Are elderly people living alone an at risk group?

OBJECTIVE: To test the hypothesis that elderly people living alone are an at risk group with a high level of morbidity that makes high demands on health and social services. DESIGN: Secondary analysis of data from a community survey of 239 people aged 75 and over, identified from general practitioners' age-sex registers. SETTING: Nine practices in the London boroughs of Brent and Islington. MAIN OUTCOME MEASURES: Scores on the mini-mental state examination; stated satisfaction with life; assessment of mobility; numbers of diagnoses of major physical problems; numbers of prescribed drugs tak

The influence of family expressed emotion on the course of schizophrenia in a sample of Spanish patients. A two-year follow-up study.

A sample of 60 Spanish schizophrenic patients was studied to ascertain the relationship between their relatives' expressed emotion (EE) and relapse at follow-up. The relatives' EE and patients' relapse were operationalised following Leff & Vaughn's criteria. At nine months a significant association was not found between the relatives' EE and relapse, but this association became significant on reclassifying the relatives' EE scores after decreasing to four points the cut-off point for critical comments. At 24 months no association was found between EE and relapse. There was a tendency for patie

Psychosocial factors, curative therapies, and behavioral outcomes. A comparison of testis cancer survivors and a control group of healthy men.

In a retrospective study of 223 testis cancer survivors and 120 controls matched sociodemographically, we examined the relative impact of sociodemographic and clinical factors on long-term outcomes in the areas of sexual function, relationships, employment, and mental outlook. For most of the survivors, testis cancer did not lead to unemployment (4.5%), divorce (6.8%), or disabling psychological problems. Multivariate analysis results confirm that cancer survivors report significantly more infertility and sexual performance distress, but not more desire distress, than the control group. Surviv

Lifelong eccentricity and social isolation. II: Asperger's syndrome or schizoid personality disorder?

Several scales are described for measuring aspects of eccentricity and social isolation; in particular, for assessing schizoid and schizotypal personality and for rating abnormal non-verbal expression. The latter is shown to be reliable, and the former to have a measure of validity. There was an association between schizoid personality traits and abnormalities of speech and non-verbal expression. However, abnormal non-verbal expression, but not schizoid personality traits or DSM-III schizotypal personality disorder, was particularly likely to occur in those subjects who had evidence of neurolo

Associated neuropsychiatric problems in Swedish school children with mild mental retardation.

A total of 83 mildly mentally retarded children aged 13-17 years were examined. These children are considered representative of all clearly mildly mentally retarded children born in 1966-1970 and living in Göteborg, Sweden. Fifty-seven per cent of the children were suffering from a handicapping psychiatric condition. Epilepsy and male sex were found to be associated with psychiatric abnormality.

Schizophrenic thought disorder at followup: its persistence and prognostic significance.

To assess the prominence, persistence, and prognostic significance of positive thought disorder in schizophrenia, we studied a large sample of psychiatric inpatients at the acute phase and then followed them up twice--1 1/2-2 years and 4 years after hospital discharge. Positive thought disorder was more prominent at the acute phase in schizophrenic and manic patients than in other types of psychotic and nonpsychotic disorders. At followup, positive thought disorder did not persist for all schizophrenics, but a significantly larger percentage of schizophrenic than other psychotic and nonpsychot

Influence of educational and sociocultural factors on hypertension care.

The influence of occupational category (OC) on the prevalence of hypertension (HT) and cardiovascular risk factors and on their management was studied in Paris in a general population and in a series of patients followed up in two university hospital hypertension clinics. Three OCs were defined as follows: high executives and members of liberal professions middle executives and clerks and manual workers. In the general population, the prevalence of HT was positively associated with OC, and ranged from 12.8% (1) to 19.3% (3). Body Mass Index (BMI) and tobacco and alcohol consumption were higher

General hospital geriatric psychiatric treatment: a follow-up study.

Fifty-two patients previously treated and discharged from an acute-care general hospital ward were followed up for an average of 30.5 months. The results show that while it is true that depressive episodes in the elderly are often easily and rapidly treated, many of these patients do poorly in the long term for a variety of reasons related to depressive dysphoria, organic brain syndrome, and physical disability. Depressive or dysphoric relapse in the community seems to go largely unrecognized. It is suggested that follow-up must be done soon after discharge to prevent the high level of rejecti

The influence of propranolol on the exposure in vivo of agoraphobics.

This study examines the influence of 40 mg of propranolol on agoraphobics throughout 5-hour periods of exposure in vivo on 3 alternate days. Twenty-three patients were studied using a double-blind parallel design and 19 followed up for 3 months. The propranolol group spent significantly less time than the placebo group travelling alone in the month after treatment, and had improved significantly less on a measure of general symptoms at 3 months. The adverse influence of propranolol on treatment outcome appeared mainly due to a waning effect in the last hour of exposure. Attempts to measure cop

Reduced serotonin synthesis and regional cerebral blood flow after anxiolytic treatment of social anxiety disorder.

Social anxiety disorder (SAD) is associated with increased fear-related neural activity in the amygdala and we recently found enhanced serotonin synthesis rate in the same region. Anxiolytic agents like selective serotonin re-uptake inhibitors (SSRIs) and neurokinin-1 receptor (NK1R) antagonists reduce amygdala activity and may attenuate serotonin formation according to animal studies. Here, we examined the effects of SSRI pharmacotherapy, NK1R antagonism, and placebo on serotonin synthesis rate in relation to neural activity, measured as regional cerebral blood flow (rCBF), and symptom improv

Oral health-related quality of life of children with craniofacial conditions.

OBJECTIVE: Evaluation of the Dutch Child Oral Health Impact Profile (COHIP), assessing the level of concordance between parents and children. The internal consistency and the predictive validity of the COHIP for self-reported general health were examined. METHODS: Sample size was 35 pairs of parents and children age 11 to 14 with craniofacial conditions. Cronbach alphas were calculated and the level of concordance between parents and children was studied using t tests and intraclass correlations. Predictive validity was assessed using Pearson correlations and linear regression analyses. RESU

Case-finding for depressive and exhausted mood in the general population: reliability and validity of a symptom-driven diagnostic scale. Results from the prospective MONICA/KORA Augsburg Study.

PURPOSE: Depressed and exhausted mood warrants epidemiological scrutiny as a cardiovascular risk factor. We evaluated the reliability and validity of a symptom-driven diagnostic scale. METHODS: To assess a depressed mood, items were derived from an extended battery of psychosocial questionnaires in a population-based study including 11,949 subjects aged 25 to 74 years. RESULTS: Eight items from the 24-item von Zerssen checklist were extracted by factor analysis resulting in an approximately normally distributed scale (range 0-24; mean=8.23, SD 4.76). Median for men was 8.00 and 10.00 for wom

The social consequences of teenage parenthood.

The adolescent mothers consistently experienced great difficulty in realizing their life plans, when compared with their classmates who did not become pregnant premaritally in their early teens. Marital instability, school disruption, economic problems, and difficulty in family size regulation and child-rearing were some of the complications brought on by their premature, unscheduled childbearing.

Schizophrenic performance on form E of Cattell's 16PF test.

Form E of Cattell's 16PF test was administered to 515 hospitalized schizophrenics. This sample was compared to Cattell's standardization population for both raw scores and sten scores. Comparisons were made between males and females, acute patients and chronic patients, and amongst three categories of schizophrenia (paranoid, undifferentiated, schizo-affectives). The results indicate that the "schizophrenic profile," reported in previous research with Form A, did not obtain expected differentiations among schizophrenic categories. The question of the usefulness of Form E in diagnosing schizoph

Neighborhood disadvantages and longitudinal progression of multimorbidity among adult survivors of childhood cancer: a single-center St. Jude Lifetime Cohort Study in the U.S.

BACKGROUND: The impact of cancer therapy on chronic health condition (CHC) burden in childhood cancer survivors is well-established; however, the role of neighborhood-level social determinants of health (SDOH) remains underexplored. METHODS: In this single-center cohort study in the U.S., we analyzed 2508 survivors enrolled in the St. Jude Lifetime Cohort Study between 9/13/2007 and 4/20/2020, who were ≥5 years from diagnosis, ≥18 years of age, and had ≥2 clinical assessments. SDOH encompassed social vulnerability (Social Vulnerability Index) and environmental/healthcare access vulnerabilitie

Understanding Behavioural Determinants of Cardiovascular Disease and Stroke Prevention Among Culturally and Linguistically Diverse Communities: A Qualitative Descriptive Study.

BACKGROUND: Cardiovascular disease (CVD) and stroke disproportionately affect culturally and linguistically diverse (CALD) communities. In Australia, CALD describes communities that differ from the dominant population by language, ethnicity and/or cultural beliefs and practices. CALD status often intersects with social disadvantage, including lower socioeconomic position, experiences of discrimination and reduced access to culturally safe health services, contributing to inequities in cardiovascular risk and outcomes. Understanding how these factors influence engagement in CVD and stroke preve
