Sleepiness is not the inverse of alertness: evidence from four sleep disorder patient groups.

The constructs "sleepiness" and "alertness" are often assumed to be reciprocal states of consciousness. This distinction is of increasing concern in relation to psychomotor performance tasks such as driving. We developed two separate subjective scales of alertness to complement existing sleepiness scales. Subjective sleepiness and alertness were compared in four groups of sleep-disordered patients. In a 175-patient cohort [25 narcoleptics and 50 each with sleep apnea, insomnia and periodic leg movement disorder (PLMD)], the Epworth Sleepiness Scale (ESS) was used to measure sleepiness while th

Systematic review and guide to management of core and psychiatric symptoms in youth with autism.

OBJECTIVE: Evidence-based guidance of clinical decision-making for the management of Autism Spectrum Disorder (ASD) is lacking, particularly for co-occurring psychiatric symptoms. This review evaluates treatment evidence for six common symptom targets in children/adolescents with ASD and provides a resource to facilitate application of the evidence to clinical practice. METHOD: A systematic search identified randomized controlled trials (RCTs) and high-quality systematic reviews published between 2007 and 2016, focused on: social interaction/communication impairment, stereotypic/repetitive be

Statistical analysis of tracheal breath sounds during wakefulness for screening obstructive sleep apnea.

Obstructive sleep apnea (OSA) is a prevalent disorder. The accepted method of diagnosis in widespread clinical practice, polysomnography (PSG), is costly and very time consuming; therefore, quick screening methods, especially when there is a need for quick diagnosis, is of great interest. Diagnostic methods which exploit subtle differences in breath sounds recorded during wakefulness, such as our group's Awake-OSA technology, have shown their capability to diagnose OSA at the research stage. Simplifying the breath sound recording procedure employed in the Awake-OSA diagnostic method would incr

Resting-state activity in development and maintenance of normal brain function.

One of the most intriguing recent discoveries concerning brain function is that intrinsic neuronal activity manifests as spontaneous fluctuations of the blood oxygen level-dependent (BOLD) functional MRI signal. These BOLD fluctuations exhibit temporal synchrony within widely distributed brain regions known as resting-state networks. Resting-state networks are present in the waking state, during sleep, and under general anesthesia, suggesting that spontaneous neuronal activity plays a fundamental role in brain function. Despite its ubiquitous presence, the physiological role of correlated, spo

Daytime hypoxemia, sleep-disordered breathing, and laryngopharyngeal findings in multiple system atrophy.

BACKGROUND: The mechanism underlying nocturnal sudden death in patients with MSA remains unclear. It may be explained by upper airway obstruction, such as vocal cord abductor paralysis; an impairment of the respiratory center, such as Cheyne-Stokes respiration; or an impaired hypoxemic ventilatory response. OBJECTIVE: To investigate the mechanism of sleep-disordered breathing in multiple system atrophy (MSA). DESIGN: We recruited 21 patients with probable MSA who were admitted sequentially to our hospital, and performed daytime blood gas analysis, pulmonary function tests, polysomnography, a

Inter-Relationships Between Depressive Symptoms And Sleep Problems Among Older Adults With Depression: Findings of a National Survey in the USA.

BACKGROUND: Depressive symptoms and sleep problems are prevalent among older adults with depression. To reduce their adverse impact, it is important to understand the changes in symptom patterns as the Coronavirus disease 2019 (COVID-19) pandemic emerged. This longitudinal study examined the interactive changes between depressive symptoms and sleep problems among older adults with depression before and during the COVID-19 pandemic from a network perspective in the USA. METHODS: This network analysis study was based on data from the three waves (2016, 2018, and 2020) of the Health and Retireme

Seasonal differences in depressive affect, worry-related sleep disturbance, and low energy among UK young people.

Seasonal differences in young people's mental health may be concentrated in specific symptoms. This population-based repeated cross-sectional study examined winter-non-winter differences in depressive affect, worry-related sleep disturbance and low energy among people aged 16-24 years in the United Kingdom. Understanding Society Calendar Year Datasets were analysed for 2022-2023, with extensions covering 2020-2023. Winter was defined as an interview data in December, January or February. The primary outcome was the mean of three equally weighted standardised symptom components. Standardised Pe

Wind turbine proximity and health: Longitudinal evidence from US households.

Rapid growth of wind energy plays a key role in global efforts to reduce carbon emissions, yet public concerns persist about its potential health effects, particularly through noise exposure. While some studies and media reports suggest that wind turbines may contribute to sleep disturbances, anxiety, and even suicide, existing evidence remains limited and inconclusive. This study combines geolocated data on turbines from the US Wind Turbine Database with longitudinal survey data on over 120,000 households (2011-2023) and consumer purchasing records to assess whether proximity to wind turbines

Bilateral choroid plexus hypertrophy without CSF volume changes predicts sleep disruption in young alcohol users.

BACKGROUND AND OBJECTIVES: The choroid plexus (ChP) maintains blood-CSF barrier integrity, regulates cerebrospinal fluid (CSF) production, modulates neuro-immune signaling, and supports brain homeostasis. ChP structural abnormalities have been reported in aging, neurodegenerative disease, and several psychiatric disorders, and animal and neonatal studies suggest that alcohol can alter ChP structure and function. Whether habitual alcohol use is associated with ChP morphometry in adults, and whether any such alteration relates to sleep, is not known. We examined ChP volume, CSF volume, alcohol c

Sleep disturbance triggers aberrant activation of vagus circuitry and induces intestinal stem cell dysfunction.

Sleep disturbances are associated with pathogenesis of numerous chronic disorders, including chronic gastrointestinal diseases. However, the mechanism that transmits sleep disturbance-induced aberrant neural signaling from the brain to the gut remains elusive. We show that acute sleep deprivation (SD) impairs intestinal stem cell (ISC) function, leading to shortening of crypt-villus architecture and Paneth cell loss. We identified the dorsal motor nucleus of vagus (DMV) as the SD-sensitive central nervous system center that transmits sleep effects to the gut. SD aberrantly activates DMV neuron

Profiles and Transitions of Sleep Disturbance and Depression Among Chinese Adolescents: The Predictive Roles of Life Stress and Resilience.

Purposes: Sleep disturbance and depression co-occur frequently, yet their co-occurring and transitional nature among adolescents remains underexplored. Meanwhile, few studies have examined the potential predictive effect of environmental factors (e.g., life stress) and individual factors (e.g., resilience) on their interactive profiles and transitions. This study investigated the profiles and transitions of sleep disturbance and depression for Chinese adolescents, along with the predictive role of life stress and resilience in profiles and transitions. Methods: A total of 17,404 adolescents (M

Prenatal exposure to manganese and objectively measured sleep disturbances in early childhood.

Inadequate sleep in childhood can impact long-term neurodevelopmental outcomes; thus, identifying modifiable risk factors amenable to intervention is a priority. Development of sleep neuroarchitecture begins in utero. Manganese (Mn) is a trace element known to be both an essential element for optimal neurodevelopment and, at insufficient or excess levels, a neurotoxicant. We examined associations between prenatal urinary Mn levels and objectively measured sleep outcomes in children aged 6.7 ± 2.0 years and explored sex-specific effects. Sleep was assessed using multi-day actigraphy (n = 222) a
