Depression in Persons With Epilepsy: Lessons From Case Review.

BACKGROUND: Major depressive disorder is highly prevalent among persons with epilepsy (PWEs). Between 30% and 50% of PWEs suffer from depression. Many factors contribute to this prevalence, including the psychosocial impact of the diagnosis, restrictions on driving and certain types of work, and adverse effects associated with antiseizure medications. Without proper treatment, depressed PWEs have increased risks for suicide, strained relationships, lowered seizure control, and impairment in functioning. Our objective was to use the existing literature and insights from our experience in treati

A Review of Academic Use of the Term "Minor Attracted Persons".

Although it originated within online pro-pedophile groups, the term "minor attracted person" (MAPs) has been adopted by some academic researchers as a neutral and non-stigmatizing alternative to the term "pedophile." The transferral of this term from pedophile advocates to academic scholarship has been highly controversial. Claims that the use of the term "minor attracted people" normalizes or endorses pedophilia deserve closer scrutiny. This paper is based on a rapid evidence review of all peer-reviewed papers between 2015 and 2023 that used variants of the term "minor attracted" in their tit

'Personalized medicine': phenotyping pediatric obstructive sleep apnea.

PURPOSE OF REVIEW: Obstructive sleep apnea (OSA) is common in children. Phenotyping pediatric OSA has a crucial role in personalized diagnosis and treatment to improve outcomes for this population. This review sets forth a clinical approach that allows for phenotyping pediatric OSA. RECENT FINDINGS: The emerging concept of phenotyping pediatric OSA is based on identifying a primary cause, which leads to a more precise understanding of the pathogenesis in any individual patient. Phenotyping enables treatment focusing on the primary cause, but does not exclude the need for supplemental manageme

Is the response to antihypertensive drugs heterogeneous? Rationale for personalized approach.

Arterial hypertension represents the most important cardiovascular risk factor with a direct responsibility for a large share of cardiovascular mortality and morbidity in the world. Despite the wide availability of antihypertensive therapies with documented effectiveness, blood pressure control still remains largely unsatisfactory in large segments of the population. Guidelines for the management of arterial hypertension suggest the preferential use of five classes of drugs-angiotensin-converting enzyme inhibitors, angiotensin II type I receptor inhibitors, calcium channel blockers, thiazide/t

[The value of local antibiotics for foot infection in persons with diabetes].

Diabetes-related foot infections(DFI), including diabetes-related foot osteomyelitis, are generally treated with systemic antibiotics. However, systemic antibiotics are often overused and administered for excessively long periods, making patients more susceptible to multidrug-resistant bacteria. Local antibiotics may be beneficial to DFI patients. This article reviews the advantages and disadvantages, types and clinical evidence of local antibiotics in the treatment of DFI, and points out the research gaps and future research direction in this field. This article aims to provide a reference fo

Personalized use of ketamine and esketamine for treatment-resistant depression.

A large and disproportionate portion of the burden associated with major depressive disorder (MDD) is due to treatment-resistant depression (TRD). Intravenous (R,S)-ketamine (ketamine) and intranasal (S)-ketamine (esketamine) are rapid-acting antidepressants that can effectively treat TRD. However, there is variability in response to ketamine/esketamine, and a personalized approach to their use will increase success rates in the treatment of TRD. There is a growing literature on the precision use of ketamine in TRD, and the body of evidence on esketamine is still relatively small. The identifi

The Promise and Future of Radiomics for Personalized Radiotherapy Dosing and Adaptation.

Quantitative image analysis, also known as radiomics, aims to analyze large-scale quantitative features extracted from acquired medical images using hand-crafted or machine-engineered feature extraction approaches. Radiomics has great potential for a variety of clinical applications in radiation oncology, an image-rich treatment modality that utilizes computed tomography (CT), magnetic resonance imaging (MRI), and positron emission tomography (PET) for treatment planning, dose calculation, and image guidance. A promising application of radiomics is in predicting treatment outcomes after radiot

Restless legs syndrome: From clinic to personalized medicine.

Restless legs syndrome (RLS) is a common neurological sensorimotor disorder that impairs sleep, mood and quality of life. RLS is defined by an urge to move the legs at rest that increases in the evening and at night, and is frequently associated with metabolic and cardiovascular diseases. Symptoms frequency, age at RLS onset, severity, familial history and consequences of RLS vary widely between patients. A genetic susceptibility, iron deficiency, dopamine deregulation, and possible hypo-adenosinergic state may play a role in the pathophysiology of RLS. Polysomnographic recordings found often

Towards personalized therapies for genetic disorders of surfactant dysfunction.

Genetic disorders of surfactant dysfunction are a rare cause of chronic, progressive or refractory respiratory failure in term and preterm infants. This review explores genetic mechanisms underpinning surfactant dysfunction, highlighting specific surfactant-associated genes including SFTPB, SFTPC, ABCA3, and NKX2.1. Pathogenic variants in these genes contribute to a range of clinical presentations and courses, from neonatal hypoxemic respiratory failure to childhood interstitial lung disease and even adult-onset pulmonary fibrosis. This review emphasizes the importance of early recognition, th
