Recurrent hyperparathyroidism from benign neoplastic seeding: a review with recommendations for management.

Benign neoplastic seeding represents a rare cause of hyperparathyroidism found at parathyroid reoperations. It consists of pathologically benign neoplastic parathyroid tissue scattered throughout a previous parathyroid surgical site. We describe a case of recurrent and then persistent hyperparathyroidism occurring in a patient during an 18-year period. The patient's third operation showed nonmalignant parathyroid tissue throughout the right neck bed, which necessitated extensive parathyroid and fibrofatty tissue dissection and a thyroidectomy. This unexpected finding of extensive parathyroid s

Stage I cutaneous malignant melanoma: risk factors of loco-regional recurrence after wide local excision and clinical perspectives.

Two hundred and nineteen patients admitted to the Centre Claudius Regaud over a 14-year period for a stage I cutaneous malignant melanoma were retrospectively evaluated for loco-regional recurrence rates, risk factors and survival rates following wide primary excision. Five and 8 year survival rates corrected for deaths owing to concurrent illness were 77% and 73%. The loco-regional control rate was 69% (151/219). Distant metastases occurred in 59% (40/68) of patients who had a loco-regional recurrence, versus 11% (16/151) of patients when loco-regional control was obtained (P < 0.001). Multiv

Surgical management of BANS malignant melanoma.

Because melanomas of the upper back, posterior arm, neck, and scalp (BANS) have been shown to be at high risk for recurrence and metastases, a tendency is to carry out wide excision (WE) and elective radical node dissection (RND) for patients with such lesions. To investigate whether this regimen improves the survival rate, we reviewed retrospectively 62 patients with clinical stage I BANS melanoma. Forty-five patients were treated by WE alone and 17 patients were treated by WE and RND (WE + RND). The male:female ratio was 1.5:1 for WE and 1.8:1 for WE + RND. The mean age was 52.9 +/- 15.3 yea

[An operated case of multiple endocrine neoplasia type IIb and review of the Japanese literature].

Multiple endocrine neoplasia, type IIb (MEN IIb) is a rare syndrome characterized by the occurrence of medullary thyroid carcinoma (MTC), pheochromocytoma and mucosal neuroma. A 35-year-old male patient with MEM IIb having megacolon, marfanoid habitus and no family history of the disease underwent surgery. Because MTC was present in both lobes, total thyroidectomy and modified neck dissection were performed. Pheochromocytoma was found bilaterally and bilateral adrenalectomy with adrenal autotransplantation in the rectus abdominis muscle was carried out. Postoperative course was satisfactory ex…

Cetuximab-associated pulmonary toxicity in concurrent chemoradiation for the treatment of a squamous cell carcinoma of the head and neck.

BACKGROUND: Cetuximab is a common EGFR monoclonal antibody used with radiotherapy to treat head-and-neck cancer. Severe pulmonary toxicity, including interstitial lung disease (ILD), caused by cetuximab is rare. METHODS: We describe a patient who developed ILD and acute respiratory failure after concurrent chemoradiation with cetuximab for oropharyngeal squamous cell carcinoma, and review the literature. RESULTS: A patient developed acute respiratory failure 2 months after starting concurrent chemoradiation with cetuximab and was hospitalized in intensive care after a procedure for progressi

Pulmonary thromboembolism after head and neck surgery.

In a retrospective study of all patients who had pulmonary embolism during a 37-year span at M. D. Anderson Hospital, we identified 502 patients, 30 of whom had a primary malignancy in the head and neck region. Only five of these patients had a clinically significant pulmonary embolus during the immediate postoperative period; these patients are the subject of a more detailed review with a case study as an example. Three of these patients died. Heart disease was identified as the most common predisposing factor. We review the results of this clinical study of pulmonary thromboembolism in patie

Soft-tissue defects of the head and neck that are too extensive to be reconstructed with local flaps without excessive impairment of either … s the oral cavity and oral pharynx and the muscle provides good coverage of the carotid artery when neck dissection has been accomplished. A great advantage of the flap is the ability to achieve reconstruction in one stage without the need for a controlled salivary fistula. Although the deltopectoral flap is the author's preferred regional flap for reconstruction …

Gastric Regional Lymph Node Metastases from a Squamous Cell Carcinoma of Unknown Primary Site: A Case Report.

INTRODUCTION: A cancer of unknown primary site is a malignant tumor for which the primary site is unknown despite a thorough examination, and which has been histologically proven to be a metastatic lesion. Metastases to intraperitoneal and to gastric regional lymph nodes are rare. CASE PRESENTATION: A 75-year-old woman was diagnosed with a gastric submucosal tumor with infiltration to other organs. Endoscopic ultrasound-guided fine needle aspiration revealed cells that appeared to be derived from epithelial tissue, but a definitive diagnosis could not be obtained. Because the possibility of g

The optimal temporary occlusion time for the treatment of internal carotid artery aneurysms.

Post-operative silent DWI hyperintensities are potential risk factors for cognitive dysfunction. Microsurgical clipping offers high occlusion rates and low recurrence, and temporary occlusion is often essential to safely dissect aneurysms. However, addressing these DWI changes is crucial for improving outcomes and minimizing complications. This study aimed to determine the optimal temporary occlusion time for ICA aneurysm treatment. A retrospective analysis was performed on 267 cases of ICA aneurysms. Clinical outcomes and factors associated with post-operative DWI hyperintensities were analyz

Dissection of RET p.M918T-driven progression of hereditary vs. sporadic medullary thyroid cancer.

BACKGROUND: Whether inherited in the context of multiple endocrine neoplasia 2B at germline level or acquired in a lifetime, all RET p.M918T (RET c.2753T>C) mutations should activate the RET tyrosine kinase receptor alike, with similar degrees of medullary thyroid cancer (MTC) progression when disparities in disease onset and multifocal growth are accounted for. METHODS: This cross-sectional analysis of RET p.M918T-driven progression of hereditary MTC (33 patients) vs. sporadic MTC (36 patients) sought to explore this hypothesis. RESULTS: Patients with hereditary disease were significantly y

Clinical and genetic characterization of adult patients presenting with non-syndromic vascular aneurysms and dissections.

BACKGROUND: Genetic disorders affecting the arterial tree in the form of aneurysms and dissections are highly morbid conditions that strike younger persons leading to bleeding, infarction, or even death. Although clinically recognizable syndromes, notably Marfan, Ehlers Danlos, and Loeys-Dietz syndromes encompass the principal diagnosable phenotypes along the genetic vascular disorder spectrum, a substantial subset of patients cannot be adequately classified under a known diagnosis through clinical or molecular diagnostic methods. Here we describe patients presenting with multiple-aneurysms an

18F-FDG uptake in reactive neck lymph nodes of oral cancer: relationship to lymphoid follicles.

UNLABELLED: PET using (18)F-FDG is acceptable as a preoperative diagnostic tool for head and neck cancer. PET combined with CT provides precise localization of neck lymph nodes. Reactive lymphadenopathy is well known as a principal cause of false-positive findings on PET/CT for nodal staging. We investigated the reactive lymph nodes of oral cancer to elucidate the (18)F-FDG-avid area in these nodes. METHODS: Surgically dissected neck lymph nodes of oral cancer were retrospectively reviewed. Of the patients without pathologic nodal metastasis who underwent preoperative PET/CT, 11 patients with
