all of whom required further surgery. In 40 patients with united fractures wi

[The risk non-union following closed-focus nailing and reaming. Results of 1059 interventions using the Kunstcher method].

The authors have studied a series of 1059 compound and closed fractures of the diaphysis of long bones treated by intramedullary nailing according to the technique of Kuntscher, to assess the incidence of non-union. Between January 1st 1967 and December 31st 1980, 503 fractures of the tibia, 440 of the femur and 116 of the humerus were so treated in both adolescents and adults. The fractures were mostly sustained in road traffic accidents. The risk of aseptic non-union was small, but occurred more readily when the skin was damaged and when the site of the fracture was at the junction of the lo

External validation of the FRACTING, LEG-NUI, and NURD scores for predicting non-union after tibial intramedullary nailing.

BACKGROUND: Non-union remains a challenging complication of tibial diaphyseal fractures treated with intramedullary nailing. Several clinical prediction scores have been developed to identify patients at increased risk of impaired fracture healing. This study aimed to externally validate the FRACTING, LEG-NUI, and NURD scores for the prediction of non-union in an independent cohort of patients with tibial diaphyseal fractures. METHODS: We retrospectively reviewed consecutive adult patients with tibial diaphyseal fractures treated with reamed intramedullary nailing at a level I trauma center b

Atypical renal phenotype with tubular proteinuria and hypercalciuria in siblings with nail-patella syndrome: evidence for a dual genetic diagnosis.

Nail-patella syndrome (NPS) is an autosomal dominant disorder caused by LMX1B mutations and typically associated with glomerular proteinuria. We report three brothers carrying a maternally inherited heterozygous LMX1B mutation (NM_001174147.2: c.309C > G (p.Cys103Trp)), all presenting with classical skeletal features of NPS. Two siblings developed early-onset proteinuria predominantly composed of low-molecular-weight proteins, hypercalciuria, and ophthalmologic abnormalities including cataracts and glaucoma; one also exhibited nephrocalcinosis. Further genetic analysis identified a hemizygous

Retrograde Intramedullary Nailing Versus Locking Plate Fixation in Extra-Articular Distal Femur Fractures: A Prospective Study.

INTRODUCTION: Distal femur fractures require stable fixation that permits restoration of alignment and early functional recovery. Retrograde intramedullary nailing distal femur nailing (DFN) and distal femoral locking plate fixation locking compression plate (LCP) are commonly used for extra-articular fractures, but their comparative functional and radiological outcomes remain debated. AIM: This study compared operative, functional, radiological, and complication outcomes of DFN and LCP in adult patients with extra-articular distal femur fractures. Self-inflicted thoracoabdominal nail-gun injury presenting as STEMI: diagnostic challenges in a time-critical care pathway.

A 60-year-old male with a history of depression and a prior nail-gun thoracic injury was found in shock at home, with inferior ST-segment elevation on the paramedic electrocardiogram prompting pre-hospital ST-elevation myocardial infarction pathway activation. Initial high-sensitivity troponin was elevated, consistent with the presumed diagnosis. Normal coronary angiography and preserved ventricular function were discordant with the patient's ongoing haemodynamic instability. Four small anterior chest puncture wounds and radiopaque foreign bodies demonstrated on chest radiograph and angiograph

Intraoperative Abdominal Penetration of the Lag Screw: A Rare Complication During Proximal Femoral Nail Anti-Rotation Surgery.

Although the proximal femoral nail anti-rotation (PFNA) is accompanied by several benefits, using the nail is technically challenging and may pose some errors, leading to osteosynthesis failure. Here, we report a critical presentation of an uncommon side effect of PFNA surgery. Intraoperative abdomen penetration via lag screw is a unique complication of PFNA surgery of which surgeons should be aware.

Network-based analysis of images and lesions expressed by digital interpretation and transmission of bronchoscopy findings on social media (NAILED-IT).

INTRODUCTION: Diagnostic bronchoscopy has advanced rapidly over the last two decades with the adoption of novel technologies. Concurrently, social media platforms have proliferated, with some pulmonologists sharing and promoting these new technologies and diagnostic successes. The impact of this content on perceptions of diagnostic yield and utility remains unclear. METHODS: A scoring system was developed to evaluate social media posts related to bronchoscopy. The BIASED score ranges from 1 to 6 points, awarding one point each for the inclusion of: Brand/technology mention, Imaging (diagnosti

Mercury exposure in breastfeeding mothers and associated health risks to infants: Evidence from breast milk and nail biomarkers.

Mercury (Hg) is a toxic environmental pollutant that can enter the human body through diet and environmental exposure and may be transferred to infants via breastfeeding. This study aimed to determine Hg concentrations in maternal fingernails and breast milk, identify factors influencing mercury levels in milk and assess potential health risks to breastfed infants. A total of 74 breastfeeding mothers were engaged from urban health centers. Hg concentrations were measured using atomic absorption spectrophotometry and dietary, maternal and infant characteristics were recorded through structured …

Comparison of fluoroscopic versus electromagnetic distal locking in humeral shaft osteosynthesis using intramedullary nail: a retrospective study.

PURPOSE: Intramedullary nailing is a standard method for osteosynthesis of diaphyseal humeral fractures. Distal locking, however, typically requires fluoroscopic guidance, which increases radiation exposure. This study aimed to compare standard fluoroscopy-guided distal locking with the Ezy-Aim system (Austofix, Australia) based on electromagnetic pulse detection from the perspectives of surgical times, radiation exposure, and complications. METHODS: A retrospective study included 58 patients treated for diaphyseal humeral fractures (AO/OTA 12A, 12B) between 2019 and 2023. Group A (n = 28) re

Femoral artery pseudoaneurysm as a late complication of flexible intramedullary nailing for a diaphyseal femur fracture in a child: a case report.

Flexible intramedullary nails are a safe, effective, and low-morbidity option for the treatment of diaphyseal femur fractures in children. They generally provide acceptable results regarding bone consolidation and clinical stabilization. However, late vascular events may occur. In this report, we describe the case of a male patient who underwent osteosynthesis with flexible nails for a femur fracture at age 9 and 5 years later developed local pain and swelling, leading to detection of a left femoral artery pseudoaneurysm. Subsequently, surgical correction with revascularization using an ipsila…

Impact of Operative Position on Outcomes of Proximal Femoral Nailing: A Comparison Between Supine on a Traction Table and Lateral Decubitus.

Introduction Unstable intertrochanteric fractures are common in older adults and are best stabilised with cephalomedullary nails. Surgeons favour either a supine position on a traction table or a lateral decubitus position on a standard table, yet evidence comparing their clinical value is limited and often conflicting. The traction table may simplify reduction and imaging but introduces set-up delays and post-related complications; lateral positioning avoids the post but relies on manual traction. This study aims to determine whether patient position meaningfully impacts intraoperative, radio…
