[Orthopaedic treatment of floating elbow in a child: a case report].

The floating elbow in the child is rare and serious. The authors report a traumatic case of floating elbow without vascular and nervous lesions associated in a 6-year-old child. We treated these both fractures of the distal forearm and the supracondylar humeral by respectively cast plaster and a collar and cuff with an anatomical reduction. The radiological and clinical results were excellent with the 29 months follow-up.

Treatment of forearm fractures by Hoffman external fixation. A study of 93 patients.

Ninety-three patients with acute fractures of the diaphysis of one or both bones of the forearm were treated by a Hoffmann external fixator, using a half-frame configuration. Whenever possible, closed methods of reduction were used. The versatility of the Hoffmann external fixator allows multiple attempts and possible secondary resumption of the reduction. The half-frame configuration is stable enough, allowing the restoration and maintenance of the precise anatomy of the forearm bones and early active postoperative motion. The median length of retention of the external fixator was 13 weeks. B

Skeletal management of humeral shaft fractures associated with forearm fractures.

Nine cases of ipsilateral humeral shaft and forearm fractures in multiply injured patients were reviewed in an attempt to ascertain the preferred management of these complex injuries. Satisfactory anatomic and functional results were achieved only in those cases treated by rigid internal fixation of the humeral fractures. Nonoperative management is associated with a significant risk of malunion or nonunion of the humerus.

The influence of timing and rigidity of internal fixation on bony union of fractures of the forearm.

Sixty-one patients with fractures of the shafts of both bones of the forearm treated by internal fixation were studied retrospectively. The importance of the rigidity of the fixation and the time that elapsed before operation was investigated. Eight cases of non-union and one of delayed union were noted after fractures which had been operated on during the first week by non-rigid internal fixation. On the other hand, in the group of rigidly fixed fractures, the time of operation was not found to have a statistically significant effect on the healing process.

Wrist and Forearm Fractures from Ballistic Injuries.

Ballistic fractures of the wrist and forearm warrant special consideration due to the proximity of neurovascular structures. Each case requires a thorough assessment for concomitant peripheral nerve and vascular injury as well as the presence of possible development of compartment syndrome, which may be difficult to detect in the setting of the multiply injured patient. The extent of bony and soft tissue injury with the degree of contamination present should be factored into decisions regarding antibiotic treatment and surgical debridement, and the strategy for fracture stabilization and manag

Ultrasound Secondary Signs for the Diagnosis of Pediatric Distal Forearm Fractures: A Diagnostic Study.

OBJECTIVE: To evaluate the diagnostic accuracy of ultrasound secondary signs of fractures in pediatric patients aged 5-15 y presenting to the emergency department with a clinically non-deformed distal forearm injury. METHODS: This diagnostic study was conducted in South East Queensland, Australia. Emergency clinicians performed point-of-care ultrasound on eligible patients and recorded secondary signs of fractures (pronator quadratus hematoma [PQH] sign, periosteal hematoma, visible angulation) or physeal fractures (fracture-to-physis distance [FPD], physis alteration). The reference standard

Artificial intelligence-based detection of paediatric appendicular skeletal fractures: performance and limitations for common fracture types and locations.

BACKGROUND: Research into artificial intelligence (AI)-based fracture detection in children is scarce and has disregarded the detection of indirect fracture signs and dislocations. OBJECTIVE: To assess the diagnostic accuracy of an existing AI-tool for the detection of fractures, indirect fracture signs, and dislocations. MATERIALS AND METHODS: An AI software, BoneView (Gleamer, Paris, France), was assessed for diagnostic accuracy of fracture detection using paediatric radiology consensus diagnoses as reference. Radiographs from a single emergency department were enrolled retrospectively goi

The use of the internal joint stabiliser for elbow instability Report of two cases and systematic review.

The management of elbow fracture-dislocations is challenging. The internal joint stabiliser (IJS) (Skeletal Dynamics, Miami, FL) has been advocated as an alternative to traditional techniques. This article shares our initial clinical experience and provide a systematic review analysing the ability of the IJS to maintain radiographic joint reduction and the associated complication profile. Two cases of elbow fracture-dislocations treated at our centre using the IJS are presented. A systematic review of the literature was conducted using the online databases Medline, Scopus and EMBASE. Deformed pediatric forearm fractures: Predictors of successful reduction by emergency providers.

INTRODUCTION: Forearm fractures are common pediatric injuries. Most displaced or angulated fractures can be managed via closed reduction in the operating room or in the Emergency Department (ED). Previous research has shown that emergency physicians can successfully perform closed reduction within ED; however, the fracture morphology amendable to ED physician reduction is unclear. The aim of this study is to detail the fracture characteristics associated with successful reduction by ED physicians. METHODS: We conducted a retrospective study of children (aged <18 years) presenting to the ED of

Do split paediatric forearm POP casts need to be completed? A biomechanical study.

INTRODUCTION: Displaced paediatric forearm fractures are most often treated by manipulation under anaesthetic, followed by the application of a circumferential Plaster of Paris (POP) splint. Some surgeons choose to split the cast in order to facilitate immediate "spreading" with minimal distress to the patient, should the distal limb become compromised. Usually however, this does not occur, and the cast is completed at a later visit to the plaster room. Time, money and inconvenience could be saved if this modification was not necessary, and the final plaster would be lighter. OBJECTIVE: To es

A rare case of an Essex-Lopresti injury secondary to a gunshot wound.

BACKGROUND: Radial head fractures are one of the more common fractures of the body, and the most common fractures around the elbow. CASE REPORT: While these can present in isolation, concomitant injuries to the ulna, elbow joint and surrounding ligaments, interosseous membrane, proximal radioulnar joint, and distal radioulnar joint can also occur. It is imperative that the treating surgeon rules out any concomitant injuries when evaluating a patient with a radial head fracture as the treatment options vary greatly depending on the structures which are damaged. CONCLUSIONS: Radial head resect

Incarceration of the radial head associated with a radial head fracture, radiocapitellar dislocation, and proximal radioulnar translocation.

We present a case report of an individual who sustained a locked proximal radioulnar joint fracture-translocation after a mechanical fall. After a failed attempt at closed reduction, intraoperative exploration revealed that the dislocated and fractured radial head was incarcerated along the medial aspect of the coronoid. After open reduction and internal fixation, the patient went on to uneventful healing and return of adequate function. To our knowledge, this is the first case report documenting an incarcerated fracture-translocation along the radiocapitellar and proximal radioulnar joints th

Bilateral comminuted radial shaft fractures from a single gunshot: fixation with alternative techniques.

Acute bilateral radial shaft fractures are an unusual entity that has not been previously reported in the literature. Given its bilaterality, this rare clinical entity is best treated with stable internal fixation. Here we report the case of an 18-year- old right-hand-dominant man who sustained a low-caliber gunshot injury. He had been driving with both hands on the steering wheel when he was struck by a single bullet. The bullet caused displaced fractures of the left proximal radial shaft and the right distal radial shaft. Each fracture had extension outside the mid-diaphysis. The patient und

Radial head fractures and the role of radial head prosthetic replacement: current update.

Radial head fractures are often secondary to a direct axial force, such as that involved in motor vehicle accidents and falls on an outstretched hand. The Hotchkiss-modified Mason classification is an excellent assessment tool in that it provides commonly accepted direction regarding treatment. For more unstable, comminuted displaced radial head fractures that cannot be reconstructed, replacement of the radial head is warranted. The surgeon should attempt open reduction and internal fixation with restoration of the radial head in anatomical alignment for most type II and some type III fracture

Anterior elbow dislocation with potential compartment syndrome: a case report.

Anterior elbow dislocation is an infrequent lesion, usually produced by direct trauma to the proximal ulna after a fall on the elbow in flexion, and is often associated with soft tissue injuries. The authors report a case of a complex injury produced by a high-energy trauma in the right arm of a 65-year-old patient. His limb was trapped inside an industrial spin-dryer, resulting in a closed anterior elbow dislocation, diaphyseal ulnar shaft, radial styloid process fractures, and an associated compartment syndrome. The injury mechanism and its treatment are described to better manage the soft t
